Ebola Virus Disease and Pediatric Mental Health: Long-Term Mediating Effects of Caregiver Mental Health.

OBJECTIVE: Previous research suggests that adult survivors of Ebola virus disease (EVD) experience mental health problems following their infection. This study examines long-term mental health consequences of EVD for child survivors, and those affected by EVD but not themselves infected, compared to controls. This study also examines the mediating effects of caregiver mental health on children's mental health outcomes. METHOD: Using a case-control study design, we recruited 663 children and adolescents ("children" throughout; aged 10-17 years) and their caregivers at 2 time points, from 6 geo

Implementation of an evidence-based tobacco control intervention for school teachers in India: Evaluating the effects of a capacity-building strategy.

BACKGROUND: Tobacco-Free Teachers, Tobacco-Free Society (TFT-TFS) is an evidence-based intervention that promotes tobacco use cessation among teachers and tobacco control policies among schools in India. This study tested an implementation model to build Bihar Department of Education (DOE) capacity to support and deliver TFT-TFS within schools, leveraging DOE training infrastructure. METHOD: We used a training-of-trainers (TOT) "cascade" implementation strategy to embed the TFT-TFS program into the Bihar DOE infrastructure. We trained 46 Cluster Coordinators to train and support Headmasters t

Considerations for shared decision management in previously untreated patients with hemophilia A or B.

UNLABELLED: Recent advances in therapeutics are now providing a wide range of options for adults and children living with hemophilia. Although therapeutic choices are also increasing for the youngest individuals with severe disease, challenges remain about early management decisions, as supporting data are currently limited. Parents and healthcare professionals are tasked with helping children achieve an inclusive quality of life and maintain good joint health into adulthood. Primary prophylaxis is the gold standard to optimize outcomes and is recommended to start before 2 years of age. A rang

Sexual differences in neuronal and synaptic properties across subregions of the mouse insular cortex.

BACKGROUND: The insular cortex (IC) plays a pivotal role in processing interoceptive and emotional information, offering insights into sex differences in behavior and cognition. The IC comprises two distinct subregions: the anterior insular cortex (aIC), that processes emotional and social signals, and the posterior insular cortex (pIC), specialized in interoception and perception of pain. Pyramidal projection neurons within the IC integrate multimodal sensory inputs, influencing behavior and cognition. Despite previous research focusing on neuronal connectivity and transcriptomics, there has

Coexisting conditions and concomitant medications do not affect venetoclax management and survival in chronic lymphocytic leukemia.

BACKGROUND: The question of which parameters may be informative on venetoclax outcome in chronic lymphocytic leukemia (CLL) is still unclear. Furthermore, the choice to treat with venetoclax can be challenging in patients with baseline characteristics or comorbidities that may potentially favor some specific adverse events. OBJECTIVES: This study was aimed to evaluate whether age, fitness status, patients'/disease characteristics, or concomitant medications may predict outcomes in CLL patients receiving venetoclax. DESIGN: Retrospective observational study. METHODS: Impact of age, presence

S1P receptor modulators and the cardiovascular autonomic nervous system in multiple sclerosis: a narrative review.

UNLABELLED: Sphingosine 1-phosphate (S1P) receptor (S1PR) modulators have a complex mechanism of action, which are among the most efficient therapeutic options in multiple sclerosis (MS) and represent a promising approach for other immune-mediated diseases. The S1P signaling pathway involves the activation of five extracellular S1PR subtypes (S1PR1-S1PR5) that are ubiquitous and have a wide range of effects. Besides the immunomodulatory beneficial outcome in MS, S1P signaling regulates the cardiovascular function via S1PR1-S1PR3 subtypes, which reside on cardiac myocytes, endothelial, and vasc

Prescription for COVID-19 by non-medical professionals during the pandemic in Colombia: a cross-sectional study.

BACKGROUND: The COVID-19 pandemic has led to an increase in the behavior of self-medication (SM). Given the massive release of misleading information during the pandemic, some pharmacies recommend drugs such as ivermectin, azithromycin, and hydroxychloroquine that are not useful for preventing or treating COVID-19 and could expose patients to unnecessary adverse drug reactions (ADRs), drug-drug interactions (DDIs), disease masking, and antibiotic resistance. RATIONALE: SM with drugs advertised for COVID-19 can have consequences, and people should be aware of approved uses, potential contraind

Future projections of opioid use and cost in patients with chronic osteoarthritis pain in Spain.

BACKGROUND: Opioids are widely used in moderate-to-severe chronic pain which is non-responsive to standard analgesics. Prescriptions have increased in Europe in the last decade, although remain lower than in USA. This work projected the future utilization and costs of opioids in chronic osteoarthritis (OA) pain in the Spanish National Health System (NHS). METHODS: An epidemiological model was populated with the opioid dispensing trends from 2010 to 2019 using Spanish Medicinal Agency rates of opioid utilization in subjects over 18 years of age and the real-world OPIOIDS study to estimate chro

Patient harm from cardiovascular medications.

BACKGROUND: Medication harm can lead to hospital admission, prolonged hospital stay and poor patient outcomes. Reducing medication harm is a priority for healthcare organisations worldwide. Recent Australian studies demonstrate cardiovascular (CV) medications are a leading cause of harm. However, they appear to receive less recognition as 'high risk' medications compared with those classified by the medication safety acronym, 'APINCH' (antimicrobials, potassium, insulin, narcotics, chemotherapeutics, heparin). Our aim was to determine the scale and type of medication harm caused by CV medicati

Whole population cohorts vs sampled comparators designs for evaluating health and educational outcomes of children with inborn rare conditions: a simulation study.

OBJECTIVES: Linked administrative data covering whole populations are fundamental resources for longitudinal studies of children with rare conditions (cases) and unaffected peers (comparators). Data minimization regulations sometimes limit the number of sampled comparators per case (hereafter abbreviated SCn) for research, with unknown impact on study findings. STUDY DESIGN AND SETTING: Using Monte-Carlo draws, we simulated data from 100,000 children with and without an exemplar condition, congenital hypothyroidism (CHT), with sex and comorbidity as covariates. Three illustrative outcomes (Y:

Type 1 Diabetes: A Guide to Autoimmune Mechanisms for Clinicians.

Type 1 diabetes (T1D) results from the destruction of pancreatic beta cells by autoreactive T lymphocytes, leading to insulin deficiency and lifelong insulin dependence. It develops in genetically predisposed individuals, triggered by environmental or immunological factors. Although the exact causes of T1D remain unknown, the autoimmune pathogenesis of the disease is clearly indicated by the genetic risk conferred by allelic human leukocyte antigens (HLA), the almost obligatory presence of islet cell autoantibodies (AAbs) and immune cell infiltration of pancreatic islets from patients. At the

Updating the PRISMA reporting guideline for scoping reviews: a scoping review.

BACKGROUND AND OBJECTIVES: The Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) extension for scoping reviews (ScRs) (PRISMA-ScR) was published in 2018 to provide reporting guidance. A substantial increase in published ScRs in recent years coupled with the omission or lack of engagement of interest-holders, including patients and public partners, as research partners throughout its development necessitate updating PRISMA-ScR. Important methodological advancements have emerged since PRISMA-ScR 2018, including automation and data extraction, which need to be incorporat
