Tonic pupil and orbital glial-neural hamartoma in infancy.

PURPOSE/METHODS: Tonic pupils in early childhood are rare. We studied an otherwise healthy 31/2-month-old girl who had a right pupil that was poorly reactive to light, without other signs of oculomotor nerve palsy. RESULTS/CONCLUSIONS: Constriction of the right pupil after instillation of 0.125% pilocarpine eyedrops confirmed denervation hypersensitivity, consistent with a tonic pupil. There was no strabismus or proptosis. A magnetic resonance imaging scan demonstrated a right orbital mass, interposed between the lateral and inferior recti muscles. Biopsy was consistent with a benign, glial-n

[Post-polio syndrome with isolated dysphagia].

The case of a 69 year old, male patient with slowly progressive dysphagia and an acute exacerbation is presented. He was referred to rule out an organic origin to his disorder. Apart from mild residual weakness of both legs after an acute paralytic poliomyelitis at the age of 20, the physical examination was essentially normal. No upper motor neuron pathology was found. Electromyography showed prevalent spontaneous and chronic denervation of bulbar and skeletal muscles. X-ray cinematography revealed abnormalities of bolus passage. On the basis of history and the benign clinical course over a 2

[Recurrent amyotrophic neuralgia with an unusual clinical expression. Clinical and electrophysiological study].

Neuralgic amyotrophy (NA) is an entity with great variability in its neurological expression. The patient reported here developed symptoms of NA one year before admission, with anterior interosseous nerve palsy. At the time of examination she reports pain in the left upper limb followed by sensory loss over lateral antebrachial cutaneous nerve. Electrophysiologic examination showed signs of chronic denervation in the right anterior interosseous nerve muscles, and reduced amplitude of the sensory nerve action potential of the left lateral antebrachial cutaneous nerve. This case confirms the var

Muscle-specific lack of Gfpt1 triggers ER stress to alleviate misfolded protein accumulation.

Pathogenic variants in GFPT1, encoding a key enzyme to synthesize UDP-N-acetylglucosamine (UDP-GlcNAc), cause congenital myasthenic syndrome (CMS). We made a knock-in (KI) mouse model carrying a frameshift variant in Gfpt1 exon 9, simulating that found in a patient with CMS. As Gfpt1 exon 9 is exclusively expressed in striated muscles, Gfpt1-KI mice were deficient for Gfpt1 only in skeletal muscles. In Gfpt1-KI mice, (1) UDP-HexNAc, CMP-NeuAc and protein O-GlcNAcylation were reduced in skeletal muscles; (2) aged Gfpt1-KI mice showed poor exercise performance and abnormal neuromuscular junction

FKBP25 regulates myoblast viability and migration and is differentially expressed in in vivo models of muscle adaptation.

FKBP25 (FKBP3 gene) is a dual-domain PPIase protein that consists of a C-terminal PPIase domain and an N-terminal basic tilted helix bundle (BTHB). The PPIase domain of FKBP25 has been shown to bind to microtubules, which has impacts upon microtubule polymerisation and cell cycle progression. Using quantitative proteomics, it was recently found that FKBP25 was expressed in the top 10% of the mouse skeletal muscle proteome. However, to date there have been few studies investigating the role of FKBP25 in non-transformed systems. As such, this study aimed to investigate potential roles for FKBP25

Arytenoid abduction for bilateral vocal fold immobility.

PURPOSE OF REVIEW: The pathophysiology of bilateral vocal fold immobility includes two broad categories: mechanical fixation and neurogenic paralysis. A mobile arytenoid can be surgically abducted, and this procedure has been reported as a treatment for patients with bilateral neurogenic laryngeal paralysis. This article reviews the theoretical basis and clinical outcomes of this procedure. RECENT FINDINGS: Two concepts form the theoretical basis for arytenoid abduction. First, in most cases of neurogenic paralysis, laryngeal muscles are not denervated; there is considerable residual or regen

Ciliary neurotrophic factor-induced sprouting preserves motor function in a mouse model of mild spinal muscular atrophy.

Proximal spinal muscular atrophy (SMA) is caused by homozygous loss or mutation of the SMN1 gene on human chromosome 5. Depending on the levels of SMN protein produced from a second SMN gene (SMN2), different forms of the disease are distinguished. In patients with milder forms of the disease, type III or type IV SMA that normally reach adulthood, enlargement of motor units is regularly observed. However, the underlying mechanisms are not understood. Smn(+/-) mice, a mouse model of type III/IV SMA, reveal progressive loss of motor neurons and denervation of motor endplates starting at 4 weeks

Prior poliomyelitis: an immunohistochemical study of cytoskeletal proteins and a marker for muscle fibre regeneration in relation to usage of remaining motor units.

Patients with prior poliomyelitis with paresis and excessive use (n = 8) and low use (n = 6) of residual anterior tibial motor units (MUs) during walking were subjected to muscle biopsy of the anterior tibial muscle (TA). Antibodies directed against cytoskeletal proteins, spectrin and desmin, and against Leu-19, a myoblast and satellite cell related antigen, were applied. In the patients with excessive use of residual MUs there was an almost total predominance of hypertrophic type I fibres. In the hypertrophic fibres, staining for spectrin and desmin was normal while staining for Leu-19 was se

Sesn2 is Associated with Attenuated Muscle Atrophy and Altered Expression of Key Myogenic and Autophagy Markers in Mdx Mice.

