A new system of classification for spinal injuries.

BACKGROUND CONTEXT: A comprehensive classification for spinal trauma has proved difficult to achieve as attested to by the number of systems in use today. In addition, few systems can be applied to all levels of the spine, and the first two cervical vertebrae are usually treated as altogether separate from the remainder of the spine. Consequently, outcome data and comparison of published data are difficult, at best. PURPOSE: The purpose of this presentation is to introduce a new system for classification of spine trauma applicable to all levels of the spine. STUDY DESIGN/SETTING: A different

The optimal transarticular c1-2 screw length and the location of the hypoglossal nerve.

BACKGROUND: Injury to the hypoglossal nerve is a complication associated with transarticular C1-2 screw placement. This complication can be caused by a misdirected or too long screw. Little is known about the optimal screw length and its relationship to the hypoglossal nerve. METHODS: Twenty cervical spine specimens were used to study the optimal length of the transarticular C1-2 screw. Using the Magerl technique, a 3.0 mm drill bit was inserted into the C2 lateral mass, passing through the C1-2 facet joint and penetrating the upper portion of the ventral cortex of the lateral mass of the atl

[Magnetic resonance imaging of rheumatoid pannus in cranio-vertebral synovial joints--report of an operated case].

MRI imaging can provide useful informations as to the status of the brain stem and spinal cord in patients with rheumatoid cervical spine disease. Especially MRI made it possible to visualize the soft tissue mass in the cranio-vertebral joints. However, as far as we know, there is no report describing the pathology of the soft tissue mass diagnosed by MRI. We would like to report an operated case of RA, showing that the soft tissue mass was verified as the so-called rheumatoid pannus in the field of pathology. A 67-year-old woman was admitted to our hospital because of tetraparesis and anterio

[Oropharyngeal dysphagia associated with Chiari I malformation and syringomyelia J].

BACKGROUND: Dysphagia associated with neurological disease is an important clinical manifestation in the diagnosis of injury that justifies the compression of the brainstem and lower cranial nerves. OBJECTIVE: To emphasize the study of dysphagia in a patient with Chiari I malformation associated with syringomyelia in the absence of primary gastroenterological symptoms. CLINICAL CASE: We describe the case of a 62 year-old woman with oropharyngeal dysphagia of six years of evolution, cervicobrachialgia, ptosis and facial diplexia. CONCLUSIONS: Magnetic resonance imaging is an essential elemen

[Ankylosing spondylitis (Bechterew's disease)--a current review (author's transl)].

Ankylosing spondylitis is a disease with clinically and radiologically very variable features. It is the intention to direct the attention from the old by orthopedists' influenced association of the stiff back to the polymorphism of the disease especially to the early stages which still frequently are recognized too late. The paper deals with the nosology, the various states of the clinical course with the numerous radiological criteria including therapeutical approaches.

Evaluation of occipitocervical subluxation in rheumatoid arthritis patients, using coronal-view reconstructive computed tomography.

STUDY DESIGN: Reconstructive computed tomography (CT) study of occipito-atlanto and atlantoaxial joints in RA patients. SUMMARY OF BACKGROUND DATA: The occipitocervical region is one of the most common sites of rheumatoid arthritis (RA). Although lateral radiography has been used for the diagnosis of atlantoaxial subluxation and vertical subluxation, reconstructive CT imaging of the occipito-atlanto and atlantoaxial joints is more sensitive in detecting morphologic changes in this region. We investigated this region in RA patients, using coronal-view reconstructive CT images, and examined the

Combined occipital-cervical and atlantoaxial disassociation without neurologic injury: case report and review of the literature.

STUDY DESIGN: Case report and literature review. OBJECTIVE: To discuss the diagnosis, limitations, and treatment of combined occipital-cervical and atlantoaxial disassociation with normal neurologic function. SUMMARY OF BACKGROUND DATA: Injuries to the craniocervical junction can lead to devastating neurologic deficits. Occipital-cervical disassociation is a well-documented injury pattern that can lead to pain, spinal cord injury, and death. Early diagnosis and treatment can preserve neurologic function. Combined injuries to both the occipital-cervical and atlantoaxial segments are less comm

Unstable atlas fracture treatment by anterior plate C1-ring osteosynthesis using a transoral approach.

STUDY DESIGN: A retrospective study was conducted to evaluate anterior plate fixation of unstable atlas fractures using a transoral approach. OBJECTIVE: To further investigate the safety and efficacy of this surgical technique, as there is currently a paucity of available data. While most atlas fractures can be managed by external immobilization with favorable results, surgery is usually preferable in highly unstable cases. Surgical stabilization is most commonly achieved using a posterior approach with fixation of C1-C2 or C0-C2, but these techniques usually result in loss of joint function

A case of intracranial hypoglossal neurinoma without hypoglossal nerve palsy: operative view of the preserved rostral trunk.

