Temporal bone fracture related facial palsy: efficacy of decompression with and without grafting.

PURPOSE OF REVIEW: This systematic review investigates the recent literature and aims to determine the approach, efficacy, and timing of facial nerve decompression with or without grafting in temporal bone fractures with facial palsy. RECENT FINDINGS: The surgical management of facial palsy is reserved for a small population of cases in which electrophysiologic tests indicate a poor likelihood of spontaneous recovery. The transmastoid (TM), middle cranial fossa (MCF), and translabyrinthine (TL) approaches to the facial nerve provide access to the entire intracranial and intratemporal segments

Development of an intraoral device for facial muscle retraining and its clinical application.

The aim of this work was to develop a facial neuromuscular retraining technique for cases of facial palsy, involving an easy-to use intraoral device that allows correcting abnormal neuromuscular patterns and improving esthetics. Facial palsy is a motor alteration of multiple origin that results in facial asymmetry. Treatment remains controversial to date and includes, surgical anastomosis and decompressive and plastic surgery, corticosteroids therapy, injection of botulinum toxin, and administration of other medicinal drugs. Physical therapy involving electrical stimulation and feedback are al

Endoscopy-Assisted Transmeatal Cochlear Implantation in Multiple Ear Deformities.

To study congenitally deaf children with inner ear malformations that usually have comorbid anomalous facial nerves and middle ear deformities. To determine the feasibility of endoscopy-assisted transmeatal cochlear implantation with the purpose of reducing the risks of iatrogenic facial nerve injury. This report presents a unique technique in a pediatric case with multiple ear anomalies: microtia, cochlear hypoplasia with an aberrant facial nerve, a sigmoid sinus deformity leading to a narrow mastoid cavity, and a flat promontory wall without round window. A cochlear implant electrode array w

Craniofacial trauma: an assessment of risk related to timing of surgery.

Following the retrospective analysis of approximately 4000 head-injury patients, 49 were identified with a combination of displaced facial fractures and significant cerebral trauma. The purpose of this study was to define clinical and radiographic features in these patients that are associated with a poor prognosis, which in turn might influence the timing of facial fracture repair. The presence of an upper-level facial fracture, low Glasgow coma score, intracranial hemorrhage, displacement of normally midline cerebral structures, and multisystem trauma was associated with a statistically sign

Arteriovenous fistulas of the facial artery after mandibular surgery: treatment by embolization.

OBJECTIVE: We describe three cases of arteriovenous fistula (AVF) that developed between the facial artery and facial vein after mandibular surgery, two after sagittal split osteotomy of the mandible and one after the removal of an infected dental bridge, and discuss the embolization techniques that were used for their successful treatment. CONCLUSION: AVF between the facial artery and vein can be successfully treated by embolization and is usually best achieved by occluding the facial artery on both sides of the arterial injury.

Orbital abscess a rare complication after bichectomy surgery.

Bichectomy is the partial removal of the Bichat's fat pad for the aim of smoothing the facial contour. The complications of bichectomy include soft tissue infections, hematoma, facial paralysis (especially buccal branch paralysis), stenon canal injuries and related complications such as sialocele and sialoadenitis, however these are not common complications in clinical practice. Here we report a case of a 29-year-old white female with a right sided orbital abscess following bichectomy. Right sided orbital abscess and orbital cellulitis was managed with endoscopic decompression and drainage. Th

A Rare Complication of Infraorbital Nerve Hyperesthesia in Surgically Repaired Orbital Fracture Patients.

Injury to the peripheral sensory branches of the trigeminal nerve can follow a wide variety of craniofacial injuries. Many patients with facial fractures complain about the symptom of numbness to the distribution of injured nerve, which is indicative of hypoesthesia. Hyperesthesia involving the infraorbital nerve is rare in comparison to hypoesthesia secondary to facial trauma. The authors report on 2 patients with infraorbital nerve hyperesthesia in surgically repaired orbital fracture patients. Surgical decompression of the infraorbital nerve led to rapid resolution of hyperesthesia. To the …

[Cranio-maxillofacial traumatology].

During the last 50 years, significant developments have taken place in the treatment of cranio-maxillofacial fractures. The aim of treatment is to achieve the most complete possible restoration of facial functions Epidemiologic figures for these fractures are sparsely available in the literature, but traffic accidents, acts of violence and sports injuries appear to be the main causes. Currently, treatment of facial trauma is aimed at early and (nearly) complete restoration of function. To achieve this goal almost exclusive use is made of stable osteosynthesis methods in The Netherlands. An opt

Traumatic pseudoaneurysm of the superficial temporal artery: two cases and review of the literature.

Aneurysms of the facial vasculature have been reported since the mid-seventeenth century from a variety of accidental, violent, and surgical injuries. Of the 386 cases of facial aneurysm reported in the world literature, 327 involve the superficial temporal artery. The nomenclature is somewhat confusing, because pseudoaneurysms are often referred to as simply "aneurysms" in the literature. Given that superficial temporal artery pseudoaneurysms might be seen in the guise of a cyst or lipoma, it is imperative that the dermatologist be aware of their presentation. To date, there have been 7 cases

Surgical management of the paralyzed eye.

Blindness is the most dreaded complication of an untreated paralyzed eyelid following injury to the facial nerve. Injuries to the facial nerve are mainly postsurgical. Assessment of neural injury using serial testing is important to be able to differentiate between temporary and permanent paralysis. In the former case, medical management could be sufficient and, in the latter case, a surgical procedure is required. The physician has the choice among several procedures to repair upper eyelid paralysis with the gold standard being the gold weight implant. Lower eyelid ectropion can be repaired u

Management of Long-Standing Flaccid Facial Palsy: Periocular Considerations.

Ineffective eyelid closure can pose a serious risk of injury to the ocular surface and eye. In cases of eyelid paresis, systematic examination of the eye and ocular adnexa will direct appropriate interventions. Specifically, 4 distinct periorbital regions should be independently assessed: eyebrow, upper eyelid, ocular surface, and lower eyelid. Corneal exposure can lead to dehydration, thinning, scarring, infection, perforation, and blindness. Long-term sequelae following facial nerve palsy may also include epiphora, gustatory lacrimation, and synkinesis.

[Total work disability: a national inquiry in France].

Physicians are often asked to draft a descriptive medical certificate for victims of traumatic maxillofacial injuries. Among other requisites, this certificate must mention the subject's total work disability. If the injury results from intentional or unintentional assault and battery, the duration of the work disability will determine the competence of the court that will try and condemn the perpetrator. Thus the total work disability certificate has penal value and should be used only for this purpose. Using the data acquired from a national inquiry in France, we evaluated common practices a…

A focal ischemic stroke of the somatosensory thalamus produces persistent spontaneous pain-like behavior.

Ischemic injury to the somatosensory ventrobasal (VB) thalamus is strongly associated with central post-stroke pain (CPSP), yet preclinical models of selective focal ischemic VB injury remain limited. Here, we developed a mouse model of ischemic VB stroke using photothrombotic occlusion. Stroke-affected mice exhibited persistent spontaneous facial grimace throughout the four-week observation period, consistent with a sustained affective behavioral state. Hindpaw mechanical hyposensitivity and postural abnormalities were transient, emerging early after stroke and resolving within two weeks. The
