We present the case of a 53-year-old Caucasian woman with seven basal cell carcinomas and one malignant melanoma in situ along her back overlying her spine, which was irradiated in 1968 for ankylosing spondylitis. These lesions developed between 1997 and 1999. She has no other known risk factors for cutaneous malignancy, in particular no history of excessive sun exposure. She has skin type 2. IMPORTANCE: Keratinocyte carcinomas are skin cancers that arise from keratinocytes and are composed of basal cell carcinomas (BCCs) and cutaneous squamous cell carcinomas (cSCCs). The combination of multiple sessions of Photodynamic therapy (PDT) and surgery have been used to treat an extensive superficial lesion of squamous cell carcinoma in situ. Methyl aminolevulinate (MAL) mediated PDT was applied to reduce the tumoral area and a small surgical removal was performed to complete elimination of the lesion. One and a half years after heart transplantation an Australian man developed his first skin cancer. In the period until his death, 7 years after the transplantation, 34 histologically confirmed squamous cell carcinomas developed, 17 intra-epidermal carcinomas, and 9 basal cell carcinomas. Most skin cancers were confined to chronically sun exposed sites. Exposure to sunlight and human papillomavirus are important factors in the development of skin cancer in renal transplant recipients, and further studies are needed to establish the role of these risk factors in cardiac transplant recipients.

This preliminary report suggests that cryosurgery has a definite place in the management of certain external periocular and ocular problems.
