Epikeratophakia lenses may be manufactured from donor cornea either with a lathe or using devices that make flat cuts on to previously deformed cornea. A refractive keratoplasty technique has been devised where the donor lenticule is placed between Bowman's membrane and the epithelial layer. We have termed this operation "intraepikeratophakia." The technique is based on the same principles as keratophakia. The epithelium is removed surgically and reapproximated over the transplanted lenticule. Twenty-two patients underwent conventional epikeratophakia, and ten underwent intraepikeratophakia. Of the 32 patients in this study, three required removal of the donor lenticule. We investigated the effect of diffuse hemorrhage on Snellen visual acuity in a model of vitreous hemorrhage in phakic and aphakic eyes. Two cylindrical test chambers with optical paths of 17 mm and 23 mm were constructed to simulate the effect of vitreous hemorrhage in phakic and aphakic eyes. The visual acuity was measured in two subjects in a double-masked fashion for 17 dilutions of whole blood between 1/100 and 1/12,800. To fill the vitreous cavity completely with silicone oil has been difficult in aphakic eyes. This procedure has been simplified with the use of an air pump system combined with a 30-gauge needle to tap the anterior chamber. In this manner, the silicone oil bubble reaches the pupillary margin and does not flow out of the sclerotomy sites.

A concentric variation in sagittal power, Fs, with zonal radius, y, has been used in some designs of varifocal and aphakic spectacle lenses. It is shown theoretically, using a paraxial approximation, that a gradient dFs/dy in sagittal power leads to the introduction of an amount y.dFs/dy of unwanted astigmatism. The validity of this result is illustrated experimentally.

A patient had both cataracts removed and a Copeland pseudophakos was implanted in one eye. Both globes were retained at autopsy for histopathologic study. The globe containing the implant did not show evidence of acute or chronic inflammation of the anterior segment, nor did it display signs suggesting secondary glaucoma. Some erosion of the iris pigment epithelium was present. In general, the globe appeared to tolerate the implant.
