Skeletal tissues from children sustaining acute skeletal trauma were analyzed with detailed radiologic and histologic techniques to assess the failure patterns of the developing skeleton. In the physis- and epiphysis-specific fracture propagation varied, usually going through the portion of the hypertrophic zone adjacent to the metaphysis. However, the physeal fracture in types 1 and 2 sometimes involved the germinal zone. There may also be microscopic propagation at oblique angles from the primary fracture plane, splitting cell columns apart longitudinally. The cartilage canals supplying the

Although uncommon, the management of ipsilateral pilon and calcaneal fractures is a challenging problem for foot and ankle surgeons. There are several issues that may affect the outcome. Aside from damage to the soft-tissue envelope, technical issues such as positioning of the patient, application of traction, and tourniquet time may complicate surgical intervention. The authors present their experience with 2 patients who sustained this fracture pattern and highlight the mechanism, management, and possible complications of these injuries.

For some cases of canine appendicular osteosarcoma (OSA), limb-sparing treatment options are often desired, one of which is stereotactic body radiation therapy (SBRT). A major complication of SBRT is fracture of the irradiated bone at the site of treatment. The present study evaluated 127 appendicular OSA sites in 122 dogs treated with SBRT to identify the most common pathologic fracture locations and configurations. A total of 50 tumours experienced a pathologic fracture, and 38 had imaging sufficient to identify fracture configuration. The distal tibia was more likely to develop a fracture t

Traumatic injuries of the hip are an increasingly common cause of morbidity and mortality. These injuries can be grouped into fairly discrete patterns including femoral head fractures and hip dislocations, femoral neck fractures, greater trochanteric fractures, intertrochanteric fractures, subtrochanteric fractures, and soft tissue injuries. For each of these entities, specific features provide helpful diagnostic, prognostic, and therapeutic information. Femoral head fractures and hip dislocations commonly occur in combination. Fractures of the femoral head confer an increased risk of avascula

OBJECTIVE: The objective of our study was to describe the patterns of patellar fracture after total knee replacement and assess their clinical significance.
MATERIALS AND METHODS: A retrospective review of patellar fractures after total knee replacement was performed. Seventeen cases were identified over a 10-year period. Radiographs, radiology reports, medical records, and operative notes were reviewed and analyzed. The clinical analysis included the time to fracture, the patient's symptoms and signs, whether the extensor mechanism was disrupted, and the treatment administered for the fractu

PURPOSE: Intramedullary headless compression screw (IHCS) fixation of metacarpal (MC) fractures is popular. A concern for distal fractures is the potential loss of fragment/head fixation and with intra-articular incursion of hardware. We evaluated the biomechanical stability of IHCS fixation of progressively distal MC osteotomies using a cadaveric simulated active motion model.
METHODS: We included 12 matched-pair fresh-frozen cadaveric hands (median age, 73.5 years; 75% men), excluding matched-pair fingers with deformities/severe arthritis at the metacarpophalangeal joint or proximal interph

OBJECTIVE: To investigate current status and latest progress of clinical research on distal femoral fractures.
METHODS: The related literature was extensively reviewed to summarize the trend of the researches and their clinical application in the treatment of distal femoral fractures.
RESULTS: Distal femoral fractures are likely to occur in young people who suffer from high-energy damage and the elderly with osteoporosis, which is always comminuted and unstable fractures, and often involved in the articular surface and combined with serious soft tissue injury. Therefore, the treatment faces

CASE: A 5-year-old boy sustained a right elbow fracture dislocation with an associated displaced comminuted coronoid fracture. Acute closed reduction as well as an initial attempt at open repair and fixation failed. This case was salvaged 8 weeks after injury with coronoid reconstruction using an iliac crest allograft.
CONCLUSION: This is the first reported case of coronoid reconstruction using iliac crest allograft in a pediatric patient. 2.5 years after reconstruction, the elbow joint is stable, pain free, and demonstrates elbow motion well within the range for full function.

SYNBONE® spheres were impacted with 7.62 × 39 mm mild steel core ammunition at a mean impact velocity of 654 m/s, SD 7 m/s, to simulate engagement distances of around 50-100 m. The wounds and fracture patterns were assessed by two forensic pathologists familiar with military cranial injury. The overall fracture pattern was assessed as being too comminuted when compared with actual injury. This suggests the SYNBONE® spheres have less utility for simulating military injury than other purposes described in the literature.

