Radiofrequency current ablation of atrioventricular accessory pathway.

Catheter ablation of atrioventricular accessory pathway by radiofrequency current is a new treatment for patients with refractory supraventricular tachycardia. 71 patients (37 male, 34 female, mean age 36 +/- 11 years) with 77 atrioventricular accessory pathways were ablated in the study. Left-sided pathways were ablated at the ventricular side of mitral annulus by using large-tip ablation catheter. While right-sided pathways were ablated along the tricuspid annulus, 70 (90.9%) accessory pathways of 64 patients (90.1%) were blocked successfully. There were no deaths. Left femoral vein thrombos

Effects of cardiac denervation on atrioventricular nodal accommodation and hysteresis.

Conduction through the atrioventricular (AV) node is significantly altered by changes in autonomic tone. AV nodal accommodation (time-dependent changes in AV nodal conduction after a sudden change in atrial cycle length) and AV nodal hysteresis (asymmetry of AV nodal accommodation after directionally opposite atrial pacing cycle length changes) have been characterized in humans. Studies in dogs after cardiac transplantation suggest that the rapid phase of AV nodal accommodation is altered following ablation of neural input to the AV node. To determine if cardiac denervation alters AV nodal acc

Nonpharmacological therapy of supraventricular arrhythmias: surgery and catheter ablation techniques.

Treatment of supraventricular tachyarrhythmias has undergone substantial change and development over the past decade. Pharmacological approaches to management of supraventricular arrhythmias have been supplanted by nonpharmacological techniques. These nonpharmacological techniques are the subject of the present review. The first part of this review will concentrate on the nonpharmacological techniques available for the treatment of supraventricular tachycardia, excluding Wolff-Parkinson-White syndrome, whereas Part 2 will concentrate on the surgical and the ablative procedures for management o

[Hemodynamic benefits of AV interval adjustment and heart rate increase during exercise in dual-chamber pacing as determined by Doppler].

In order to determine the relative significance of ventricular rate increase and AV delay on exercise cardiac output, we have studied 10 patients (8 male and 2 female, 16-59 years) with complete chronic heart block treated with AV sequential pacing. Cardiac output variations (delta CO) were estimated by pulsed Doppler comparisons of the aortic flow velocity in the supine position, at rest and during bicycle exercise. The following pacing programs were tested: DDD with AV intervals of 50, 100 and 150 ms (DDD50 o DDDD100, DDD150), VVI at 70 ppm (VVI70), and VVI at the maximal available rate in t

Contribution of atrioventricular synchrony to left ventricular systolic function in a closed-chest canine model of complete heart block: implications for single-chamber rate-variable cardiac pacing.

This study assessed the impact of atrioventricular (AV) synchrony on characteristics of left ventricular (LV) systolic function during ventricular pacing over a wide heart rate range in a conscious closed-chest canine model of complete AV block. Ten healthy adult dogs underwent thoracotomy during which complete AV block was created by formaldehyde injection, and paired ultrasonic sonomicrometers were positioned on the LV anterior-posterior minor axis. Following recovery from surgery, peak and end-diastolic LV transmural pressure, maximum dP/dt, stroke work, end-diastolic minor axis dimension,

Dopamine-induced increase in atrioventricular conduction in atrial fibrillation-flutter.

We report a patient with controlled atrial fibrillation-flutter (AFF) and mitral stenosis who developed a marked increase in atrioventricular conduction during an infusion of low dose dopamine with development of hemodynamic compromise. This side effect has not been previously described in humans. Dopamine infusion should be used with caution in patients with AFF.

Reverse alternating Wenckebach periodicity.

An atrial pacing-induced reverse conduction pattern of the alternating Wenckebach periodicity was observed in 5 of 42 children (12%) during electrophysiologic study. This conduction pattern is a reverse of the usual alternating Wenckebach periodicity: During an underlying 2:1 atrioventricular conduction block there is progressive shortening of the conduction time of the conducted impulses with termination in a lower degree of block. This reverse alternating Wenckebach periodicity may be caused by a mechanism similar to that in other Wenckebach phenomena.

Atrial pacing bigeminy: a manifestation of crosstalk.

Atrial pacing bigeminy, defined as atrial pacing with alternating short and long cycle lengths, was recorded in two out of five DDD-pacemaker implants. Another fascinating feature, the lowering of the effective pacing rate after programming to a higher rate, occurred in one patient. Both phenomena can be explained by crosstalk; appropriate programming resulted in normal DDD-function in all cases. These observations in our patients have led to some interesting conclusions regarding multiprogrammability and nominal settings.

Observations on spontaneous termination of atrioventricular nodal reentrant tachycardia.

Unusual mechanisms of spontaneous termination of atrioventricular (A-V) nodal reentrant tachycardia were observed in two patients during programmed electrical stimulation of the heart. In both patients the mechanism of termination was based on the use of another reentrant pathway than the use used during tachycardia. This pathway was located extranodally in one patient and intranodally in the other. The observations illustrate some of the complexities of reentry in the human heart and how they can play a role in spontaneous termination of A-V nodal tachycardia.

Multiple mechanisms of tachycardias in a patient with the Wolff-Parkinson-White syndrome.

In a patient with the Wolff-Parkinson-White Syndrome we observed atrial fibrillation and three distinct paroxysmal re-entrant tachycardias. Intracardiac electrograms obtained during the tachycardias showed the mechanisms to be A-V nodal, accessory pathway and sinus node re-entry. When P wave morphology, R-P relationship and QRS configuration are considered, it is illustrated how these four tachyarrhythmias may be successfully diagnosed on the surface electrocardiogram. The therapeutic implications of multiple arrhythmias with different mechanisms in the Wolff-Parkinson-White Syndrome are discu

Characteristics of atrioventricular conduction and the spectrum of arrhythmias in lown-ganong-levine syndrome.

Electrophysiological characteristics of atrioventricular (A-V) conduction and refractoriness were examined in 12 patients with Lown-Ganong-Levine (LGL) syndrome referred for assessment of the following arrhythmias: (group I) regular narrow QRS tachycardia 6/12 (50%), (group II) atrial fibrillation (AF) 2/12 (17%), group III) ventricular tachycardia (VT) 4/12 (33%). A-V node refractory periods were shorter, and enhanced A-V conduction more frequent (7/12, 58% vs 7/28, 25%) in LGL patients compared to similar studies in 28 normal controls. During laboratory study reciprocating tachycardia (RT) d

[2 or 3 level blocks in the Tawara node during atrial tachycardia].

In atrial flutter (or paroxysmal atrial tachycardia), the ventricular response is dependant on the passage through 3 superposed zones of conduction in the Tawara node, the zone of decremential conduction being the central zone N. When the ventricular response is between half and a quarter of the atrial rate there are two possible explanations: type B alternate Wenckebach period (mobitz I block in the central zone N, 2/1 block at the nodo-ventricular junction) or type A alternate Wenckebach period (Mobitz I block in the central zone N and 2/1 block at the atrio-nodal junction). These two respon
