Common peroneal nerve palsy after grade I inversion ankle sprain.

This case report presents a 28-year-old man with foot drop 48 hours after a grade I inversion ankle sprain. Clinical examination and electrodiagnostic studies showed common peroneal nerve palsy. The patient was managed with conservative treatment and rehabilitation and recovered completely 4 months after the injury. Physicians should be aware of the possibility of delayed peroneal nerve injury after grade I ankle sprain. Function of the peroneal nerve should be evaluated in all patients with inversion ankle sprain as part of initial and follow-up evaluations. Early electrodiagnostic studies ar

The origin of the premotor potential recorded from the second lumbrical muscle in normal man.

OBJECTIVE: When recording with a palm electrode, a premotor potential precedes the compound muscle action potential (CMAP), evoked from the second lumbrical (2L) muscle following median nerve stimulation. The purpose of this study was to determine the origin of the premotor potential from the 2L. METHODS: We recorded potentials with multi-channel electrodes in the palm and finger in a bipolar or referential manner, stimulating the second digit or median nerve at the wrist. RESULTS: We recorded the traveling nearfield sensory nerve action potential (SNAP) and stationary negative potential in

Diagnosis of sporadic transthyretin Val30Met familial amyloid polyneuropathy: a practical analysis.

Transthyretin (TTR) Val30Met-associated familial amyloid polyneuropathy (FAP ATTR Val30Met) is the most common form of FAP and is now prevalent in areas other than those seen within conventional endemic foci. We investigated 15 patients with FAP ATTR Val30Met without a family history of FAP who were referred for sural nerve biopsy. Initial symptoms included somatic neuropathy in all patients, while sensory dissociation and autonomic symptoms were apparent only in two and seven patients, respectively. Nonspecific neuropathic features and slight abnormalities in cerebrospinal fluid protein level

Brachial neuritis masquerading as acute coronary syndrome.

BACKGROUND: Brachial neuritis is a frequently misdiagnosed condition which can present to many medical or surgical specialties. OBJECTIVE: To report a case of brachial neuritis with bilateral phrenic nerve involvement and diaphragmatic weakness. CASE DESCRIPTION: A 63-year-old man presented with acute-onset proximal upper extremity pain and weakness. He also developed severe orthopnoea. Examination revealed proximal upper limb wasting and dramatic paradoxical breathing. Cardiac investigations were unremarkable. Electromyographic studies were consistent with a C5 radiculopathy. Phrenic nerve

Reversible demyelinating neuropathy associated with renal cell carcinoma.

Paraneoplastic neuropathy is a potential complication of renal cell carcinoma. The clinical and electrophysiologic features of such patients have not been well characterized. We describe a patient with a demyelinating neuropathy associated with papillary renal cell carcinoma that resolved following nephrectomy. The literature is reviewed with particular attention to the clinical, electrophysiologic, and kidney histopathologic characteristics of similar patients. Features arguing against other acquired demyelinating neuropathies are highlighted, and we conclude that abdominal imaging may provid

Familial, demyelinating sensory and motor polyneuropathy with conduction block.

Both multifocal, demyelinating features and prednisone responsiveness are rare in Charcot-Marie-Tooth (CMT) disease. We report a mother and son with a prednisone-responsive, multifocal, demyelinating, predominantly sensory polyneuropathy that was associated with an isoleucine92valine polymorphism of lipopolysaccharide-induced TNF-alpha factor (LITAF). The mother had a multifocal, acquired, demyelinating sensory and motor polyneuropathy (MADSAM)-like presentation. The son developed left peroneal neuropathy during acute Lyme disease with a subsequent relapsing, MADSAM-like illness, despite antib

[Case of pure motor Guillain-Barré syndrome with motor conduction block and anti-GM1/GalNAc-GD1a antibody].

A 38-year-old man presented with distal-dominant limb weakness two weeks after an upper respiratory infection. He had no sensory and autonomic signs and no cranial nerve involvement during the course of the disease. Tendon reflexes were preserved except for an absent Achilles' tendon reflex. His disability at nadir was grade 2 on the Hughes functional scale. Cerebrospinal fluid analysis showed albuminocytologic dissociation and he was diagnosed with pure motor Guillain-Barré syndrome (GBS). Thin-layer chromatography immunostaining and an enzyme-linked immunosorbent assay revealed an immunoglob

Residual neurovascular function and retinotopy in a case of hemianopia.

INTRODUCTION: For occipital cortex strokes resulting in vision disorders, questions about the viability of residual visual cortex remain. CLINICAL PICTURE: In a patient with a one-year-old, left, complete, homonymous hemianopia due to a right, posterior cerebral artery, ischaemic infarct, we assessed the visual cortex with fMRI retinotopic mapping prior to starting vision restoration therapy. OUTCOME: The patient was found to have residual neurovascular function and retinotopic representation in the surviving visual cortex around the infarcted area. CONCLUSION: The ability to respond to sti

[Sensory neuropathy and autoimmune diseases].

