Machine learning prediction of deep vein thrombosis before staged surgery in lower-leg and ankle fractures treated with temporary external fixation.

BACKGROUND: Deep vein thrombosis (DVT) detected between temporary external fixation and staged definitive surgery can complicate perioperative management of lower-leg and ankle fractures. We developed machine-learning models to estimate DVT probability during this interval. METHODS: This retrospective cohort included adults admitted during 2015-2025 who underwent staged fixation and bilateral lower-extremity venous ultrasonography. A stratified 70:30 split defined development and validation sets. Development-set imputation, Boruta feature selection, and five-fold cross-validation were used to

Successful Management of a Gustilo-Anderson Type I Open Comminuted Tibial Fracture Using Ilizarov Circular External Fixation: A Case Report.

Open tibial fractures are challenging injuries due to limited soft tissue coverage, contamination, and the risk of infection or delayed union. The Ilizarov circular external fixation system provides stable fixation while preserving periosteal vascularity, minimizing further soft-tissue disruption, and allowing early weight bearing. We report a case of a 50-year-old man who sustained a Gustilo-Anderson type I open comminuted tibial fracture following a road traffic accident. Initial management included wound irrigation, sterile dressing, immobilization, and intravenous antibiotics. Definitive t

Proposal and validation of an angular External Fixation Index (aEFI) for standardized reporting of gradual angular deformity correction: a time-normalized descriptive metric.

BACKGROUND: Gradual correction of genu varum using circular external fixators is well-established. Although the External Fixation Index (EFI) is widely used in linear bone lengthening, no standardized, time-normalized metric exists for angular deformity correction. This study introduces the angular External Fixation Index (aEFI) as a descriptive tool and evaluates internal consistency within a clinical cohort treated with oblique proximal tibial corticotomy (OPTC). METHODS: A prospective cohort study included 22 patients (30 knees) who underwent gradual genu varum correction using OPTC an…

Clinical Efficacy of Combined Internal and External Fixation with Nanosilver Dressing and Vacuum Closure for Gustilo Type I/II Open Tibial Fractures.

Gustilo type I and II open tibial fractures (OTFs) are associated with significant soft tissue injury and a high risk of infection, often resulting in prolonged healing and postoperative complications when treated using conventional internal or external fixation. This study introduces a clinically validated approach that integrates nanosilver antibacterial dressing (NSAD) and vacuum-assisted closure (VAC) with closed reduction and intramedullary nail fixation (CRINF) to improve wound and fracture healing outcomes. A total of 300 patients with Gustilo type I/II OTFs were enrolled and randomly a

External hinged fixation vs. internal joint stabilization for elbow instability: a systematic review and meta-analysis of functional outcomes and surgical complications.

BACKGROUND: Traumatic elbow instability can be managed with hinged external fixator (HEF) or internal joint stabilizer (IJS). While prior studies report device-related complications with both devices, a comprehensive analysis comparing range of motion (ROM), patient-reported outcome measures, and surgical complications is limited. This study aims to evaluate these outcomes to guide treatment decisions for complex elbow instability. METHODS: This systematic review and meta-analysis was conducted following Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. A comprehe

Risk of Fracture at External Fixator Pin Hole After Lateral Tibial Plateau Fracture Plating: A Biomechanical Comparison of Different Screw Configurations.

BACKGROUND: External fixation of tibial plateau fractures commonly provides temporary stabilization before definitive fixation with plate and screws. The purpose of this study was to determine if an external fixator pin hole distal to a tibial plate in a synthetic fracture model would increase the risk of fracture after fixation. Another objective was to determine the ideal configuration when placing tibial plate screws near an external fixator pin hole. METHODS: Thirty synthetic tibiae were tested and evenly divided into 5 groups. Tibial plateau plates were placed with 4 different screw conf

Unilateral external fixator and its biomechanical effects in treating different types of femoral fracture: A finite element study with experimental validated model.

Previous works had successfully demonstrated the clinical effectiveness of unilateral external fixator in treating various types of fracture, ranging from the simple type, such as oblique and transverse fractures, to complex fractures. However, literature that investigated its biomechanical analyses to further justify its efficacy is limited. Therefore, this paper aimed to analyse the stability of unilateral external fixator for treating different types of fracture, including the simple oblique, AO32C3 comminuted, and 20 mm gap transverse fracture. These fractures were reconstructed at the dis

Adjuvant Denosumab therapy following curettage and external fixator for a giant cell tumor of the distal radius presenting with a pathological fracture: A case report.

INTRODUCTION AND IMPORTANCE: Denosumab is used as a neoadjuvant therapy for giant cell tumours (GCT) prior to surgery to improve surgical clearance and reduce the rate of recurrence. However, the use of denosumab as adjuvant therapy following an external fixator for GCT of the distal radius has not been commonly described. We describe the use of adjuvant denosumab following curettage and external fixation in a patient with GCT of the distal radius presenting with a pathological fracture. CASE PRESENTATION: A 23-year-old male presented with a right distal radius fracture. Imaging was suggestiv
