Revisional surgery after colon interposition for benign oesophageal disease.

Although the short-term results of colon interposition for replacement of the oesophagus in part or as a whole are known to be satisfactory, there have been several reports of functional problems associated with total replacement in the long-term follow-up of patients. We have retrospectively studied patients who have required revisional surgery for anatomical and functional sequelae over a 7- to 38-year period. Although the short-segment colon interpositions have been relatively trouble free, several mechanical and functional problems requiring revisional surgery have been encountered in the

Treatment of anorectal anomalies by anterior perineal anorectoplasty.

PURPOSE: The aim of this study was as an introduction of a technical modification to the anterior perineal approach for the treatment of anorectal anomalies (AA) and analysis of its long-term results. METHODS: Forty-three children with AA underwent a modified anterior perineal anorectoplasty (APA), which involved the following technical guidelines: construction of a rectoanal muscular conduit through the levator ani muscle and the external anal sphincter, which were sutured to each other; preservation of the rectal end of the fistula; and invagination of the perineal skin through the neoanus.

An analysis of operations for gastroesophageal reflux disease: identifying the important technical elements.

BACKGROUND: Better understanding of the pathogenesis of gastroesophageal reflux disease in recent years has not been accompanied by appreciable advances in the design of antireflux operations. In many cases, operations are still being performed just as they were described 30 years ago. It is important now to go beyond the eponymous procedures traditionally associated with antireflux operations and to identify the technical elements that contribute to effective and durable fundoplications. OBJECTIVES: To compare antireflux operations and identify the important technical elements. DESIGN AND S

The Association Between Postoperative Fluid Balance and Postoperative Complications in Adults Undergoing Gastrointestinal Surgery: Protocol for a Scoping Review.

BACKGROUND: Fluid balance in patients who have undergone gastrointestinal surgery is known to be challenging to monitor, resulting in imprecise and inaccurate measurements. Despite this knowledge, any association between fluid balance accuracy and the development of postoperative complications in the early postoperative period is limited. This scoping review aims to map the existing knowledge between fluid monitoring and the development of postoperative morbidity, mortality, and length of hospital stays (LoS) in patients who have undergone gastrointestinal surgery. METHODS: The review will fo

Application of the Quadratus Lumborum Block in Pediatric Gastrointestinal Surgery: A Scoping Review.

BACKGROUND: Effective pain control is critical in pediatric gastrointestinal surgery to support recovery and improve postoperative outcomes. Regional anesthetic techniques have gained increasing interest, with the quadratus lumborum block (QLB) emerging as a promising option. As its use expands, this scoping review aimed to map the available evidence on QLB in pediatric gastrointestinal surgery. Specifically, it examined its application, gastrointestinal procedures involved, reported outcomes, and potential advantages and disadvantages. METHODS: This scoping review followed PRISMA-ScR guideli

Anticipated effects of centralising complex gastrointestinal surgery in a rural area in Germany - perspective of health professionals: a qualitative study.

BACKGROUND: For centralising complex gastrointestinal surgery, minimum volume standards have been implemented in Germany. To increase centralisation and thus patient safety, the minimum volume standards for complex oesophageal and pancreatic surgery were set to rise in 2023 and 2025, respectively. Hospitals that do not meet these minimum volume standards will lose authorisation to perform these surgical procedures and are not eligible for reimbursement. Volume-outcome studies focus on patient outcomes at high or low hospital volumes. Other impacts on the service, professionals (e.g. medical tr

Beyond P Values : Novel Minimal Important Difference of the Comprehensive Complication Index That Reflects a Meaningful Outcome for Patients Undergoing Major Abdominal Surgery.

OBJECTIVE: To estimate the minimal important difference (MID) of the Comprehensive Complication Index (CCI ® ) in patients undergoing abdominal surgery. BACKGROUND: The CCI ® is a validated metric that quantifies cumulative surgical morbidity. While the CCI ® is a sensitive endpoint to detect treatment effects, a statistically significant effect does not necessarily translate into clinical relevance. Relevant differences from the patients' perspective are best captured by the MID. METHODS: Individual patient data were extracted from surgical studies reporting CCI ® at 30 days and using patie

Comparison between perineal and abdominal approaches for the surgical treatment of recurrent external rectal prolapse: a systematic review and meta-analysis.

