Central pontine myelinolysis. Considerations on etiology, diagnosis, and treatment.

Pontine myelinolysis can be suspected clinically on the basis of the following criteria: (1) Electrolyte disturbance manifested mainly by hyponatremia; (2) progressive neurologic deficits resulting in a "locked-in" syndrome; (3) usually, but not necessarily, alcohol abuse; and (4) frequent iatrogenic precipitation of the syndrome by inappropriate rehydration of patients at risk. A major pathophysiologic mechanism for this disorder seems to be the anatomic grid structure of the base of the pons, which is more vulnerable to edema than the cerebral hemispheres. Treatment should be focused on rapi…

Central pontine myelinolysis and medullary myelinolysis.

Central pontine myelinolysis (CPM) is a rare, acute, and uniformly fatal demyelinative process that involves the pons almost exclusively. Three cases diagnosed at autopsy illustrate the characteristic clinical course and pathologic features of CPM. A unique extrapontine location of a similar process is noted in the medulla of a 6-year-old girl. The term "medullary myelinolysis" is a descriptive designation for demyelination that occurs predominantly in this area. The clinical and pathologic features of CPM are reviewed in detail, together with a brief review of theories of metabolic, nutrition…

Diffusion-weighted imaging of the fetal brainstem and cerebellum before and after prenatal myelomeningocele repair.

BACKGROUND: Myelomeningocele (MMC) is associated with hindbrain herniation (HBH) and brainstem compression as part of Chiari II. Prenatal MM … n cerebellar tonsil position relative to the foramen magnum. Regions of interest were placed in the pons, cerebellar vermis, and hemispheres to obtain apparent diffusion coefficient (ADC) values (×10-3 mm2/s). ADC values were compared by HBH grade and by HBH reversal status using Mann-Whitney U and Kruskal-Wallis tests. RESULTS: Fifty-nine fetuses wer …

Slight hyperintensity of the left piriform cortex and amygdala on T2-weighted FLAIR images in older adults and patients with probable Alzheimer's disease.

In the OC and pAD groups, SI ratios of the left PC&A to pons (L-PC&A/P) and the right PC&A to pons (R-PC&A/P) were calculated. The regions of interest were defined as large as possible on transaxial images in which the PC&As were most broadly depicted.ResultsThe mean L-PC&A/R-PC&A in the YC, OC, and pAD group …

Chronic intermittent hypoxia elicits distinct transcriptomic responses among neurons and oligodendrocytes within the brainstem of mice.

Chronic intermittent hypoxia (CIH) is a prevalent condition characterized by recurrent episodes of oxygen deprivation, linked to respiratory … ese cell classes, revealed significant differential gene expression, underscoring their distinct responses to CIH. Notably, neurons exhibited transcriptional dysregulation of genes associated with synaptic transmission, and structural remodeling. In addition, we found dysregulated genes encoding ion channels and inflammatory response. Concurrently, oli …

No effect of age, gender and total intracranial volume on brainstem MR planimetric measurements.

OBJECTIVES: MR planimetry of brainstem structures can be helpful for the discrimination of Parkinsonian syndromes. It has been suggested that ageing might influence brainstem MR measurements assessed by MR planimetry, while effects of gender and total intracranial volume (TIV) have not been assessed so far. The aim of this study was to evaluate age, gender and TIV effects on brainstem MR planimetric measures. METHODS: Brainstem MR planimetric measures of diameters (midbrain, pons, middle and superior cerebellar peduncle) and areas (pons and midbrain), the derived ratios, and the magnetic reso…

Extent of pontine pyramidal tract Wallerian degeneration and outcome after supratentorial hemorrhagic stroke.

BACKGROUND AND PURPOSE: Pyramidal tract Wallerian degeneration has been detected on magnetic resonance imaging (MRI) as T2-weighted high-intensity areas. We analyzed the relation between the extent of brain stem Wallerian degeneration and activities of daily living (ADL) after supratentorial hemorrhagic stroke. METHODS: Twenty-six patients with supratentorial hemorrhage were examined on the coronal T2-weighted image of the pons 3 months or later after stroke, and the percentage of Wallerian degeneration in the pons was calculated. The patients were divided into three groups. In group A (n = 6…

Trigeminal neuralgia with rare solitary pontine lesion: A case report and literature review.

Trigeminal neuralgia (TN) associated with brainstem lesions as revealed by Magnetic resonance imaging (MRI), is a rare condition. The MRI often shows a distinctive single pontine in cases of TN (SPL-TN). While the significance of this MRI finding remains unclear, various case reports suggest a potential link to chronic injury in the pontine pathways of the trigeminal nerve. In this report, we present the case of a 42-year-old female who was referred for TN that is refractory to medical treatment with an ipsilateral MRI lesion over the pons who had an excellent response to a trigeminal nerve bo…
