Physicians, AIDS, and occupational risk. Historic traditions and ethical obligations.

The profound reluctance of some physicians to care for patients with acquired immunodeficiency syndrome prompted us to review medical responses to other historic plagues. No consistent professional tradition emerged. Many physicians, including Galen and Sydenham, fled from patients with contagious epidemic diseases. Many of their colleagues, at considerable personal risk, remained behind to care for plague victims. This inconsistency suggests that an ethic stressing traditional professional duties may not be ideal for defining the optimal relation of the medical profession to patients with acq

Antibiotic prescribing practices of community health extension workers in East Central Uganda: an exploratory qualitative study.

BACKGROUND: Antibiotic misuse at the community level remains a critical contributor to antimicrobial resistance, particularly in resource-constrained settings. Community Health Extension Workers (CHEWs) play a pivotal role in primary healthcare delivery and antibiotic access, yet little is known about how they navigate prescribing decisions amid systemic limitations, social expectations and ethical pressures. OBJECTIVE: This study explored CHEWs' perceptions, practices and experiences related to antibiotic prescribing and dispensing in resource-constrained settings, with particular attention

Navigating complexity: nurses' experiences in delivering holistic care to patients with chronic obstructive pulmonary disease.

BACKGROUND: Chronic obstructive pulmonary disease (COPD) is a complex, progressive condition associated with substantial physical, psychological, and social burden. Holistic, person-centered care is central to effective COPD management, yet its delivery in outpatient settings remains challenging. Nurses play a pivotal role in providing such care; however, their lived experiences of delivering holistic COPD care within constrained health systems are insufficiently understood. AIM: To explore nurses’ experiences of delivering holistic care to patients with COPD in university-affiliated polyclini

Relational Well-Being in Non-WEIRD Societies: A Reformulation of Happiness From a Chinese Cultural Perspective.

Positive psychology has made substantial contributions to the scientific understanding of human flourishing, yet its conceptual architecture remains disproportionately grounded in WEIRD (Western, educated, industrialized, rich, democratic) populations. Prevailing models privilege intrapersonal processes-autonomy, self-efficacy, and personal achievement-while systematically under-theorizing the relational and moral dimensions through which happiness is experienced and evaluated in non-WEIRD societies. This article proposes a reformulation of happiness through the construct of relational well-be

Necropolitics of ıntimacy: family violence and trans lives in Türkiye.

This article examines the violence experienced by trans individuals in Türkiye within family and intimate relationships, treating it as a structured, enduring regime rather than a series of isolated acts. Drawing on semi-structured interviews with 17 trans individuals, the study uses reflexive thematic analysis to trace how violence originates within the family and extends into intimate relationships, public spaces, and institutional encounters. The analysis brings together Agamben's state of exception and bare life with Mbembe's necropolitics and politics of enmity to show how trans individua

Abortion law in the Crown Dependencies: liberalisation in the UK's small, independent neighbours.

There has been significant exploration of UK abortion law variation in recent years. However, this comparative research has not extended to the neighbouring Crown Dependencies. The Isle of Man, Guernsey, and Jersey operate in unusual constitutional contexts, with more autonomy than the UK's devolved nations. Whilst they often match UK policy in many areas, this is not automatic. In this article, we examine recent progress the Crown Dependencies have made on abortion regulation. Following a brief constitutional background and overview of UK abortion law, we examine the differing ways all three

Physical restraint in the intensive care units: a narrative review of current practices and outcomes.

BACKGROUND: Physical restraint remains common in the intensive care unit (ICU), but it is increasingly questioned following recent high‑quality evidence, particularly the R2D2‑ICU trial, which challenges the assumption that reducing restraint inherently improves short‑term neurological outcomes. MAIN BODY: Epidemiological data reveal significant variability in the prevalence of restraint use, which is influenced by a complex interplay of patient characteristics, caregiver factors, and systemic elements. The evidence on its effectiveness for mainly preventing the pulling out of tubes is questi

Challenges and Pearls of Evaluation and Treatment of Adolescents and Emerging Adults With Scrupulosity Obsessive-Compulsive Disorder.

Obsessive-compulsive disorder (OCD) affects approximately 1% to 2% of youth.1 Patients with OCD may present with varying degrees of insight into the excessiveness of their compulsive behaviors. This is certainly true among youth, of whom 20% to 45% are thought to have poor or absent insight.2 Scrupulosity OCD-characterized by intrusive guilt and fear about moral or religious wrongdoing-is associated with worse symptom severity and insight.3 This form of OCD involves obsessive fear about offending a deity or acting immorally, leading to compulsions like excessive praying, reassurance-seeking, a

The Knowledge of Neglect: Reflections of Mothers Experiencing Homelessness in Dublin on Social Care Interventions Involving Their Children.

This paper examines the reflections on caregiving provided by a group of Irish mothers who were homeless, struggling with addiction, and had lost primary custody of their children. Drawing on Das's (Slum acts. Polity Press, Cambridge, 2022) discussion of 'inordinate knowledge', or knowledge of real-life and morally dubious scenarios that resist attempts at resolution through philosophical reasoning, the paper explores how women in these situations grappled with the realisation of having neglected their children. I demonstrate my interlocutors' ongoing ethical self-evaluations and continued att

Workforce impact of emergency department boarding.