Hypoglossal neurinomas usually manifest with hemiatrophy and weakness of the tongue. A rare case of intracranial hypoglossal neurinoma without preoperative hypoglossal nerve dysfunction and its operative view are presented. A 36-year-old female who presented with headaches and vertigo was admitted to our hospital. The neurological examination revealed bilateral papilledema and mild truncal ataxia, although weakness and atrophy of the tongue were not observed. Magnetic resonance and computed tomography images demonstrated a large foramen magnum tumor without enlargement of the hypoglossal canal

Cavernous hemangioma of the mesencephalon: tonsillouveal transaqueductal approach.

OBJECTIVE: Recent advances in microsurgical techniques facilitate surgical resection of brainstem lesions that were previously considered inoperable. In this article we present, for the first time, the tonsillouveal transaqueductal approach to access a progressively symptomatic cavernoma within the depth of the tegmentum of the mesencephalon. METHODS: A 52-year-old woman presented with a history of slowly progressive right-sided hemiparesis and ataxia. On magnetic resonance imaging, a relatively large cavernoma involving the tegmentum of the mesencephalon was shown. The sylvian aqueduct was p

[Dedifferentiated chordoma of the skull base. A case report].

Sarcomatous transformation in chordoma is a very rare condition and has been emphasized as a distinct entity because of its more aggressive clinical course. Here we describe a case of dedifferentiated chordoma arising from the skull base region of an 11-year-old boy, with tumor recurrence within one year. This tumor showed features of pleomorphic cell sarcoma with areas more typical of chordoma. Most of tumor cells expressed cytokeratin, epithelial membrane antigen, vimentin and S-100 protein, thus confirming the diagnosis of dedifferentiated chordoma.

Injuries involving the transverse atlantal ligament: classification and treatment guidelines based upon experience with 39 injuries.

Comprehensive anatomic and clinical analyses of 39 patients with injuries involving the transverse atlantal ligament or its osseous insertions were performed to assess the morphology of the injured ligaments and the patients' capacity to heal. Injuries of the upper cervical spine were screened with plain radiographs, thin-section computed tomography, and magnetic resonance imaging studies. The injuries were classified as disruptions of the substance of the ligament (Type I injuries, n = 16) or as fractures and avulsions involving the tubercle for insertion of the transverse ligament on the C1

Lymphotoxin-β receptor-NIK signaling induces alternative RELB/NF-κB2 activation to promote metastatic gene expression and cell migration in head and neck cancer.

Head and neck squamous cell carcinomas (HNSCC) preferentially spread to regional cervical tissues and lymph nodes. Here, we hypothesized that lymphotoxin-β (LTβ), receptor LTβR, and NF-κB-inducing kinase (NIK), promote the aberrant activation of alternative NF-κB2/RELB pathway and genes, that enhance migration and invasion of HNSCC. Genomic and expression alterations of the alternative NF-kB pathway were examined in 279 HNSCC tumors from The Cancer Genome Atlas (TCGA) and a panel of HNSCC lines. LTβR is amplified or overexpressed in HNSCC of the larynx or oral cavity, while LTβ, NIK, and RELB

Comparison of occipitocervical and atlantoaxial fusion in treatment of unstable Jefferson fractures.

BACKGROUND: Controversy exists regarding the management of unstable Jefferson fractures, with some surgeons performing reduction and immobilization of the patient in a halo vest and others performing open reduction and internal fixation. This study compares the clinical and radiological outcome parameters between posterior atlantoaxial fusion (AAF) and occipitocervical fusion (OCF) constructs in the treatment of the unstable atlas fracture. MATERIALS AND METHODS: 68 consecutive patients with unstable Jefferson fractures treated by AAF or OCF between October 2004 and March 2011 were included i

Analysis of predisposing factors in elderly people with type II odontoid fracture.

BACKGROUND CONTEXT: Type II odontoid fracture is the most frequent individual fracture in elderly people. An older person usually sustains a Type II odontoid fracture in a fall from standing or a seated height. A relationship between osteoarthritis in the upper cervical spine and Type II odontoid fracture has been reported. However, to our knowledge, few reports have investigated statistically whether disproportionate degeneration between joints influences the susceptibility to fracture. PURPOSE: The purpose of this study was to assess predisposition to Type II odontoid fracture in the elderl

Fusions at the craniovertebral junction.

INTRODUCTION: The surgical management of craniovertebral junction instability in pediatric patients has unique challenges. While the indications for internal fixation in children are similar to those of adults, the data concerning techniques, complications, and outcomes of spinal instrumentation comes from experience with adult patients. Diminutive osseous and ligamentous structures and anatomical variations associated with syndromic craniovertebral abnormalities frequently complicates the approaches and limits the use of internal fixation in children. Cervical arthrodesis in the pediatric age

Vertical translocation. Part II. Outcomes after surgical treatment of rheumatoid cervical myelopathy.

This is a prospective observational study in 116 patients with rheumatoid arthritis and vertical translocation who underwent cervical spine surgery after developing symptomatic myelopathy. These patients, whose mean age was 62 years, had suffered from rheumatoid arthritis for almost 25 years. Surgery was performed via a combination of anterior (67 transoral decompressions) and posterior approaches. Surgical morbidity was recorded in 39% of patients, with a 30-day mortality rate of 10.3%, which was largely related to poor preoperative neurological grade. Neurological improvement of at least one
