BACKGROUND: Spinal intradural hematomas rarely occur following spinal anesthesia. In this case, a 34-year-old postpartum female developed conus-cauda paraplegia (T12/L1-L5) due to an acute subdural hematoma after spinal anesthesia.
CASE DESCRIPTION: Four-day postpartum, a 34-year-old female presented with severe paraplegia (1/5 motor power), complete T12-L1 sensory loss, and bowel incontinence. The lumbar magnetic resonance imaging (MRI) revealed a subacute subdural hematoma extending from the L1 to L5 levels. An urgent L1-L5 laminectomy was performed for clot evacuation. Within 3 postoperati

Epidural blockade is an important option for anesthesia in parturients undergoing abdominal delivery. Despite the multiple benefits of this method, there is at least one significant downside--a relatively high occurrence of unsatisfactory anesthesia that requires intervention. Depending on the presumed mechanism of epidural block failure and other clinically relevant factors (e.g., timing of diagnosis, urgency of the procedure, and so forth), certain effective measures are recommended to successfully manage this demanding situation. In general, it is important to make every effort to make the

Achondroplasia is the most common bone dysplasia. Many patients have concomitant challenging neuraxial and airway anatomies. We describe a case of a G1P0 who presented for cesarean delivery due to achondroplasia, with comorbidities of Class III obesity and previous spinal fusion of unknown level. No preoperative anesthesia consultation had been requested, nor was previous neuraxial imaging available. After several failed spinal attempts with a 25-G Whitacre needle, a 22-G spinal needle was used with success. This case catalyzed our development of a clinical decision tool to algorithmically tro

Aneurysmal subarachnoid hemorrhage (aSAH) is a rare but devastating complication with increased morbidity and mortality. It is still unclear whether the incidence is increased during pregnancy and in the peripartum period. However, the incidence of cerebral aneurysmal rupture is higher during the third trimester than in the first trimester. The risk of aneurysmal rupture and subarachnoid hemorrhage (SAH) during general anesthesia or spinal anesthesia is unclear. We report a case of left supraclinoid aneurysm rupture after spinal anesthesia for Low Segment Caesarean Section (LSCS) in the immedi

This report describes a case of cauda equina syndrome possibly caused by arachnoiditis due to levobupivacaine after spinal-epidural anesthesia. A 39-yr-old woman delivered by cesarean section under a combined spinal-epidural anesthesia. After an uneventful procedure and surgery, she complained of weakness in her lower extremities, which increased in a few hours. Neurologic examination revealed severe weakness in both her lower extremities, perianal anesthesia, and absence of muscle stretch reflexes. She was unable to urinate. Magnetic resonance imaging performed immediately revealed entirely n

A 41-year-old woman with single atrium and single ventricle at 6 weeks of gestation was scheduled for dilation and evacuation of the fetus. The PaO2 was 39 mmHg, while she was breathing room air. Dilatation of the uterine cervical canal was performed under spinal anesthesia using 2.0 ml of 0.5% hyperbaric bupivacaine one day before dilatation and evacuation of the fetus. A sensory anesthesia level of T 10-S 5 was achieved. The systolic blood pressure decreased to around 70 mmHg. On the next day, dilation and evacuation of the fetus was performed under spinal anesthesia using 1.5 ml of 0.5% iso

This paper is an overview of various features of regional anesthesia (RA) and aims to introduce spine surgeons unfamiliar with RA. RA is commonly used for procedures that involve the lower extremities, perineum, pelvic girdle, or lower abdomen. However, general anesthesia (GA) is preferred and most commonly used for lumbar spine surgery. Spinal anesthesia (SA) and epidural anesthesia (EA) are the most commonly used RA methods, and a combined method of SA and EA (CSE). Compared to GA, RA offers numerous benefits including reduced intraoperative blood loss, arterial and venous thrombosis, pulmon