INTRODUCTION: Emergency department (ED) boarding, or holding admitted patients in the ED due to a lack of available inpatient beds, presents a major challenge to healthcare systems. This study examines the impacts of ED boarding on clinician wellness, burnout, moral injury, and workplace violence. METHODS: We distributed a survey to members of the American Academy of Emergency Medicine over 4 weeks in early 2025. It included 9 questions on experiences with boarding, demographics, and practice setting, plus an optional free-response question. Descriptive statistics were performed, and free-res

Psychological Group Interventions for Reducing Distress Symptoms in Healthcare Workers: A Systematic Review.

Healthcare workers exposed to emergencies and chronic stressors are at high risk of developing mental health problems. This review synthesized existing studies of group psychological therapy to reduce distress symptoms in healthcare workers (i.e., as complex and heterogeneous emotional states, characterized by the presence of symptoms associated with post-traumatic stress disorder, burnout, anxiety, depression and moral injury). Searches were conducted using PRISMA guidelines and databases such as PubMed, PsycINFO, Medline and Web of Science, along with manual searches of reference lists of re

Did Alexander Fleming Deserve the Nobel Prize?

Penicillin is a serendipitous discovery par excellence. But, what does this say about Alexander Fleming's praiseworthiness? Clearly, Fleming would not have received the Nobel Prize, had not a mould accidently entered his laboratory. This seems paradoxical, since it was beyond his control. The present article will first discuss Fleming's discovery of Penicillin as an example of moral luck in science and technology and critically assess some common responses to this problem. Second, the Control Principle that says that people are not responsible for things beyond their control will be defended.

The neural correlates of justified and unjustified killing: an fMRI study.

Despite moral prohibitions on hurting other humans, some social contexts allow for harmful actions such as killing of others. One example is warfare, where killing enemy soldiers is seen as morally justified. Yet, the neural underpinnings distinguishing between justified and unjustified killing are largely unknown. To improve understanding of the neural processes involved in justified and unjustified killing, participants had to imagine being the perpetrator whilst watching 'first-person perspective' animated videos where they shot enemy soldiers ('justified violence') and innocent civilians (

An alternative approach to the prevention of doping in cycling.

BACKGROUND: Framed by an overly reductionist perspective on doping in professional cycling as an individual moral failing, anti-doping policies tend to envisage a combination of education and repression as the primary intervention strategies. We offer an alternative approach, which seeks to understand doping practices as embedded in social relations, especially in relation to team organisation and employment conditions. METHODS: We undertake an in-depth analysis of the functioning of nine of the 40 world professional cycling teams, and the careers of the 2,351 riders who were or have been pro

Attitudes towards nicotine, alcohol and drug dependence among physicians in Israel.

Prevalence of substance use and substance use disorders in Israel is similar to those in other developed countries. The aim of this study was to examine attitudes of physicians in Israel towards nicotine, alcohol and drug use and dependence. A national sample of physicians from different fields of medical specialty (n = 208, response rate 26%) responded to a 50-item questionnaire. Questions included general questions regarding substance use and addictions, as well as specific questions focusing on nicotine, alcohol, cannabis and heroin use. The poor response rate in this survey dictates cautio

[Assessment of sociocognitive functions in neurological patients Presentation of a French adaptation of two tools and implementation in frontal dementia].

This paper presents a French battery designed to assess emotional and sociocognitive abilities in neurological patients in clinical practice. The first part of this battery includes subtests assessing emotions: a recognition task of primary facial emotions, a discrimination task of facial emotions, a task of expressive intensity judgment, a task of gender identification, a recognition task of musical emotions. The second part intends to assess some sociocognitive abilities, that is mainly theory of mind (attribution tasks of mental states to others: false believe tasks of first and second orde

[PSYCHOLOGICAL FEATURES IN CHILDREN AND ADOLESCENTS WITH RESPIRATORY TUBERCULOSIS].

The development of a biopsychosocial model of disease acquires a great practical value for modern phthisiology, which presupposes an in-depth study of the psychological and social constituents of disease as important factors of therapy optimization and an increase in the efficiency of the rehabilitative measures being implemented. The problem of psychological causes and factors associated with the occurrence of a tuberculous process in children and adolescents has been inadequately developed in phthisiology. A psychodiagnostic study was conducted in 90 patients from Moscow pediatric tuberculos

Prenatal diagnosis--discrimination, deliverance or democracy?

Prenatal diagnosis utilizes invasive procedures such as amniocentesis, chorionic villus sampling, cord blood sampling and pre-implantation genetic diagnosis. These techniques can diagnose serious foetal illnesses and this therefore provides valuable information to couples, helping them to prepare for the birth of an affected child. It also affords women the freedom to decide whether to terminate a pregnancy. The selective termination of foetuses with serious disabilities does not represent disability discrimination because women and parents are actually rejecting the disability, rather than th

Triage ethics in mass casualty incident simulation: A phenomenological exploration.

BackgroundDisaster scenarios challenge both novice and experienced nurses to navigate complex ethical dilemmas in resource-limited environments. Traditional nursing education often leaves new nurses unprepared for the ethical demands of disaster nursing. Utilitarianism must often guide triage ethics and decision-making. There is a critical need to equip nursing students with these ethical competencies.Research question/AimThis study explores nursing students' lived experiences using introductory triage ethics in mass casualty incident simulation (MCIS).Research designA qualitative, interpretiv
