OBJECTIVES: To obtain data concerning the incidence and management of reservoir herniation in inflatable penile prosthesis surgery in a clinical investigation. Reservoir herniation after scrotal placement of inflatable penile prosthesis is an unusual complication, and a review of the published medical reports reveals only anecdotal reports and no definitive articles.
METHODS: A multi-item self-addressed questionnaire was mailed to the members of the Society for the Study of Impotence questioning the occurrence of reservoir migration from the prevesical space to the inguinal canal or scrotum.

Gastric outlet obstruction secondary to benign or malignant disease can be treated with self-expanding metallic stents. This obviates the need for surgical bypass procedures and returns most patients to a normal diet. Prosthesis displacement is described as a late complication. A case of stent migration to the rectum, presenting with the symptoms and signs of incomplete large bowel obstruction, is described.

We describe the case of a child in whom proximal migration of the peritoneal catheter and extrusion of the ventricular catheter resulted in the entire ventriculo-peritoneal shunt along with the shunt chamber (Orbis Sigma II valve) lying in a sub-galeal pocket in the occipital region in a tightly coiled fashion. An 18-year-old patient who had correction of pectus excavatum deformity in our department 4 years earlier was admitted because of stabbing chest pain. He had not attended to postoperative controls and had not come for extraction of the steel strut, although he had been contacted. He was diagnosed to have a broken steel strut, and the strut was noted to be embedded in the myocardium. This unreported complication of pectus excavatum operation forced us to review sternal support techniques.

Vocal fold palsy is a cause of dysphonia. Due to incomplete glottic closure during phonation, patients with a unilateral vocal fold palsy present with a weak and breathy voice and recurrent aspiration. PURPOSE: One of the modalities of treating dry eyes is punctal plugs. They are usually used for temporary occlusion of the lacrimal drainage system. Among the complications associated with silicone punctal plugs are extrusion, downward migration, irritation, and epiphora. To our knowledge, this is the first report of dacryocystitis and canaliculitis as a result of spontaneous migration of punctal plugs into the lacrimal drainage system.
METHODS: We describe the sequelae of spontaneous migration of silicone punctal plugs into the lacrimal drainage system in two patients with dry eyes.

A case is reported of the presence of an intrauterine device into the bladder due to uterine perforation with calculus formation in a 39 year old woman, with two pregnancies after its installation. The patient was seen for complaints of pelvic pain, pollakiuria, dysuria and tenesmus, without uterine sintomatology. We describe herein the case of a 3-year-old child in whom a jejunoileal fistula was caused by the ingestion of magnets. This case report demonstrates that if more than one magnet is found as a foreign body in the intestine, they should not be left untreated even if there are no sharp edges and, it seems they could be evacuated spontaneously. This recommendation is made because the magnets will attract each other and hold the intestinal walls between them, causing necrosis and resulting in intestinal perforation or a fistula.

Report of two cases of sudden acute pulmonary embolism of an inferior vena cava filter, initially indicated to prevent cruoric pulmonary thrombo-embolism. These two iatrogenic complications were both due to the secondary failure of an interventional percutaneous procedure and led to aggressive and urgent surgical removal of the filter by sternotomy and cardiopulmonary bypass. CSF hydrocele as a complication of migration or extrusion of the peritoneal end of the V-P shunt has rarely been reported. Here the case of a 6 month old infant, born at 28 weeks gestational age, is reported. The baby was noted to have scrotal swelling, exacerbated by crying, two months after insertion of ventriculoperitoneal shunt. The hydrocele resolved following revision of the shunt. Possible pathophysiological causes are discussed.

Enterovesical fistulas affect mainly male patients, due to the interposition of the uterus in women. They can be caused by neoplastic, inflammatory or traumatic processes. A case of ilovesical fistula due to migration of a silastic plate used to repair an epigastric infissional hernia is presented. This case has two distinctive features: first, the cause is extraurological, causing a fistula between both systems due to an inflammatory process. Secondly, we report the long distance travelled by the synthetic mesh, causing a peculiar clinical picture five years after its implantation.

A case is reported in which a surgical gastrostomy tube migrated into the duodenum and eroded through the jejunal wall into an adjacent loop of jejunum. This resulted in an entero-enteric fistula, small bowel obstruction and intussusception. Since the number of gastrostomy tubes being placed endoscopically and surgically is increasing, gastroenterologists and surgeons should be alerted to these and other potential complications.