If a sensory neuropathy appear from an autoimmune disease, it was important to precise the type with clinical and electrophysiological characteristic features (distal symmetric polyneuropathy, chronic inflammatory demyelinating polyneuropathy, Sensory mononeuritis multiplex, sensory neuronopathy, small-fiber neuropathy). The type of neuropathy have an influence on the complementary explorations. It was very important to determine the mechanism between the neuropathy and the autoimmune disease for the treatment. We present a young patient with systemic lupus erythemathosus and sensory neuropath

Electrophysiology in demyelinating polyneuropathies.

Demyelinating neuropathies are disorders of the peripheral nervous system in which the myelin sheath of axons is affected by immune-mediated or genetically determined processes. In single axons, demyelination yields conduction block due to extinction of action potentials or conduction slowing. Motor nerve conduction studies assess these phenomena in nerves, which consist of many axons. It is important to distinguish demyelinating from axonal polyneuropathies as this allows accurate diagnosis and institution of appropriate treatment. Immunological treatment in acquired demyelinating polyneuropa

Development of Isaacs' syndrome following complete recovery of voltage-gated potassium channel antibody-associated limbic encephalitis.

Autoantibodies against voltage-gated potassium channels (VGKC-Abs) are associated with acquired neuromyotonia (Isaacs' syndrome) and related disorders such as Morvan's syndrome and some cases of limbic encephalitis. The mechanisms underlying the various phenotypes induced by VGKC-Abs are not fully understood. Recently, we reported a case of LE with VGKC-Abs accompanied by severe intestinal pseudo-obstruction and thymoma. Thymectomy and immunosuppressive therapy induced dramatic clinical improvement of LE symptoms, and VGKC-Abs titers decreased from 1254 pM to 549 pM (normal>100 pM). Seventeen

Unilateral diaphragmatic paralysis and segmental motor paresis following herpes zoster.

We report the case of a 73-year-old woman who complained of acute onset of pain and weakness of her left shoulder and proximal arm muscles 3 weeks after a diagnosis of herpes zoster. Electromyography revealed involvement of the C5-6 myotomes and the upper trunk of the brachial plexus. Chest X-ray and electromyographic studies documented paralysis of the left diaphragm. One year after onset, muscle strength returned to normal, but radiographic and electrophysiologic findings of diaphragm paralysis were unchanged.

Response inhibition and impulsivity in schizophrenia.

Response inhibition in schizophrenia remains controversial, with behavioral correlates largely unknown. Inpatients with schizophrenia and controls completed a stop task and an impulsiveness questionnaire. Slower inhibitory processes were evident in schizophrenia, but there was no association with impulsivity. The nature of inhibition and impulsivity in schizophrenia is complex, and could reflect schizophrenia subgroups or disease states.

Tibial and peroneal nerve conduction studies in ankle sprain.

BACKGROUND: Lower limb injuries and ankle sprain occur frequently among football players, due to the high incidence of physical contact. As a result, lower limb nerves are subject to injury. This project aims to evaluate the lower limb nerve conduction among students playing football to investigate the probable relationship between ankle sprain in football and nerve conductivity. MATERIALS AND METHODS: Fifty volunteer students aged between 19 and 25 were studied. They fell into three groups; one comprising of 20 healthy football players whose football experience exceeded three years, the seco

Facial nerve conduction after sclerotherapy in children with facial lymphatic malformations: report of two cases.

Surgical excision is thought to be the standard treatment of choice for lymphatic malformations. However, when the lesions are limited to the face only, surgical scar and facial nerve injury may impair cosmetics and facial expression. Sclerotherapy, an injection of a sclerosing agent directly through the skin into a lesion, is an alternative method. By evaluating facial nerve conduction, we observed the long-term effect of facial lymphatic malformations after intralesional injection of OK-432 and correlated the findings with anatomic outcomes. One 12-year-old boy with a lesion over the right-s

Varied electrophysiologic patterns in spinocerebellar ataxia type 2.

The autosomal dominant ataxias are a heterogenous group of disorders. Almost 30 different genetic loci have been identified. Spinocerebellar ataxia type 2 (SCA2) is one of many autosomal dominant cerebellar ataxias. Electrophysiologic studies in SCA2 have shown mainly a sensory neuropathy or neuronopathy. To determine if electrophysiologic testing reveals concomitant or isolated involvement of motor nerves in SCA2 we reviewed historic and electrophysiologic data on all cases of genetically confirmed SCA2 who underwent nerve conduction studies and needle electromyographic during initial evaluat
