Multivessel off-pump coronary artery bypass grafting in a nonagenarian: anesthesiologic management.

A 90-year-old male admitted with history of angina (three-vessel disease) on medical therapy for hypertension and chronic renal failure was scheduled for elective coronary artery bypass grafting (CABG). After standard premedication and monitoring anesthesia was induced with propofol and maintained with isoflurane. Middle dose opioids and atracurium were also given. Multivessel revascularization was done through median sternotomy and anastomoses were performed with the aid of coronary stabilization and shunting. Cerebral and renal perfusion were maintained with high arterial pressure (140/70 mm

Early outcomes of patients with chronic kidney disease after revascularization for critical limb ischemia.

BACKGROUND: The aim of this study was to report early outcomes of patients with non-dialysis-dependent chronic kidney disease (NDD-CKD) after revascularization for critical limb-threatening ischemia (CLTI). METHODS: Perioperative data of patients from the CRITISCH (critical limb ischemia) Registry, who also had NDD-CKD (stages 3 and 4), were compared to their counterparts with normal renal function (NRF) or mild renal insufficiency (stages 1 and 2). Patient characteristics and type of first-line treatment were assessed. Amputation-free survival was the primary composite endpoint. Secondary en

Prasugrel Outperforms Ticagrelor in Acute Coronary Syndrome: A Real-World Analysis.

Dual antiplatelet therapy (DAPT) with aspirin and a P2Y12 inhibitor is standard for patients with acute coronary syndrome (ACS) undergoing percutaneous coronary intervention (PCI), yet real-world comparisons of ticagrelor and prasugrel remain limited. We evaluated their relative effectiveness and safety using the TriNetX Global Collaborative Network (2010-2024). Adult ACS patients receiving prasugrel or ticagrelor within 24 h post-PCI were included. The primary endpoint was a 1-year composite of all-cause mortality, myocardial infarction (MI), and cerebral infarction; secondary endpoints inclu

Outcomes of Bypass Surgery in Patients with Moyamoya Syndrome Secondary To Sickle Cell Disease: a Multicenter Study.

BACKGROUND: Moyamoya syndrome (MMS) associated with sickle cell disease (SCD) is a severe vasculopathy that significantly increases stroke risk. While cerebral revascularization is increasingly considered in this population, concerns about perioperative safety and long-term outcomes have limited its use in clinical practice. METHODS: We conducted a multicenter, retrospective cohort study of 553 patients with MMS who underwent surgical revascularization across 13 centers. Patients were grouped by SCD status (SCD-MMS vs. moyamoya disease (MMD)). Primary outcomes included perioperative stroke, p…

Advanced hybrid closed chest revascularization: an innovative strategy for the treatment of multivessel coronary artery disease†.

OBJECTIVES: Conventional hybrid revascularization (CHR) combines minimally invasive placement of an internal mammary artery graft to the anterior wall and percutaneous coronary intervention (PCI) of non-anterior wall targets. In this study we assess perioperative and midterm outcomes of advanced hybrid revascularization (AHR) defined as the combination of single or multivessel (MV) totally endoscopic coronary artery bypass grafting (TECAB) with single or multivessel PCI. METHODS: In total, 90 AHR patients [median age 62 years (35-86)] were compared with 90 CHR patients [median age 60 years (3

Successful cerebral artery stent placement for total occlusion of the vertebrobasilar artery in a patient suffering from acute stroke. Case report.

A 64-year-old man suffering from crescendo brainstem symptoms due to acute total occlusion of the vertebrobasilar artery was successfully treated by cerebral artery stent placement. The total occlusion of a long segment of the vertebrobasilar artery was completely recanalized by implanting two flexible, balloon-expandable coronary stents. The patient's clinical outcome 30 days later was favorable. No complications occurred during or after the procedure. This therapeutic option may prove to be a useful means to revascularize an acute total occlusion of the vertebrobasilar artery.

Venous-Preserving Sylvian Dissection.

OBJECTIVE: The Sylvian fissure has been dissected through the frontal side of the superficial Sylvian veins with sacrifice of the tributaries of the frontal superficial Sylvian vein, which may lead to venous infarction. In addition, disturbed venous drainage makes the brain susceptible to brain retraction. Therefore, preservation of the vein is essential in modern neurosurgery to decrease morbidity. METHODS: We describe the technical nuances of Sylvian dissection with an emphasis on preserving the veins. RESULTS: The arachnoid between the frontal and temporal superficial Sylvian veins is div

Intraoperative Superficial Temporal Artery-middle Cerebral Artery Bypass Failure during Combined Bypass Surgery in Children with Moyamoya Disease.

Pediatric patients with moyamoya disease frequently show rapid progression with a high risk of stroke. Indirect revascularization is widely accepted as a surgical treatment for pediatric moyamoya disease, but it does not augment cerebral blood flow immediately, which leaves patients at risk for stroke peri-operatively. This delay in flow augmentation may make adding direct bypass the better option. This study documents our cases of direct bypass failure that underwent indirect bypass supplemented with superficial temporal artery-middle cerebral artery bypass to evaluate the adverse effects of

The Orbital Grading system yields higher precision than the Matsushima grading system in assessing angiographic outcomes after EDAS for Moyamoya disease: an interrater reliability analysis.

