Avulsion of the perineal body in a single nulliparous woman following a motorcycle accident: a case report.

Miss OO was seen at the gynaecology clinic in April 2008 with history of a road traffic accident three years earlier. She was thrown off a motorcycle and fell astride a culvert, sustaining perineal injury. She was treated at a private hospital, but subsequently developed faecal incontinence and was usually soiled with faecal matter during coitus. Examination revealed loss of perineal body and reconstruction was done, with success. With increase in motorcycle accidents on our roads, many more gynaecologically-related traumas are bound to occur. Proper and timely referral will lead to improved m

General geriatrics and gastroenterology: constipation and faecal incontinence.

The incidence of constipation increases with age but no consistent changes of colonic or anorectal motility have been shown in elderly people. Instead, neurological diseases, constipating drugs, bedriddenness and weak straining ability may explain this increased prevalence of constipation. The amount of dietary fibre in the diet may be reduced because of poor chewing ability. Parkinson's disease is accompanied by both slow colonic transit and impaired relaxation of the anal sphincter. Drug-induced constipation is particularly likely with anti-parkinsonism drugs (either anti-cholinergic or dopa

Second-line treatment for faecal incontinence.

BACKGROUND: In the treatment of faecal incontinence, more than 30% of patients experience continuation of their problem. We discuss new therapeutic procedures for dealing with faecal incontinence. METHODS: Discussion of authors' own work in relation to the literature. RESULTS: First-line care includes diets, constipating drugs, biofeedback therapy, anal repair and operations for prolapse and fistulas. For the failures of these first-line treatments there is hope with second-line therapies. Creation of a neosphincter is possible with a dynamic graciloplasty (DGP) or an artificial bowel sphinc

Incontinence and anorectal dysfunction in patients with diabetes mellitus.

The elucidation of the pathogenesis of faecal incontinence in patients with advanced diabetes mellitus has progressed during the last 15 years. In contrast to earlier concepts which promoted the concept that high stool volumes overwhelm normal continence mechanisms, recent studies indicate that the vast majority of diabetic patients with faecal incontinence have normal or only moderately increased daily stool volumes, but also exhibit multiple abnormalities of anorectal sensory and motor functions. These changes are not observed in continent diabetic patients. Treatment consists of pharmacolog

Anorectal manometry: techniques and clinical applications.

Anorectal manometry has gained wide acceptance as a helpful method to objectively assess the apparatus of defecation provided by the anorect … sease, more often it is used to assess common disorders such as chronic idiopathic constipation and fecal incontinence. Anorectal manometry has also been considered as a preoperative evaluation in patients having abdominal and anorectal procedures during which the state of continence can be jeopardized.

[Anal incontinence. Critical study of the surgical treatment].

Two million French people aged over 45 are thought to suffer from faecal incontinence. The most frequent anatomical or functional causes of this disability are: idiopathic incontinence of falling down perineum due to denervation of its striated muscle; some neurological diseases; rupture or destruction of the sphincter due to traumatic, iatrogenic or obstetrical causes; complete rectal prolapse. Each of these situations has its medico-surgical solution (post-anal repair, sphincterorrhaphy, rectopexy, Delorme's operation, myoplasty, artificial sphincter), always bearing in mind that the fundame

[Psychotherapy in the treatment of encopresis in children with idiopathic megacolon].

The authors treated by psychotherapy 70 children aged 3 to 15 years with encopresis and constipation. In all irrigographic examination revealed idiopathic megacolon. In the course of treatment and after treatment 93.1% of the children are free from complaints, of these 48% are without any treatment, the others are given intermitently small doses of lactulose and Clavigrenin. 4.6% of children have occasional problems, 2.3% of the patients did not improve. Improvement of the condition depends on the cooperation of the parents, in particular the mothers. In mothers of the treated children by mean

[Diagnosis and treatment of anal fistula].

Anorectal fistulas can only be cured by operative treatment based on a thorough knowledge of anal anatomy. Recurrences are frequently a result of the surgeon's failure to expose the entire fistula tract out of fear of impaining anal continence. Surgery can provide a permanent cure in 95% of all such cases. Curative treatment of fistulas in patients with colitis, Crohn's disease or AIDS may not be feasible, but some improvement can be achieved. Anal sphincter function should be measured prior to surgery. The anal sphincter is always stronger in males than in females, who are more likely to expe

Puerperal perineal repair.

Perineal lacerations, which may or may not include the rectal sphincter and mucosa, commonly result from birth trauma. The surgical treatment of these complications is commonly performed during a period not associated with pregnancy - usually in the perimenopausal years or after the patient has achieved her ideal family size. Puerperal perineal repair, described here, has not been associated with a higher incidence of morbidity or recurrences. The technique is well known to gynecologists, and it can be safely employed during the immediate postpartum period. At this time, it is economically adv

Incontinence in full-thickness rectal prolapse: low level of improvement after laparoscopic rectopexy.

AIM: The study aimed to quantify incontinence before and after laparoscopic rectopexy in patients suffering from rectal prolapse. METHOD: Eighty-five patients underwent laparoscopic rectopexy to treat rectal prolapse between 2003 and 2009. Symptomatic and functional data were collected prospectively before and after surgery by self-administered questionnaires including the Cleveland Clinic Fecal Incontinence Score (CCIS) and constipation, gastrointestinal quality of life and urinary incontinence questionnaires. Incontinence was considered to be present when the CCIS remained at ≥ 5 after surg…

Sacral neuromodulation for persistent faecal incontinence after laparoscopic ventral rectopexy for high-grade internal rectal prolapse.

AIM: Internal rectal prolapse is recognized as an aetiological factor in faecal incontinence. Patients found to have a high-grade internal r … re offered sacral neuromodulation. Symptoms were evaluated before and after the procedure using the Fecal Incontinence Severity Index (FISI) and the Gastrointestinal Quality of Life Index (GIQLI). RESULTS: Temporary test stimulation was successful in 47 (94%) of the patients who then underwent implantation of a permanent pulse generator. The median FISI score 1 yea …