Duchenne muscular dystrophy (DMD) is a common lethal neuromuscular disorder which is characterized by progressive skeletal muscle atrophy. Despite the beneficial role of Sestrin2 (Sesn2) in improving denervation-induced skeletal muscle atrophy, the effect of Sesn2 on the skeletal muscle of DMD remains largely unknown. To regulate the expression of Sesn2, we systemically modulated its expression in mdx mice via tail-vein injection of AAV9 vectors. The tibialis anterior (TA) muscles were subsequently harvested for analysis by immunofluorescence and Western blotting to assess myofiber morphology

CGRP+ fibers sprout within gastrocnemius muscle following complete spinal cord injury in rodents.

Following spinal cord injury nociceptive afferents exhibit functional and anatomical plasticity within the spinal dorsal horn. This plasticity is, at least in part, maladaptive, contributing to secondary complications related to neuropathic pain, autonomic dysreflexia, and spasticity. While previous studies have shown that nociceptors, particularly CGRP+ C-fibers, are also capable of sprouting peripherally following pathologies of chronic inflammation, muscle disuse, and denervation, the peripheral plasticity of C-fibers following spinal cord injury remains largely uninvestigated. Here, we sho

Physiological mechanisms of neuromuscular impairment in diabetes-related complications: Can physical exercise help prevent it?

Diabetes mellitus is a chronic disorder that progressively induces complications, compromising daily independence. Among these, diabetic neuropathy is particularly prevalent and contributes to substantial neuromuscular impairments in both types 1 and 2 diabetes. This condition leads to structural damage affecting both the central and peripheral nervous systems, resulting in a significant decline in sensorimotor functions. Alongside neuropathy, diabetic myopathy also contributes to muscle impairment and reduced motor performance, intensifying the neuromuscular decline. Diabetic neuropathy typic

Myosin VI localization and expression in striated muscle pathology.

Myosin VI (MVI) is a unique unconventional myosin translocating, unlike other myosins, towards the minus end of actin filaments. It is involved in numerous cellular processes such as endocytosis, intracellular trafficking, cell migration, and transcription. In mammalian skeletal muscles it localizes mainly to sarcoplasmic reticulum and is also present within the muscle nuclei and at the neuromuscular junction (Karolczak et al. Histochem Cell Biol 2013; 23:219-228). We have also shown that in denervated rat hindlimb muscle the MVI expression level is significantly increased and its localization

Paravertebral muscles in disease of the cervical spine.

OBJECTIVES: Cervical spine disorders are common in the older population. The paravertebral muscles are essential to the support and stabilisation of the cervical spine but have been little studied. The aim was to determine whether pathological changes develop in these muscles in patients with severe cervical spine disease, which, if present, might contribute to the pathogenesis and symptomatology of their disorder. METHODS: Open biopsies of superficial and deep paravertebral muscles were obtained during the course of surgical procedures to alleviate cervical myelopathy. Most of these patients

Effects of the Number of Muscle-Nerve-Muscle Grafts on Rat Facial Nerve Functional Recovery.

INTRODUCTION: Facial nerve denervation can be devastating for patients. Primary neurorrhaphy and interposition (IP) nerve grafting are common reinnervation techniques. Muscle-nerve-muscle (MNM) grafting is a lesser known alternative. After previously demonstrating significant return of function using MNM grafting in a rat facial nerve model, the authors compare the use of multiple MNM nerve grafts with that of single MNM and IP nerve grafts. METHODS: Thirty-six male rats were randomized into 4 groups: (1) repair with IP grafts, (2) 1 MNM graft, (3) 3 MNM grafts, and (4) control with no interv

Pre- and postjunctional actions of hydralazine in vascular and nonvascular smooth muscle in vitro.

This study investigated the in vitro prejunctional and postjunctional actions of hydralazine in vascular and nonvascular smooth muscle. Low concentrations (micromolar) of hydralazine blocked phenylephrine-induced increases in perfusion pressure in the innervated rat kidney, whereas high concentrations (greater than 10 microM) were required in the perfused, innervated rabbit ear artery. High concentrations of hydralazine were required to block phenylephrine-induced contractions of innervated rat vas deferens and anococcygeus muscle. After in vitro denervation, rabbit ear arteries became sensiti

Selective neurectomy for nonflaccid facial palsy.

PURPOSE OF REVIEW: To review the evolving role of selective neurectomy in the management of patients with synkinesis including the history of selective neurectomy, operative techniques, and clinical outcomes. RECENT FINDINGS: Modified selective neurectomy alone or in conjunction with other procedures achieves more durable outcomes based on objective measures such as time to recurrence of symptoms and units of botulinum toxin required postoperatively. This is also reflected on patient reported quality of life outcome measures. Regarding operative technique, lower rates of oral incompetence are

Reflex co-ordination of corporal and antral contractions in the conscious dog.

This study characterizes the role of extrinsic nerves in the co-ordination of corporal and antral contractions in the dog. Fasting motor activity was recorded in conscious dogs with stomachs previously divided into separate corporal and antral pouches. Both corpus and antrum showed synchronized phases of activity and quiescence recognizable as migrating motor complexes (MMCs, duration 81.2 +/- 9.6 min, n = 4). Moreover, individual contractions were temporally linked such that corpus contractions, occurring at 76 +/- 4 s intervals, were each followed by a burst of one to three antral contractio