From March 1, 1990 to August 31, 1991, there were four hundred and forty eight female patients undergoing major lower abdominal obstetric and gynecologic operations in our hospital. The most frequently performed operation in our series was Cesarean section (80.8%). Besides, there were operations for ectopic pregnancy, ovarian cyst, fallopian tube problems and abdominal total hysterectomy as well as radical hysterectomy for cervical cancer. Most of the operations were done under spinal anesthesia (91.3%). The rest were performed under epidural anesthesia (2.2%), general anesthesia (4.9%) or a c

Introduction: Hysterectomies are one of the most common major open gynecologic surgeries. Open gynecological surgery results in a large wound and severe postoperative pain, and adequate postoperative analgesia is necessary. As part of multimodal analgesia strategies, regional anesthesia techniques are widely used in such surgeries to reduce opioid consumption and enhance analgesic efficacy. Spinal anesthesia, as a regional anesthesia technique, and Rectus Sheath Block (RSB) can provide adequate anesthesia and are explored in this prospective randomized study. Objective: The aim of this study w

The aim of this study was to examine whether thioxanthine-type antipsychotics (chlorprothixene and cis(z)-flupenthixol) and phenothiazine-type antipsychotics (chlorpromazine and fluphenazine) produced spinal anesthesia. Using a rat model of intrathecal injection, we evaluated spinal anesthesia of antipsychotic drugs (chlorprothixene, cis(z)-flupenthixol, chlorpromazine, and fluphenazine) and bupivacaine, a known local anesthetic. At a same dose of 2.31 μmol/kg, chlorprothixene had the most potent spinal blockades (P<0.001) and the longest duration of action (P<0.001) of motor function and noci

Combined spinal anesthesia and postoperative epidural analgesia is widely used in orthopedic surgery. Uncommon but serious neurologic complications of neuraxial anesthesia (NA) include direct trauma during needle or catheter insertion, central nervous system infections, and neurotoxicity of local anesthetics. Cauda equina syndrome (CES) is a rare complication after NA but can result in severe neurologic deterioration that may require surgical intervention. We present a case of a 69-year-old woman with postpolio syndrome who developed CES after combined spinal anesthesia and postoperative epidu

INTRODUCTION: The primary goal of spinal anesthesia in lower limb surgeries is to achieve a successful sensory and motor block. Adequate level of spinal block for lower limb orthopedic surgery is T10. Due to multiple factors affecting the level of spinal anesthesia, it is not always easy to control the level of spinal anesthesia. We proposed that maintaining patients in a 10° reverse Trendelenburg position after spinal anesthesia can significantly control the height of the sensory block, resulting in stable hemodynamics.
MATERIALS AND METHODS: This study is a single centric, prospective, singl

OBJECTIVE: Spinal anesthesia with local anesthetics is a viable alternative to general anesthesia in orthopedic surgery, and it is currently considered the standard of care for knee arthroscopy. The use of chloroprocaine may offer several potential advantages over other local anesthetics, including, above all, its rapid onset and short duration of action. The aim of the present retrospective study is to evaluate the post-surgical outcomes of patients who underwent knee arthroscopy using spinal anesthesia with chloroprocaine in an outpatient orthopedic setting.

BACKGROUND: We aimed to compare the hemodynamic response to two norepinephrine infusion rates for prophylaxis against post-spinal hypotension in elderly population undergoing hip surgery.
METHODS: This randomized non-inferiority clinical trial included elderly patients scheduled for hip surgery under spinal anesthesia. Patients received norepinephrine infusion at a rate of 0.1 mcg/kg/min (0.1-mcg group) or 0.07 mcg/kg/min (0.07-mcg group) after spinal anesthesia. Intraoperative mean arterial pressure (MAP) and heart rate were recorded every 2 min for 46 min after spinal anesthesia. The primar

After spinal anesthesia, early ambulation frequently caused spinal headache. If the patient was discharged on the day of the operation, spinal headache occurred very frequently. With epidural anesthesia, if the dura was erroneously punctured with the needle, headache often occurred. In order to prevent spinal headache, we tried the Blood Patch Method during operation (Prophylactic Blood Patch Method). Epidural catheterization and spinal tap were simultaneously performed. The epidural catheter was threaded 3 cm cephalad in the epidural space. Five ml Venous blood of the patient was injected thr
