Orthostatic tremor is a rare and often neglected diagnosis with a synchronised bilateral high frequency tremor of 13-18 Hz, affecting primarily the legs and trunk muscles while standing. The patient is troubled by an intense feeling of unsteadiness. Tremor and unsteadiness diminish significantly by sitting down or starting to walk. The exact pathophysiology is yet unknown but is considered to involve a disruption in the cerebello-thalamo-cortical network supporting balance and adjustements of standing position. The feeling of unsteadiness is probably increased by a proprioceptive disturbance c

Nonoperative treatment for lumbar spinal stenosis must address anatomic and biomechanical factors. The entire functional kinetic chain and patient specific goals must be considered. In addition to passive modalities, manual therapy, and patient education, an active program consisting of flexion-based lumbar stabilization exercises, hip mobilization, proprioceptive training, and general conditioning should be initiated. More studies are needed to establish the benefit of a comprehensive, multifaceted treatment approach and to prove its clear benefit over the natural history of lumbar spinal ste

We describe a computer-based method for the special education of deaf children in their stage of expanding an initial vocabulary; the method uses a symbolic keyboard to facilitate real-time communication between the deaf child and the machine. The lack of auditory feedback is partially compensated by using visual and proprioceptive sensorial modalities as alternative representations of words and concepts. To illustrate the use of the computer with display and symbolic keyboard we comment on a program for the teaching of spatial relationships. The same method is used in teaching the structure o

A well-defined degenerative neurological condition has been associated with cholestatic liver disease in children. This syndrome, heralded by gait and limb ataxia, areflexia, and proprioceptive and vibratory sensory loss, has also been observed in abetalipoproteinemia (Bassen-Kornzweig syndrome), cystic fibrosis, and intestinal malabsorption states. A significant body of evidence suggests that vitamin E (alpha-tocopherol) deficiency is in large part responsible for this condition. In this article, a patient manifesting this syndrome is reported, and the current status of the vitamin E deficien

The Sequelae of transcallosal surgery were assessed clinically in 15 children Isolated transection of the corpus callosum does not leave any pronounced when there are no lesions of the cerebral hemisphere. In cases with disturbances of some centers or analyzers, especially in the proprioceptive analyzer in the cortex of the parietal lobe, loss of recent memory, hypokinesia, ideomotor apraxia and astereognosia may develop. The sings are mixed and it is difficult to classify them. We can speak of disjunctive syndromes. The transcallosal approach is suitable in the case of lesions in the anterior

BACKGROUND: The triangular fibrocartilage complex is the main stabilizer of the distal radioulnar joint. While static joint stability is constituted by osseous and ligamentous integrity, the dynamic aspects of joint stability chiefly concern proprioceptive control of the compressive and directional muscular forces acting on the joint. Therefore, an investigation of the pattern and types of sensory nerve endings gives more insight in dynamic distal radioulnar joint stability.
QUESTIONS/PURPOSES: We aimed to (1) analyze the general distribution of sensory nerve endings and blood vessels; (2) ex

Amputation medicine has evolved into a specialized, technology-driven discipline. With increasing global amputation rates, advanced surgical techniques, innovative prosthetic solutions and digital rehabilitation are gaining importance. Transcutaneous osseointegrated prosthetic systems (TOPS) can improve mobility, proprioception and quality of life but carry risks such as stomal infections or periprosthetic fractures. Neuroreconstructive procedures including targeted muscle reinnervation (TMR), targeted sensory reinnervation (TSR), regenerative peripheral nerve interface (RPNI), agonist-antagon…

BACKGROUND: Congenital anomalies of the atlas are rare and usually occur in conjunction with other congenital variants. They include a wide spectrum of anomalies ranging from clefts to hypoplasia or aplasia of its arches that may contribute to spinal cord compressive syndrome.
CASE DESCRIPTION: A 54-year-old male presented with the sudden onset of a severe quadriparesis and loss of proprioception after a minor fall. The magnetic resonance (MR) scan showed cord compression at the C1 level attributed to C1 arch hypoplasia. Two months following a decompressive C1 laminectomy without fusion, and …

Brachymetatarsia can be congenital, idiopathic or secondary and describes an abnormal shortening of a metatarsal bone. The indications for treatment are not only due to cosmesis. The shortening of a ray changes the biomechanics of the foot and can lead to metatarsalgia. A frequent clinical feature is dorsal dislocation of a toe causing painful shoe conflict and disturbed proprioception. There are three main options for operative correction: lengthening osteotomy, interposition of a tricortical bone and gradual lengthening by callus distraction. While one stage lengthening procedures such as os…

A nine-year-old male castrated European Shorthair cat was presented with a six-day history of progressive depression and ataxic gait. Neurological examination revealed depression, absent menace in the left eye, absent pupillary light reflex in the right eye, anisocoria, circling to the right, and delayed proprioception in all limbs. Magnetic resonance imaging showed a space-occupying right temporal lobe lesion adjacent to a small defect in the temporal bone suggestive of a meningo-encephalitis with concurrent abscess formation. The site was surgically approached by a rostrotentorial craniectom…

A New Zealand family with 3 members affected by late onset hereditary cerebellar ataxia is reported, distinguished by the development of near global thermoanalgesia. Because proprioception and autonomic function were preserved, and ataxia only slowly worsened, this proved to be a benign syndrome. The pattern of sensory loss indicated a 'length-dependent' neuropathy. Evaluation of sural nerve biopsy revealed a marked loss of axons, especially of those with diameters 1-7 microns and 0.2-1.5 microns. It therefore seemed likely that impairment of pain and temperature in the syndrome resulted from …

BACKGROUND: Chronic shoulder pain is a common musculoskeletal problem associated with unreleased pain and functional dysfunction that can evolve into central sensitization. Some forms of manual therapy may exacerbate pain and central sensitization. This study investigated the impact of joint position sense therapy (JPST), a moderate joint proprioception training technique, on central sensitization, shoulder functional dysfunction, and pain in patients with chronic shoulder pain compared with more intense exercises or aggressive manual therapies.
METHODS: We assessed the pressure pain threshol…

Horseback riding requires the ability to adapt to changes in balance conditions, to maintain equilibrium on the horse and to prevent falls. Postural adaptation involves specific sensorimotor processes integrating visual information and somesthesic information. The objective of this study was to examine this multisensorial integration on postural control, especially the use of visual and plantar information in static (stable) and dynamic (unstable) postures, among a group of expert horse rider women (n = 10) and a group of non-athlete women (n = 12). Postural control was evaluated through the c

Plantaris muscle (lat. musculus plantaris) is a fusiform muscle of the superficial posterior leg compartment, characterized with a small muscle belly and very long and slender tendon. Many anatomical variations of plantaris muscle have been reported previously, including its inconstancy. Evolutional studies strongly suggest that plantaris muscle is a rudimentary muscle that plays minor role in gait biomechanics. However, plantaris muscle seems to have very important proprioceptive role since it has very high density of muscle spindles. Clinically, plantaris muscle is involved in differential d
