Stroke in Ehlers-Danlos Syndrome Kyphoscoliotic Type: Dissection or Vasculitis?

BACKGROUND: Patients with the kyphoscoliotic type of Ehlers-Danlos syndrome have an increased risk of vascular complications such as aortic dissection and perforation. Cerebral ischemia has only rarely been documented. PATIENT DESCRIPTION: This 13-year-old girl with the kyphoscoliotic type of Ehlers-Danlos syndrome experienced a large right middle cerebral artery distribution infarction. Full intravenous heparinization was started in response to presumed arterial dissection. Magnetic resonance imaging studies including magnetic resonance angiography and digital subtraction angiography, howeve

Endovascular management of a carotid aneurysm into the sphenoid sinus presenting with epistaxis.

Non-traumatic cavernous internal carotid artery (ICA) aneurysms are rare, and favour the occurrence of massive recurrent epistaxis, which is associated with a high mortality rate. We report the case of a 67-year-old woman presenting a ruptured ICA aneurysm extending into the sphenoid sinus, revealed by epistaxis. Selective coil embolization of the aneurysm was performed. Flow-diverter stents were deployed in order to utterly exclude the aneurysm and prevent revascularization. Anti-platelet treatment was provided to lower the risk of in-stent thrombosis. A left frontal hematoma associated with

Contrast Extravasation on Computed Tomography Angiography Imitating a Basilar Artery Trunk Aneurysm in Subsequent Conventional Angiogram-Negative Subarachnoid Hemorrhage: Report of Two Cases with Different Clinical Courses.

Contrast extravasation on computed tomography angiography (CTA) is rare but becoming more common, with increasing use of CTA for various cerebral vascular diseases. We report on two cases of spontaneous subarachnoid hemorrhage (SAH) in which the CTA showed an upper basilar trunk saccular lesion suggesting ruptured aneurysm. However, immediate subsequent digital subtraction angiography (DSA) failed to show a vascular lesion. In one case, repeated follow up DSA was also negative. The patient was treated conservatively and discharged without any neurologic deficit. In the other case, the patient

Endovascular Treatment of a Ruptured Posterior Inferior Cerebellar Artery Aneurysm during Pregnancy.

Aneurysmal subarachnoid hemorrhage (SAH) during pregnancy is quite rare, however it has a high maternal mortality rate. A pregnant woman in the 16th gestational week was admitted to our hospital with a drowsy level of consciousness. A brain magnetic resonance (MR) image showed hemorrhage on the prepontine cistern, and both sylvian fissures, and MR angiography and cerebral digital subtraction angiography demonstrated an aneurysm at the left posterior inferior cerebellar artery (PICA). We performed endovascular coil embolization attempting to minimize radiation exposure. She was discharged with

[Abdominal pain with a microvascular cause].

A 50-year-old Caucasian man was admitted with unexplained abdominal pain. Based on CT scan results, a perforation resulting from colitis of the transverse colon was suspected. Additional colonoscopy showed ulcerative stenosis of the transverse colon. Initially, the patient recovered on conservative treatment; however, because of persistent stenosis, an elective resection of the transverse colon was performed. A CT scan revealed free gas around the anastomosis on the fourth postoperative day. Relaparotomy revealed leakage and diffuse ischaemic changes around the anastomosis as well as perforati

New technologies for the management of major extremity wounds.

Soft tissue reconstruction for large or difficult defects, particularly of distal extremities, may be associated with a variety of problems. Crush injuries and infection are often background settings requiring use of considerable resources, time, and skilled personnel for appropriate treatment. Harvested tissue, known as a flap, is used for transferring healthy, viable tissue from one location to the area of tissue loss. Conventional techniques, procedures, and imaging studies used in soft tissue wounds are associated with limitations that often mandate the use of microvascular tissue transfer

[Neuroimaging evaluation of intracerebral hemorrhage].

Spontaneous intracerebral hemorrhage (ICH) is responsible for 10-15% of all strokes. Standard evaluation includes a CT examination, in which ICH is initially hyperdense but gradually decreases in density over days to weeks. In emergency situations, native CT can be supplemented with CT angiography, which reliably shows aneurysms and other vascular abnormalities larger than 2-3 mm. For detecting ICH in hyperacute situations, MRI is as sensitive as CT but signal characteristics are more complex, strongly depending on the oxygenation status of hemoglobin and the redox status of iron. In terms of

[Imaging of subarachnoid hemorrhage].

Even if acute subarachnoid hemorrhage (SAH) accounts for only 5% of strokes, its diagnosis is very important because its clinical consequences can be tragic. Recent technological advances in medical imaging have improved diagnostic and therapeutic management of patients with SAH. Nonenhanced CT of the head is the initial imaging modality in suspected SAH for the detection of ruptured intracranial aneurysms. Digital subtraction angiography (DSA) remains the reference exam. Multidetector row CT angiography may potentially replace DSA in the emergency setting, as it provides image data that allow

Evaluation of renovascular hypertension: comparison of functional MRI and contrast-enhanced MRA with a routinely performed renal scintigraphy and DSA.

