Cognitive Deficits in Executive Functions and Decision-Making Impairments Cluster Gambling Disorder Sub-types.

To identify Gambling Disorder (GD) subtypes, in a population of men seeking treatment for GD, according to specific executive function domains (i.e., cognitive flexibility, inhibition and working memory as well as decision making) which are usually impaired in addictive behaviors. A total of 145 males ranging from 18 to 65 years diagnosed with GD were included in this study. All participants completed: (a) a set of questionnaires to assess psychopathological symptoms, personality and impulsivity traits, and (b) a battery of neuropsychological measures to test different executive functioning do

Risk Factors for Loss to Follow-Up among People Who Inject Drugs in a Risk Reduction Program at Karachi, Pakistan. A Case-Cohort Study.

INTRODUCTION: Retention of male people who inject drugs (PWIDs) is a major challenge for harm reduction programs that include sterile needle/syringe exchange in resource-limited settings like Pakistan. We assessed the risk factors for loss to follow-up among male PWIDs enrolled in a risk reduction program in Karachi, Pakistan. METHODS: We conducted a prospective cohort study among 636 HIV-uninfected male PWIDs enrolled during March-June 2009 in a harm reduction program for the estimation of incidence rate. At 24 months post-enrollment, clients who had dropped out of the program were defined a…

Operationalizing recipient-centeredness: three facets of values-driven care in syringe services programs.

BACKGROUND: Syringe services programs (SSPs) offer critical, evidence-based services for people who use drugs (PWUD), including infectious disease and overdose prevention. SSPs operate from harm reduction and health equity frameworks, prioritizing non-coercive care and client autonomy; these values foster engagement among a community often stigmatized or discriminated against in traditional healthcare settings. As SSPs increasingly expand their clinical service offerings, they may encounter tensions among their core values, internal or funder-driven pressures for service expansion, and partner…

An interrupted time series analysis of fentanyl, naloxone, and opioid-involved deaths in five counties in Eastern Missouri.

INTRODUCTION: Rates of opioid overdose deaths (OOD) have increased since the introduction of illicitly manufactured fentanyl in the U.S. dru … Specifically, fentanyl and naloxone effects were only found in a high-need urban area with existing harm reduction and street outreach programs, but not in surrounding suburban or rural areas. State-level naloxone distribution decision-makers should prioritize funding harm reduction services in both urban and non-urban settings to help increase targeted naloxone …

Evaluation of a Residential Managed Alcohol Program for Aboriginal Peoples Experiencing Homelessness and Alcohol Dependence: Short-Term Impacts of an Australian Trial.

INTRODUCTION: Homelessness and alcohol dependence can be barriers to accessing essential services such as health care, housing, and social supports. Managed alcohol programs (MAP) have emerged as an effective harm reduction strategy for people experiencing homelessness and alcohol dependence. The aim of the present study was to evaluate the short-term impacts of the first MAP run in South Australia outside of COVID-19 restrictions and the first in Australia to be conducted in a healthcare setting. It was designed to be culturally appropriate for Aboriginal and Torres Strait Islander clients. …

Stable patterns, shifting risks: the impact of British Columbia's decriminalization and recriminalization policies on drug use behaviours.

BACKGROUND: Canada's historical reliance on criminal justice approaches to drug policy has intensified structural and social stigma, and high-risk behaviours among people who use drugs. In response to pressure from local advocates, British Columbia implemented a pilot decriminalization policy in January 2023, permitting adults to possess up to 2.5 g of specified unregulated substances, cumulatively. While not designed to address the toxic drug supply directly, it aimed to reduce stigma and encourage engagement with health and harm reduction services. In May 2024, however, drug possession in pu…

Preferred stimulant safer supply and associations with methamphetamine preference among people who use stimulants in British Columbia: Findings from a 2021 cross-sectional survey.

BACKGROUND: As safer supply programs expand in Canada, stimulant safer supply is often overlooked despite the harms and criminalization faced by people who use stimulants. METHODS: The 2021 Harm Reduction Client Survey was administered at 17 harm reduction sites around British Columbia, Canada. The survey included a question about what specific substance participants would want to receive as stimulant safer supply. We investigated preference of stimulant safer substance by looking at frequency of stated preference and by using multivariable logistic regression to understand factors associated…

Increase in drugs-of-abuse findings in post-mortem toxicology due to COVID-19 restrictions-First observations in Finland.

A lot has been published on the anticipated effects of the current COVID-19 pandemic on users of illegal drugs. In this study, we present evidence-based data on such effects, namely, the increased number of drug findings in post-mortem investigations. All post-mortem toxicology cases positive for at least one of the following: buprenorphine, amphetamine or cannabis, were investigated in the first 8 months of the year 2020, and the monthly numbers were compared to those in the previous 5 years from 2015 to 2019. These substances served as indicator analytes that could reveal changes in the drug

Adverse drug-related effects among electronic dance music party attendees.

BACKGROUND: Drug use is prevalent among electronic dance music (EDM) party attendees, but research is needed to determine the extent of adverse drug-related outcomes in this population in order to better inform prevention and harm reduction efforts. METHOD: 1029 adults were surveyed entering EDM parties in New York City in 2018. Those reporting past-year use of a drug were asked if they experienced a harmful or very unpleasant effect after use in which they were concerned about their immediate safety. They were also asked about co-use of other drugs and whether they sought help. RESULTS: We …

Heroin type, injecting behavior, and HIV transmission. A simulation model of HIV incidence and prevalence.

BACKGROUND AND AIMS: Using mathematical modeling to illustrate and predict how different heroin source-forms: "black tar" (BTH) and powder heroin (PH) can affect HIV transmission in the context of contrasting injecting practices. By quantifying HIV risk by these two heroin source-types we show how each affects the incidence and prevalence of HIV over time. From 1997 to 2010 PH reaching the United States was manufactured overwhelmingly by Colombian suppliers and distributed in the eastern states of the United States. Recently Mexican cartels that supply the western U.S. states have started to p

Willingness to use an in-hospital supervised inhalation room among people who smoke crack cocaine in Vancouver, Canada.

INTRODUCTION AND AIMS: People who use illicit drugs (PWUD) often engage in drug use during hospitalisation. Adverse outcomes may arise from efforts to conceal inpatient drug use, especially in hospital settings that rely on abstinence-based policies. Harm reduction interventions, including supervised drug consumption services, have not been well studied in hospital settings. This study examines factors associated with willingness to use an in-hospital supervised inhalation room (SIR) among people who smoke crack cocaine in Vancouver, Canada. DESIGN AND METHODS: Cross-sectional data from two o…