Traumatic atlantoaxial rotatory fixation (AARF) with accompanying odontoid and C2 articular facet fracture is a very rare injury, and only one such case has been reported in the medical literature. We present here a case of a traumatic AARF associated with an odontoid and comminuted C2 articular facet fracture, and this was treated with skull traction and halo-vest immobilization for 3 months. After removal of the halo-vest immobilization, his neck pain was improved and his neck motion was preserved without any neurologic deficits although mild torticolis was still observed in closer inspectio

A cranial approach to the humerus was developed for application of a compression plate for repair of comminuted humeral fractures in a weanling foal, a calf, a heifer, and a mature cow. An incision was made from the cranial portion of the greater tubercle of the humerus to the radius. The cranial aspect of the humerus was exposed by retraction of the brachiocephalicus muscle and blunt dissection between the biceps brachii and brachialis muscles. A second plate was applied laterally in the heifer and cow by retraction of the brachialis muscle and elevation of the extensor carpi radialis muscle.

Authors report on experiences in using radius head prosthesis on the basis of early follow-ups. The operation is performed in cases of closed, comminuted fractures of the radial head if the fracture is irreponable on the basis of X-ray and intraoperative findings. Controls until now have shown excellent functional results. Based on literary data they describe the possibilities of late complications after using silicone prosthesis. The number of patients and the short time elapsed after operations does not allow far-reaching conclusions yet. A description of the use of radius head prosthesis in

INTRODUCTION: Soft tissue swelling after acute fracture surgery is a challenge which may increase wound dehiscence, delay early range of motion, and increase infection rate postoperatively. This study investigates closed incision negative pressure therapy (ciNPT) using wide-range cover over the incision site and the peripheral swelling trauma zone to promote early active motion and to mitigate joint stiffness, bulla formation, and tendon adhesion.
METHODS: Twenty-nine patients were enrolled between January 2018 and December 2018. Patients with high-energy soft tissue trauma and comminuted fra

Mandibular fractures are common maxillofacial injuries among children and adolescents, but treatment remains controversial. The aim of this study was to analyse the methods and outcomes of open reduction and internal fixation (ORIF) for non-condylar mandibular fractures in paediatric patients among 14 maxillofacial centres. Patients ≤16 years of age undergoing ORIF for non-condylar mandibular fractures between 2011 and 2022 were included. Age, gender, dentition stage, site and type of fracture, surgical approach, material, thickness, and number of plates, and outcome were recorded. 179 patient

Computed tomography is commonly used to evaluate patients with blunt facial trauma. With the high definition of the current scanners, even small fractures of the facial skeleton can be visualized. In complex midface injuries, it can be difficult for the radiologist to know which fractures are important to point out to the surgeon. An understanding of the anatomically relevant and surgically accessible craniofacial buttresses is critical for management of these injuries. Naso-orbitoethmoid fractures are classified according to the degree of injury to the medial canthal attachment. If the nasofr

Fractures of the thumb metacarpal occur most frequently at the base. These fractures can be subdivided into intra-articular and extra-articular types. Intra-articular fractures present treatment challenges because they have a tendency to displace due to deforming forces acting at the base of the thumb. An understanding of the anatomy, biomechanics, and fracture pattern will aid in deciding on the best treatment option for each fracture type. Surgical treatment is recommended for unstable fractures. Anatomic restoration of the articular surface in Bennett and Rolando fractures is not essential

Early anatomic (or near anatomic; lingers do not impinge or overlap during flexion or extension) stable fracture fixation provides the foundation for successful wound management and for the repair, reconstruction, and healing of all damaged tissues in a mutilating hand injury. It also plays an instrumental role in pain control and affords an optimal opportunity for timely and favorable rehabilitation of and recovery from mutilating injuries of the hand. Kirschner or other wiring systems or mini external fixators may be used for simple fractures, in children, when rapid fracture fixation is nec

Complications developed in a dog that underwent intramedullary pin and cerclage wire fixation of a comminuted femoral fracture. The fracture was unstable, the intramedullary pin protruded through the skin over the hip, and Staphylococcus sp was isolated from the fracture site. The loose pin was removed, and the fracture was restabilized with a 6.0-mm interlocking intramedullary nail and 3 interlocking screws. Cephalexin was given orally for 6 weeks. Eight weeks after the second surgery, the fracture was healed radiographically. Seventeen months later, the dog had a persistent mild lameness tha

BACKGROUND: Treatment of complex patella fractures represents a challenging clinical problem. Whereas controversy exists regarding the most appropriate fixation method, anterior plating is an emerging treatment option. A novel design of anterior variable-angle locking plates features radiating legs with integrated screw holes aiming at capturing fragments of the distal comminuted zone via either a locking or a cortical polar screw. The aim of this study was to compare the biomechanical performance of the novel anterior variable-angle locking plates used for fixation of complex patella fracture
