A 65-year-old female visited our clinic with painful red vesicular dermatitis of the forehead and around the left eye. She had received acupuncture for headache and shoulder stiffness 6 days before visiting our clinic. A patch test with nickel sulfate gave positive results. We treated her with the greater occipital nerve block and trigger point injection which relieved her pain. We reached the diagnosis of greater occipital trigeminal syndrome with contact dermatitis from the acupuncture needle.

Occupational contact dermatitis (OCD) ranks first of all occupational diseases in many countries. The incidence rate is believed to be around 0.5-1.9 cases per 1000 full-time workers per year. Epidemiological studies play an important role in observing disease trends, analysing risk factors, and monitoring the effect of preventive measures. Updating our clinical concept of atopic dermatitis (AD) evolves from the better understanding of all the immunologic aberrations expressed by the polygenic combinations and permutations associated with the atopic diathesis. Recognizing the immunopathologic features of AD readily underscores that AD without "atopy" is an oxymoron. One hundred and sixty-one beauticians and hairdressers (146 women and 15 men) were examined for the presence of hand dermatitis and those with hand eczema were patch tested with a battery of antigens standardised for beauticians. The clinical assessment of dermatitis in the workplace is put forward as a series of four decisions: (1) is it dermatitis and not psoriasis, tinea, scabies, etc? (2) Is it contact dermatitis and not atopic, seborrhoeic, discoid, stasis or unclassified eczema? (3) Is it irritant contact dermatitis or allergic contact dermatitis? (4) What is/are the probable cause(s)?

Despite the development of new drugs and improvements in working conditions, the prognosis for recovery from contact dermatitis has not changed significantly over the past 50 years. This chapter reviews the prognosis for both irritant and allergic contact dermatitis and discusses the main reasons that contact dermatitis persists, including misdiagnosis and continued exposure to irritants and allergens.

BACKGROUND: An epidemic of cercarial dermatitis caused by Schistosoma spindale cercaria occurred in November 1988 in a district called Chaiya, Surajthani province, in Southern Thailand.
CASE REPORTS: Fifty-eight Thai farmers in Chaiya, Surajthani gave a history of itch following immersion in water while planting rice. The area of rice field was flooded for 10 days before farmers started working. The duration of the disease ranged from 2 days to 1 month. Forty-one patients had the disease for the first time, seventeen had a history of previous manifestations.

Allergic contact dermatitis can be acute or chronic and is caused by delayed hypersensitivity to allergens in the surroundings. This retrospective study, the first of its kind in the Negev district, included 150 patients with allergic contact dermatitis. Nickel was the most frequent allergen causing contact dermatitis in both females and males, while chrome was second in frequency. Most of those with positive reactions to patch tests, were females. Irritant hand dermatitis has long been recognized in the pottery industry. In our series, among workers handling glaze, sensitization to chromate was common and allergy to other metals and to biocides also occurred. Allergy to oil additives was found in mould makers. Whilst irritant hand dermatitis does occur, allergy to metals and biocides should be looked for in workers handling glazes, and allergy to oil additives in mould makers.

A pesticide patch test series was tested in 200 subjects, 50 of whom were agricultural workers. Positive reactions to fungicides were found in 24, almost all to thiophthalimides, especially captan, difolatan and folpet. Reactions to bis-dithiocarbamates and benomyl were rare, and to other pesticides not significant.

In human volunteers, firm pressure was applied to deformable plastic chambers containing chemical irritants, glass fibers, and selected allergens to which the subjects were sensitive. Pressure variably intensified irritant dermatitis and strikingly enhanced fiberglass dermatitis. Pressure did not influence allergic contact dermatitis. Pressure is one of several mechanical factors that can aggravate nonallergic contact dermatitis.
