A returning traveler with fever.

Active duty military personnel and civilian government employees are at risk for exposure to a variety of pathogens as they meet their mission requirements throughout the world. When they present with a systemic disease, the clinician must entertain a broad differential diagnosis due to the multitude of infectious, neoplastic, and noninfectious inflammatory disorders that can have similar clinical presentations. Since treatment varies significantly, and lack of appropriate therapy may be deadly, the search for the definitive diagnosis should be aggressive and thorough. We present a case of adu

Fever in the returning traveller.

Fever is a common reason for acute hospital admission for tropical illness in UK referral units. A sensible working diagnosis can usually be formulated from a careful history and examination and initial simple investigations. The history should include details of exactly where the patient has been, what conditions he or she was living in, and the exact dates of arrival and departure. The quality of pre-travel advice and vaccinations, adherence to chemoprophylaxis against malaria, avoidance of insect bites and general behaviour abroad (including sexual history) are also important. Localizing fe…

Fever in a returned traveller.

An 8-year-old boy, presenting with fever after returning from a visit to Papua New Guinea, was found to have typhoid, despite immunization against this disease prior to travel. This most likely represented vaccine failure, although it is also possible that he acquired the infection from overseas visitors who had stayed with his family prior to his travel, or that he contracted the disease from his father. This case highlights the importance of considering typhoid as a cause of fever in a returned traveller, even if they have been previously immunized against this disease. It also highlights th…

Fevers and mouth ulcers.

Mouth ulcers are commonly caused by infection but may be due to neutropenia. The most common form of hyper-IgM syndrome is of X-linked inheritance and caused by CD40 ligand gene mutations. Consider hyper-IgM syndrome in a male child with recurrent bacterial or opportunistic infections, neutropenia, hypogammaglobulinaemia (IgG and IgA) and normal T- and B-cell counts. In X-linked hyper-IgM syndrome: - the serum IgM concentration is normal in about 50% of cases. - transient or persistent neutropenia occurs in 70% of cases. First-line therapeutic options for hyper-IgM syndrome include regular int

Fever and cancer in perspective.

CONTEXT: A relationship between feverish infection and concurrent remission from cancer has been known about for a very long time. However, a systematic investigation of the phenomenon has not yet been made. OBJECTIVE: To bring together the isolated observations about the coincidence of spontaneous remissions with feverish infections and William Coley's seminal work, as a basis for devising an immunological hypothesis about the putative anti-cancer effect of fever. CONCLUSION: Fever induction under medical guidance may be considered as part of a therapy regimen for cancers of mesodermal orig…

Thermoregulation and the pathogenesis of fever.

Infections, trauma, inflammatory processes, and some malignant diseases induce a constellation of host responses that are collectively referred to as the "acute-phase response." Elevation of core temperature is certainly part of the acute-phase response, and cytokines that raise core temperature in pathologic states are some of the same cytokines that account for other manifestations of the acute-phase response. This article examines fever as a part of the acute-phase reaction and the role of cytokines in thermoregulation.

Treatment of fever in childhood.

Although the need for routine antipyretic therapy in children has often been questioned, there are no data to contra-indicate this. Not all fevers need to be treated but many physicians do so to relieve parental concern. The most commonly used antipyretic drugs are acetylsalicylic acid (ASA), paracetamol (acetaminophen) and dipyrone (metamizol). Paracetamol and ASA have been extensively evaluated but there are few clinical trials on dipyrone. In the last decade a strong statistical association has been observed between salicylates and Reye syndrome. Paracetamol is the most common cause of acut…

[An epidemic of typhoid fever].

An outbreak of typhoid fever was observed in a group of students and accompanying teachers during a journey to Haiti in summer 1991. All (6) infected persons were symptomatic and rapidly cured by antibiotics. The Salmonella typhi strains isolated from blood cultures were all identical by ribotyping. All the participants had a medical checkup and stool cultures were obtained; asymptomatic carriers could be excluded. Secondarily infected persons were not observed. The infectious source has not been identified, but various hypotheses are considered. Oral vaccination does not appear to modify the …

Panniculitis and fever in children.

We describe three children with panniculitis and associated systemic manifestations including fever. Histopathologic features, such as the presence of lobular or septal inflammation, presence of vasculitis, character of the cellular infiltrate, and presence of erythrophagocytosis, were useful in classifying this group of panniculitides. In one patient with subcutaneous polyarteritis nodosa, corticosteroid therapy was effective; in two other patients with histiocytic cytophagic panniculitis, there were poor responses to steroids, intravenously administered immune globulin, dapsone, or antimalar…

The returning traveler with fever.

The febrile returning traveler tests a clinician's knowledge of tropical medicine as well as skills in differential diagnosis. A thorough history with special emphasis placed on the patient's travel itinerary and knowledge of the geographic location and incubation times of certain tropical diseases will narrow the diagnostic possibilities. This will allow the clinician to focus the diagnostic work-up and make wise choices of laboratory tests and procedures.