PURPOSE: Although surgery is the most effective treatment for rectal prolapse, a risk of recurrence reported in literature is 6-27%. The aim of this meta-analysis is to compare the abdominal and perineal approach for surgical treatment of recurrent external rectal prolapse. METHODS: A systematic search of PubMed and Embase was performed following the Preferred Reporting Items for Systematic Reviews and Meta-analysis (PRISMA) guidelines. A comprehensive literature search of PubMed and Embase was conducted from January 2000 to May 2024, for studies reporting surgery for recurrent external recta

Surgical Techniques for the Treatment of Hirschsprung's Disease-A Historical Systematic Review and Current Status Quo.

AIM: Hirschsprung's disease, or aganglionosis of the colon, is a congenital disease characterized by a lack of neurons in the enteric plexus of the large bowel due to either migration failure or defective differentiation of neural crest cells. We aimed to conduct a historical systematic review of available literature and personal experience to exhibit the surgical techniques performed in the past and the current status quo of surgery for Hirschsprung's disease. METHODS: We searched PubMed, EMBASE, Google Scholar databases for studies examining surgery in aganglionosis or Hirschsprung's diseas

Postoperative Gastrointestinal Dysfunction After Neuromuscular Blockade Reversal With Sugammadex Versus Cholinesterase Inhibitors in Patients Undergoing Gastrointestinal Surgery: A Systematic Review and Meta-Analysis.

BACKGROUND: Postoperative gastrointestinal dysfunction (POGD) commonly occurs following gastrointestinal (GI) surgery and is associated with specific anesthetic agents. Cholinesterase inhibitors employed for reversing neuromuscular blockade have been implicated in development of POGD. Sugammadex, a novel reversal agent, is linked with reduced POGD. However, there is a lack of comprehensive comparative review between these agents regarding their impact on POGD following GI surgery. This study aims to systematically review the effects of sugammadex on POGD compared to cholinesterase inhibitors f

Robot-Assisted Surgery for Reversed Intestinal Malrotation with Concurrent Cecal Carcinoma: A Case Report.

BACKGROUND Reversed intestinal malrotation is an extremely rare disease, with an incidence of 1 in 250 000. In Japan, application of robotic-assisted colorectal cancer surgery is expected to increase. There are no reports of robot-assisted surgery for cecal cancer with reversed intestinal malrotation. CASE REPORT An 84-year-old Japanese man with epigastric pain and abdominal distention was referred to our hospital's Department of Gastroenterology for thorough examination. Colonoscopy revealed a semicircumferential type 2 tumor in the cecum and ascending colon. Gastrografin contrast study showe

Gender and Racial Diversity Among Colon and Rectal Surgery Trainees and Leaders.

BACKGROUND: Despite recent changes in women and underrepresented minorities in medicine, there still exists large gender and racial gaps in surgical training and leadership. OBJECTIVE: We hypothesize that gender and racial representation have improved among general and colorectal surgical trainees and leadership over the past 20 years. DESIGN: This cross-sectional study examines gender and racial representation of general and colorectal surgery residents, colorectal faculty members, and the American Society of Colon and Rectal Surgeons Executive Council. SETTINGS: We extracted data from the

Anal canal gastrointestinal stromal tumour (GIST).

SummaryGastrointestinal stromal tumours (GISTs) are malignant mesenchymal tumours that originate from the interstitial pacemaker cells of Cajal. They are particularly rare, comprising only 5% of all GISTs, and often present at an advanced stage. Treatment for these tumours is still a topic of debate due to their low incidence and insidious location. A woman in her mid-70s presented with rectal bleeding and anal discomfort. A 4.5×4 cm anal GIST was diagnosed. A local excision was performed and the patient was further treated with tyrosine kinase inhibitors. At a 6-month follow-up, the MRI showe
