Transplantation of cerebral actinomycosis simulating malignant brain tumor into nude mice. Report of a case.

A case of actinomycotic brain abscess simulating malignant brain tumor is presented. Diagnosis was made by histological examination of the operative specimen and of the organs of xenotransplanted Nude mice. The patient survived 3 months after the operation. None of the characteristic organisms could be detected microscopically at the autopsy in the brain and the lung. Xenotransplantation into Nude mice may be a useful method for the relatively quick identification of the causative agent in cases suspected of actinomycosis.

Giant vertebral artery aneurysm in a child treated with endovascular parent artery occlusion and coil embolization.

BACKGROUND: Involvement of the central nervous system (CNS) by a tuberculosis abscess is a rare form of extra-pulmonary tuberculosis. With proper treatment, the abscess most commonly follows a pattern of continued reduction in size. CASE DESCRIPTION: A 71-year-old male with a past medical history of kidney transplant on immunosuppressive therapy, presented to the hospital with a 1-day history of headache. On physical examination, the patient had no focal neurological symptoms. Initial laboratory reports were unremarkable. Contrast enhanced magnetic resonance imaging (MRI) was performed, which

Klebsiella pneumoniae invasive syndrome with liver abscess and purulent meningitis presenting as acute hemiplegia: a case report.

BACKGROUND: Klebsiella pneumoniae can infect a variety of sites, with the risk of infection being higher in the immunocompromised state such as diabetes mellitus. A distinct invasive syndrome has been detected mostly in Southeast Asia in the past two decades. A common destructive complication is pyogenic liver abscess that can be complicated by metastatic endophthalmitis as well as the involvement of the central nervous system, causing purulent meningitis or brain abscess. CASE PRESENTATION: We report a rare case of an invasive liver abscess caused by K. pneumoniae, with metastatic infections…

A Pott's Puffy Tumor Associated with Epidural - Cutaneous Fistula and Epidural Abscess: Case Report.

BACKGROUND: Pott's puffy tumour is characterized by a fluctuate swelling of the frontal region as a result of osteomyelitis of the frontal bone. This inflammatory lesion may propagate endocranially, resulting in acute meningitis, epidural abscess, subdural empyema, cavernous sinus trombophlebitis, cerebritis, and frontal lobe abscess of the brain. CASE REPORT: We present an unusual case of a 33-year-old man suffering from Pott's puffy tumour whose condition was further complicated by a draining epidural-cutaneous fistula and an epidural abscess. We confirmed the diagnosis by contrast-enhanced

Nocardiosis Unmasked: Therapeutic Challenges of Multiple Abscesses in an Immunocompromised Patient.

Nocardiosis has a predilection for affecting the lungs or occurs as a disseminated disease. Cerebral involvement resulting in multiple brain abscesses is rarely reported in 2% of cases. A 30-year-old woman diagnosed with immunocompromised state six months back presented with headache, multiple episodes of seizures, and progressive right hemiparesis for the past three months. She was initially treated with antibiotics, but in view of non-improvement, an early burr-hole aspiration of the abscess resulted in the correct diagnosis. She was then subsequently treated with antimicrobial therapy and s…

Cerebellar abscess secondary to metastatic lung adenocarcinoma: a case report.

BACKGROUND: Cerebellar abscesses are rare, life-threatening infections often originating from bacterial sources, while metastatic brain lesions from lung adenocarcinoma are relatively common. However, the coexistence of a cerebellar abscess secondary to metastatic lung adenocarcinoma is exceedingly rare and presents unique diagnostic and management challenges. CASE PRESENTATION: We report a case of a 35 year-old Pakistani female patient with persistent headaches, nausea, and vertigo, who was found to have a large cerebellar mass with features suggestive of metastatic lung adenocarcinoma. The Clinicoradiological Profile of Ring-enhancing Lesions in the Brain among Children 1 Month-18 Years of Age in a Tertiary Care Hospital.

INTRODUCTION: Ring-enhancing lesions (RELs) in the pediatric brain represent a complex and heterogeneous group of pathological conditions that pose significant diagnostic and therapeutic challenges for clinicians and radiologists. Neuroimaging techniques, particularly magnetic resonance imaging (MRI) with contrast enhancement, play a pivotal role in characterizing these lesions. However, radiological findings alone are often insufficient, necessitating comprehensive clinical correlation and, in some cases, histopathological confirmation. This research aimed to provide a comprehensive analysis

Parietal lobe abscess after routine periodontal recall therapy. Report of a case.

