This paper examines the concept of catastrophic experience, its relationship to the range of acute and prolonged stressors to which women may be exposed and the broad impacts on their mental health and well-being. It identifies catastrophe in terms of multiple accumulated stresses including death, loss, victimization, demoralization, shame, stigmatization, helplessness and identity. Catastrophic experiences include personal violence in domestic circumstances of intimate partner abuse, sexual assault and child physical and sexual abuse. Women's experiences of loss through the violent deaths of

Several authors have argued that traumatic experiences are processed and remembered in a qualitatively different way from neutral events. To investigate this issue, we interviewed 121 Croatian war veterans diagnosed with posttraumatic stress disorder (PTSD) about amnesia, intrusions (i.e., flashbacks and nightmares), and the sensory qualities of their most horrific war memories. Additionally, they completed a self-report scale measuring dissociative experiences. In contrast to what one would expect on the basis of theories emphasizing the special status of traumatic memories, amnesia, and high

Despite strong evidence of high rates of childhood and adult trauma in schizophrenia, the area remains under-researched. Our objectives in the study were first, to examine the rates of exposure to childhood, adult and lifetime (child plus adult) trauma in a population with schizophrenia and a population with non-psychotic psychiatric diagnoses and second, to examine the effect of trauma on the symptoms of schizophrenia. Two groups, those with schizophrenia (n = 40), and those with a non-psychotic diagnosis (n = 30), were recruited. Data were collected for demographic, psychiatric and trauma hi

OBJECTIVE: To study sex differences in health after stressful life events in the family.
METHOD: We examined the association between a serious event (death or severe illness of spouse, death or severe illness of other family member, death of close relative) and health among 6,095 male and 21,217 female public sector employees in Finland by repeated measures Poisson regression analysis with the generalized estimating equations method. The longitudinal data comprised self-reports of 3,556 events and their timing in 2000 or 2001, monthly sickness absences between 1997 and 2003, and psychiatric m

Manifestations of chronic shock and annihilation anxiety-including autistic defenses, chaotic relationships, disorganized attachment, split-off affective states, and vulnerability to disintegration--exist side by side with apparent ego strength and high functioning, even in nonabused patients. Chronic shock stemming from uncontained distress and failed dependency during childhood can persist throughout the lifespan, creating ripples of dysfunction that mask as character distortion and contribute to therapeutic impasse. Patients rely on omnipotent defenses to provide a sense of "having skin" in

The present study examined patterns of sexual assault and its psychological correlates among gay men, lesbians, and bisexual men and women (GLB). A community sample of 342 GLB individuals completed questionnaires assessing victimization history and psychological functioning. Nearly 63% of participants reported some form of sexual assault, and nearly 40% reported sexual revictimization, defined as contact or penetrative sexual assault reported in both childhood and adulthood. Gay men and bisexual men and women were more likely to report sexual revictimization than lesbians. Sexual victimization

This study evaluated the impact of maternal anxiety about a child's epilepsy on parental overprotection and the child's adaptive functioning. Specific maternal and family characteristics that contribute to elevated maternal anxiety about epilepsy were also studied over a year's time in a group of 56 mothers with children recently diagnosed with epilepsy. Overall, the primary predictor of maternal anxiety about epilepsy was the mother's level of coping resources, although family stress aggravated anxiety at the initial time point. Maternal anxiety about epilepsy was associated with overprotecti

UNLABELLED: Childhood-onset schizophrenia is rare: its prevalence is about 50 times lower than the one observed in adulthood. It is also frequently unrecognized, notably because its clinical aspect varies with age. The authors report the case of a prepubertal girl who developed a typical clinical picture of schizophrenia (paranoid subtype) by age 9.
CASE REPORT: The patient was 10 years old when she was hospitalized for a relapse of a suspected childhood-onset schizophrenia. Several significant mental disorders were found in her family history: her mother was treated for mood disorders (inclu

GENERALLY speaking, schizophrenia is not associated with diminished positive and negative emotions. Even patients with negative symptoms such as blunted affect have generally not differed in their levels of self-reported emotionality compared to patients without negative symptoms. However, there is evidence to suggest that a subgroup of patients with negative symptoms, those with the deficit syndrome, have a diminished capacity to experience positive and negative emotions. The present study examined whether ratings of the deficit and negative syndrome were associated with lower levels of self-

