BACKGROUND: Hip resurfacing has been proposed as an alternative to total hip replacement in patients who have proximal femoral deformities or retained hardware in the proximal aspect of the femur. In these situations, placement of a conventional stemmed hip prosthesis would be difficult or impossible, possibly necessitating a complex osteotomy or a custom prosthesis. BACKGROUND: Fibrous dysplasia (FD) of the proximal femur frequently leads to secondary hip osteoarthritis and shepherd's crook deformity. Total hip arthroplasty (THA) in this setting typically requires corrective osteotomy and bone grafting. A simplified, osteotomy-free strategy has not been well characterised.
CASE PRESENTATION: A 50-year-old woman with monostotic FD and moderate shepherd's crook deformity (neck-shaft angle 110°) underwent primary THA via the direct anterior approach using a fully hydroxyapatite-coated cementless revision stem, without corrective osteotomy or bone grafting.

Eleven cases of complex femoral fractures were seen from November 1987 to November 1989; five ipsilateral femoral neck and shaft fractures and six comminuted subtrochanteric fractures. High-energy accidents accounted for most of these injuries. There were numerous associated injuries, many requiring operative procedures. Neglected fractures of the femoral neck, common in young adults in underdeveloped countries, may be complicated by nonunion or avascular necrosis (AVN). We treated 52 cases by open reduction, fixation by compression screw and a free fibular graft. The mean delay between injury and operation was 5.1 months.