Matsushima Grading System has been widely used to evaluate vessel ingrowth after bypass procedures for patients with Moyamoya disease (MMD). However, Matsushima is notably subjective and prone to measurement variability among clinicians. To address these limitations, the Orbital Grading System was developed as an objective approach to assess collateralization after EDAS, utilizing orbital craniometric landmarks to segment the ipsilateral skull into three distinct regions. The current study compares the interrater reliability of both scales. Six physicians, including two attending hybrid-neurov

Superficial Temporal Artery-Middle Cerebral Artery Bypass Treatment Planning for Cerebral Ischaemia Based on Multi-Scale Model.

Superficial temporal artery and middle cerebral artery (STA-MCA) bypass surgery is an effective method to enhance cerebral blood flow (CBF) in ischemic patients. However, the effectiveness of various bypass techniques varies with the diversity of Circle of Willis (CoW) structures. This study aims to develop a physiologically realistic hemodynamic model to optimize STA-MCA bypass planning for cerebral ischemia patients with different CoW structures. This study developed a 0D-1D geometric multi-scale haemodynamic model that coupled the stenosis model and the cerebral autoregulation model. Based

Efficacy of double-barrel versus single-branch superficial temporal artery-middle cerebral artery bypass in the treatment of moyamoya disease: Does double-barrel bypass offer any advantages?

BACKGROUND: To compare the effectiveness of the double-barrel and single-branch superficial temporal artery-middle cerebral artery (STA-MCA) bypass in the treatment of moyamoya disease. METHODS: We conducted a retrospective analysis of the clinical records of patients with moyamoya disease treated with either double-barrel bypass or single-branch bypass. Preoperative and postoperative rates of intracerebral hemorrhage, cerebral infarction, epilepsy, transient neurological deficits (TNEs), and modified Rankin Scale (mRS) score were compared. Cerebral computed tomography perfusion (CTP) values

Hemodynamic Effects of Tortuosity and Stenosis in Superficial Temporal Artery-Middle Cerebral Artery Bypass for Moyamoya Disease.

BACKGROUND: Superficial temporal artery (STA)-middle cerebral artery (MCA) bypass surgery has been widely adopted in treating moyamoya disease (MMD). Geometric variations including high tortuosity and stenosis exist in many cases, but the hemodynamic effects have not been comprehensively evaluated. We aim to evaluate the hemodynamic effects of bypass geometry variations based on patient-specific data. METHODS: In total, 17 patients with MMD who underwent STA-MCA bypass surgery with highly tortuous bypass geometry were included. For each patient, the original 3-dimensional structure of STA-MCA

Staged Common and External Carotid Artery Stenting Followed by Superficial Temporal Artery to Middle Cerebral Artery Bypass.

Superficial temporal artery (STA) to middle cerebral artery (MCA) bypass surgery for internal carotid artery (ICA) occlusive disease necessarily requires sufficient external carotid artery (ECA) blood flow. Surgical bypass is therefore precluded if there is common carotid artery (CCA) occlusion. Here we present two such cases: one patient had a CCA occlusion and the other had an ICA occlusion and ECA stenosis. Both had failed medical management, and were therefore treated with angioplasty and stenting of the ECA, followed by STA-MCA bypass. We describe the clinical and radiologic outcomes of t

Arterial spin labeling as an ancillary assessment to postoperative conventional angiogram in pediatric moyamoya disease.

OBJECTIVE: Digital subtraction angiography (DSA) is commonly performed after pial synangiosis surgery for pediatric moyamoya disease to assess the degree of neovascularization. However, angiography is invasive, and the risk of ionizing radiation is a concern in children. In this study, the authors aimed to identify whether arterial spin labeling (ASL) can predict postoperative angiogram grading. In addition, they sought to determine whether patients who underwent ASL imaging without DSA had similar postoperative outcomes when compared with patients who received ASL imaging and postoperative DS

Endovascular "Boost": External Carotid Angioplasty to Augment Flow Before Superficial Temporal-Middle Cerebral Artery Bypass.

BACKGROUND: Superficial temporal artery-middle cerebral artery (STA-MCA) bypass may be used to augment cerebral blood flow in patients with select cerebrovascular pathologies or after failed attempts at revascularization. In this article we describe the use of balloon angioplasty to boost the flow of the external carotid artery (ECA) including the STA to be later used as a donor artery for a STA-MCA bypass procedure. CASE DESCRIPTION: A 67-year-old male with bilateral carotid artery stenosis presented 2 days after a right middle cerebral artery (MCA) stroke and was admitted for medical manage

Retriever wire supported carotid artery revascularization (ReWiSed CARe) in acute ischemic stroke with underlying tandem occlusion caused by an internal carotid artery dissection: Technical note.

We hereby report a novel technical approach for the treatment of acute stroke with underlying tandem occlusion. The so-called retriever wire supported carotid artery revascularization (ReWiSed CARe) technique, utilizing the wire of a stent-retriever as a guiding wire for carotid artery stenting, is technically feasible in tandem occlusions caused by an internal carotid artery (ICA) dissection or high grade ICA stenosis. This technique eliminates the need to use a long microwire in order to maintain the position inside the true lumen of a dissection. Additionally, it leads to anterograde perfus