BACKGROUND: We evaluated for phobias the prediction of the stress-diathesis model that the magnitude of stress at onset is inversely proportional to the level of underlying diathesis.
METHODS: In more than 7500 twins from a population-based registry, we assessed the personality trait of neuroticism--as an index of phobia-proneness--and the lifetime histories of 5 phobia subtypes (agoraphobia, social, animal, situational, and blood or injury) and their associated irrational fears. Interviewers classified the mode of acquisition of the fear in phobic twins into 5 possible categories: trauma to

The purpose of this study was to explore, throughout the life cycle, the prevalence of self-harm behaviors among psychiatric inpatients with and without borderline personality disorder (BPD). Psychiatric inpatients with BPD (n = 43) were compared to those without BPD (n = 40) with regard to self-reported self-harm behaviors during their lifetime. The mean number of self-harm behaviors, including high-lethal behaviors (i.e., suicide attempts, cutting oneself, overdosing), among those with BPD dramatically increased between the ages of 18 and 24 years and was sustained through ages 50 to 59 year

Genital infection, a common chronic sexually transmitted infection, is associated with physical and psychosocial concerns. While recognizing that some patients are able to take advantage of medication to chronically suppress the virus, researchers have examined the role that stress and other variables play in the frequency of outbreaks a patient might experience. In this article, several theories are reviewed from the stress literature that relate to patients' experience with herpes: the adjustment issues associated with having herpes, the role of social support, and the issue of disclosure. We assessed the psychopathology of children and adolescents with Recurrent Abdominal Pain (RAP) and tension-type headaches (TTH), the psychopathology and Expressed Emotion (EE) of their mothers and family functioning. Additionally, we assessed the relationship of negative Life Events (LE) to RAP and headaches. Sixty-nine children and adolescents with either RAP or headaches, and their mothers were examined and compared to controls. Of the children with RAP or headache, 81.6% and 83.9% respectively carried a psychiatric diagnosis, primarily anxiety or depressive disorder, in contrast to 15% of

We asked patients with either panic disorder, social phobia, or major depressive disorder and healthy control participants to describe their most frightening experience and to describe an emotionally neutral experience. Both fear and neutral autobiographical memories were audiotaped and processed through a low-pass filter that eliminated frequencies above 400 Hz, thereby abolishing semantic content but leaving paralinguistic aspects like rate, pitch, and loudness intact, and these convey emotional cues. Raters blind to content and diagnosis rated the content-filtered speech clips on emotional

Little is known regarding the extent to which schools are implementing transition programming for students with severe mental retardation and other related issues: parent expectations of transition outcomes, postschool vocational outcomes, and parent involvement in the transition process. These issues were examined with 52 families who had transition age sons or daughters with severe mental retardation. Results revealed that (a) schools are providing a variety of instruction in the area of transition; (b) parents' ideal views of vocational outcomes are not always consistent with their realisti

Studies on adults have suggested important effects of stressful life events in provoking onset of psychiatric disorder. Only a few comparable studies on children exist, and their results are inconsistent in relation to definite timing effects. Meeting some important methodological challenges overlooked in the past research, this study set out to examine whether the onset of psychiatric disorder in children was more likely to occur shortly after a severe event, as compared with other times. The sample consisted of 99 consecutive, newly referred patients, aged 8-16 years, from a child psychiatry

OBJECTIVES: This study estimates the prevalence of stressful life events and physical abuse among North Carolina women prior to infant delivery, and examines potential associations between abuse and the other stressors.
METHODS: Data were from the North Carolina Pregnancy Risk Assessment Monitoring System, a statewide representative survey of over 2,600 postpartum women. The survey assessed women's sociodemographic characteristics and their experiences of physical abuse and 13 other stressful life events before delivery. The prevalences of each life event and abuse were estimated. Logistic re

CONTEXT: While low birth weight is the leading cause of infant mortality and morbidity, the factors influencing low birth weight are not well understood. In particular, the relationship between stressful life events and birth outcomes is unclear. It is important for health care providers to better understand the impact of stress on health outcomes.
METHODS: Data from a statewide case-control study of 2,378 Missouri mothers are used to examine the relationship of perceived stress, pregnancy attitudes and major life events as psychosocial risk factors on very low birth weight (i.e., birth weigh