This paper reports on a case of a 70-year-old physician diagnosed with a parietal lobe abscess following such treatment. After stereotactic biopsy and drainage and a 6-week course of intravenous antibiotic treatment, the patient recovered with minimal neurologic deficits. Although brain abscesses are not commonly encountered in practice, clinicians must be aware of the potential virulence of the anaerobic components of the periodontal pocket and the possibility of resulting systemic infection, which can produce a life-threatening situation.

Pyelonephritis associated with Legionella pneumophila, serogroup 4.

A patient with bladder carcinoma metastatic to lung and brain died of pneumonia. Autopsy examination showed a confluent bronchopneumonia with several small abscesses and acute pyelonephritis with abscess formation. Legionella pneumophila, serogroup 4, was isolated in pure culture from lung tissue postmortem and was also shown by direct immunofluorescence in the kidney and spleen. In the kidney, the organism was noted in areas of acute pyelonephritis. This represents the first case of an extrathoracic inflammatory lesion associated with L. pneumophila.

[Bronchial carcinoma and actinomycosis: a dangerous trap].

A 60 year old male patient was admitted to hospital with pulmonary and cerebral abscesses. A percutaneous lung biopsy under CT scanning showed actinomycosis. After 4 weeks antibiotic therapy with ceftriaxone and metronidazole there was an improvement in the pulmonary lesion but new cerebral lesions appeared. A neurosurgical cerebral biopsy showed evidence of metastatic squamous carcinoma, probably of pulmonary origin. The diagnosis had been delayed by the presence of the actinomycosis. His general condition did not permit anti-tumour treatment and the patient soon afterwards. In the presence o

Case Report: a case of prolonged post-traumatic duodenal obstruction complicated by traumatic pancreatitis and Wernicke's encephalopathy: a multidisciplinary challenge.

BACKGROUND: Post-traumatic gastrointestinal injuries, especially those involving the duodenum and proximal jejunum, present significant management challenges. Complications such as retroperitoneal abscess, high intestinal obstruction, and secondary pancreatitis can lead to prolonged fasting and severe nutritional deficiencies. We report a complex case where a patient developed a rare triad of post-operative combined abscess (retroperitoneal abscess and intra-abdominal abscess)-induced high intestinal obstruction, traumatic pancreatitis, and subsequent Wernicke's encephalopathy (WE), highlighti

Head injury unmasking other brain diseases.

Sixteen patients previously free of neurological complaints sustained minor head injuries and subsequently presented acutely with a wide range of unexpected, serious brain diseases. These included brain tumours, berry aneurysms, arteriovenous malformations and brain abscess. The possible, responsible mechanisms include direct mechanical trauma to the asymptomatic brain lesion, hydrocephalus and brain oedema. This unusual complication of head injury should be suspected when a florid neurological syndrome follows a minor head injury. CAT scanning usually identifies the responsible disease. Becau…

18 F FDG and 68 Ga FAPI PET/CT Findings in a Pituitary Abscess Secondary to Tuberculous Meningitis.

Pituitary abscess is a rare, life-threatening condition of the sella, accounting for less than 1% of all pituitary lesions. We present the first case documenting 68 Ga FAPI uptake in a pituitary abscess, in conjunction with 18 F FDG PET/CT. While both tracers localized the lesion, 68 Ga FAPI PET/CT demonstrated intense avidity against a negligible physiological background in the brain, thereby enhancing the contrast and detectability of this often-minuscule lesion. This superior tumor-to-background contrast highlights the potential of 68 Ga FAPI PET/CT for the high-contrast visualization of in

Epileptic seizure: an atypical presentation in an adolescent boy with neurobrucellosis.

Brucellosis is an infectious disease with multisystemic involvement caused by the genus Brucella. Neurological complications, including meningitis, meningoencephalitis, myelitis-radiculoneuritis, brain abscess, epidural abscess and meningovascular syndromes, are rarely encountered. We present a patient with epileptic seizures and aggressive mood due to chronic neurobrucellosis of 2.5 y duration, which was misdiagnosed as bacterial meningitis and epilepsy. This form of presentation has not previously been reported in the English language literature. We conclude that the diagnosis of neurobrucel…

A case of central nervous system infection due to Cladophialophora bantiana.

BACKGROUND: Cladophialophora bantiana is a melanised mold with a pronounced tropism for the central nervous system, almost exclusively causing human brain abscesses. CASE REPORT: We describe a case of cerebral infection by this fungus in an otherwise healthy 28-year-old coal-miner. Environmental occurrence, route of entry, and incubation period of this fungus are unknown, but our case is informative in that the first symptoms occurred about eight weeks after known traumatic inoculation. Lesions were compatible with tuberculous granulomas, and the patient initially received antitubercular trea…