OBJECTIVE: The main objective of this study was to evaluate a clinically suspected renovascular hypertension (RVH) by means of MRI. It was to find out if functional MRI (fMRI) is able to provide adequate results with regard to MAG3 captopril scintigraphy and if contrast-enhanced MR angiography (MRA) is able to provide adequate results in the stenosis grading compared with the nonselective digital subtraction angiography (DSA). METHODS: This open, monocentric, prospective, phase 3 study included patients with a clinically suspected RVH. For fMRI a dynamic TurboFLASH sequence and for MRA a sing

[A case of medial medullary infarction with persistent primitive hypoglossal artery].

A 66-year-old woman was admitted to our hospital because of vomiting, dizziness and vertigo. Neurological examination on admission revealed only upbeat nystagmus without cranial nerve symptoms, paresis, cerebellar signs or sensory disturbances. Magnetic resonance(MR) images demonstrated a new T 2 high intensity and T 1 iso-intensity signal lesion in the right upper medial medulla. This medial medullary infarction caused central vestibular dysfunction. MR angiography and digital subtraction angiography demonstrated a persistent primitive hypoglossal artery (PPHA) originating from the right inte

[A case of recanalized basilar artery following basilar artery occlusion due to spontaneous dissection].

An 85-year-old man who did not have any previous history of heart disease was admitted to our hospital with the chief complaints of disturbance of consciousness, tetraparesis, and bilateral Babinski signs. On admission, intravenous digital subtraction angiography (DSA) showed bilatral vertebral arteries without filling the basilar artery. On the second hospital day, MRI showed a septum in the basilar artery, and no infarctions were noted in the brain stem. He gradually improved with heparin therapy. On the tenth hospital day, intravenous DSA disclosed filling of the basilar artery. On the sixt

[Pulmonary sling in a patient with anomalous intrathoracic heart displacement (diagnosis and surgical treatment)].

The pulmonary sling is the origin of the left pulmonary artery from the right one, this is a rare abnormality giving a marked clinical picture. The paper presents a case of solely rare combination of the pulmonary of the pulmonary sling with abnormal cardiac placement--dextroversion. A three-year-old child underwent explorative thoracotomy for erroneously diagnosed mediastinal cyst. The thoracotomy revealed a suspected pulmonary sling. To make an accurate diagnosis, a comprehensive study (chest X-ray with esophageal contrasting, tracheobronchography, echocardiography, digital subtraction angio

Mapping the distribution of amobarbital sodium in the intracarotid Wada test by use of Tc-99m HMPAO with SPECT.

The intracarotid amobarbital sodium, or Wada, test has been used to localize speech and memory function prior to surgical treatment of temporal lobe seizures. The authors mixed technetium-99m hexamethyl-propyleneamine oxime (HMPAO) with amobarbital sodium and injected the mixture in 25 patients with epilepsy. Single photon emission computed tomography (SPECT) of the brain was then performed to determine intracerebral distribution of the amobarbital sodium. Results of SPECT were compared with those of conventional and digital subtraction angiography (DSA). The distribution of Tc-99m HMPAO and,

Randomized Controlled Trial of Patient Positioning and Operator Radiation Exposure During Lower Extremity Catheter Angiography.

Digital subtraction angiography (DSA) remains an important reference modality for evaluating chronic limb-threatening ischemia (CLTI), with left transradial access (TRA) increasingly favored for its lower complication rates and patient comfort. Radiation safety for operators is paramount, yet the impact of patient positioning on scatter radiation during lower limb diagnostic catheter angiography (CA) is understudied. This single-center randomized controlled trial evaluated whether head-first (HF) vs. feet-first (FF) supine patient orientation affects operator radiation exposure during lower ex

Utility of cone-beam computed tomography angiography for the assessment of vertebral artery dissection.

Vertebral artery dissection (VAD) has been recognized as a cause of headache and stroke. Accurate evaluation of dissection using several modalities such as catheter-based angiography, CT angiography (CTA), and magnetic resonance imaging (MRI) is essential for subsequent management. The aim of this retrospective study is to compare cone-beam computed tomography angiography (CBCT-A) with other image modalities for the evaluation of the detailed structures of VAD. Twenty-five consecutive cases identified as having VAD were included. They underwent catheter-based angiography (2D-digital subtractio

Diagnostic sensitivity and specificity of CT angiography for renal artery stenosis in children.

BACKGROUND: Children with suspected renal artery stenosis (RAS) are screened with renal Doppler ultrasonography or computed tomography (CT) angiography/magnetic resonance (MR) angiography depending on institutional preference. CT angiography produces images with superior resolution, allowing higher quality multiplanar two-dimensional reformats and three-dimensional reconstructions. However, there is a paucity of data in the literature regarding the utility and diagnostic performance of renal CT angiography in pediatric RAS. OBJECTIVE: The objective of this study is to retrospectively review o

Evaluating expert decision-pattern alignment in endovascular planning for intracranial aneurysms using a multimodal large language model.

BACKGROUND: Endovascular treatment planning for intracranial aneurysms requires integrating vascular geometry, branch preservation, device feasibility, and patient-specific risk tolerance. Because multiple strategies may be clinically acceptable for the same anatomy, treatment selection reflects expert heuristics rather than a single deterministic solution. Whether multimodal large language models (LLMs) can reproduce such expert decision patterns from angiographic inputs remains incompletely characterized. METHODS: In this retrospective single-center study, 59 patients with unruptured intrac
