Postpartum Psychiatric Outcomes following Severe Maternal Morbidity in an Urban Safety-Net Hospital.

OBJECTIVE: Severe maternal morbidity (SMM) may be associated with postpartum psychiatric morbidity. However, the direction and strength of this relationship remain unclear. Our goal was to estimate the association between SMM and postpartum inpatient mental health care utilization. STUDY DESIGN: We examined all liveborn deliveries at a large, safety-net hospital in Atlanta, Georgia, from 2013 to 2021. SMM at or within 42 days of delivery was identified using International Classification of Disease codes. The primary outcome of interest was hospitalization with a psychiatric diagnosis in the y

[Treatment of bipolar disorder during pregnancy and in the postpartum period].

BACKGROUND: Pharmacological treatment and prophylaxis of bipolar disorders during pregnancy and in the postpartum period imply complicated clinical assessments. MATERIAL AND METHOD: This article is based on a non-systematic search in PubMed and the authors' clinical experience. RESULTS: If a woman is already using a prophylactic drug at the time of pregnancy, she can in general continue to do so during pregnancy, with the exception of valproate. If the disorder starts during pregnancy; lithium, lamotrigine or second generation antipsychotic drugs are suitable alternatives. In general, drugs

Postpartum onset of panic disorder: a coincidental event?

BACKGROUND: This study investigated whether the onset of panic for the first time during the postpartum period represented a coincidental occurrence. METHOD: Sixty-four childbearing women diagnosed with panic disorder who had been treated at the Yale Anxiety Research Clinic were interviewed. Postpartum panic was defined as the first panic occurring within 12 weeks of the woman's first childbirth. RESULTS: Seven women (10.9%) met criteria for postpartum onset, which is significantly greater than the expected age-corrected percentage of 0.92%, Z = 2.29, p < .02. CONCLUSION: The onset of panic

Psychiatric morbidity in mothers attending maternal and child health clinics in Abeokuta, Nigeria.

The prevalence rate for psychiatric morbidity in post-partum women attending two maternal and child health clinics in Abeokuta was found to be 6.1%. The cases and the non-cases as determined by a two-stage screening of psychopathology and clinical interview showed that anxiety states and depressive neurosis were the main psychiatric disorders amongst the cohort. In the comparison of various sociodemographic and obstetric variable between the two sub-groups, single (unmarried) status was found to be the only differentiating variable. The low prevalence of psychiatric morbidity was explained in

Lithium treatment during pregnancy, delivery, and lactation: an update.

Because prophylactic lithium treatment is often given to manic depressive women of fertile age, the answers to five questions are pressing: (1) Does lithium administration during pregnancy expose the unborn child to risk of malformations? (2) Does such exposure lead to later developmental anomalies? (3) Do changes in the pharmacokinetics of lithium during pregnancy and delivery require special precautions? (4) Does lithium treatment during pregnancy exert other effects? (5) Is it advisable that women in lithium treatment breastfeed their infants? The author discusses these questions in the lig

Psychiatric disorders of the puerperium in South African women of Nguni origin. A pilot study.

The clinical impression gained from symptoms exhibited by Zulu and Xhosa obstetric patients referred to the Department of Psychiatry, King Edward VIII Hospital, Durban, was that certain of the psychiatric disorders presented in a different form to those observed in hospitals serving other population groups. Opinions differ as to the aetiology and relationship of disorders of the puerperium to other psychiatric syndromes with controversial hypotheses referring to both the aetiology and the symptomatology. Firstly, the disorders are considered to be a reaction to specific psychological or biolog

Reversible cerebral vasoconstriction syndrome: a severe neurological complication in early puerperium.

Reversible cerebral vasoconstriction syndrome (RCVS) is an uncommon entity characterized by transient, segmental vasoconstriction of the cerebral arteries, which may be associated with sudden-onset headache and acute neurological complications. The puerperium represents a recognized risk period for its development. We describe the case of a 29-year-old woman who, in the early puerperium, presented with sudden-onset orthostatic headache, followed by severe acute headache and impaired level of consciousness. Brain computed tomography revealed bilateral intraparenchymal hemorrhages. Angiographic

Peripartum cardiomyopathy and the path to recovery: The role of structured cardiac rehabilitation.

Peripartum cardiomyopathy (PPCM) is a rare but serious condition resulting from structural and functional cardiac impairment in the late stages of pregnancy or shortly after delivery. The patient, a 33-year-old healthy woman, presented with sudden onset of shortness of breath, fatigue and chest discomfort five days after an uncomplicated full-term vaginal delivery following an uneventful pregnancy. Diagnostic workup confirmed a PPCM. Following a multidisciplinary approach, the patient underwent a comprehensive cardiac rehabilitation program which included 24 supervised biweekly exercise traini

Blood pressure control, hypertension phenotypes, and albuminuria: outcomes of the comprehensive Basel Postpartum Hypertension Registry.

Postpartum hypertension (PPHT) affects 20% of pregnancies and is strongly correlated to cardiovascular and kidney disease. Most outcome data stems from preeclampsia (PE) neglecting other hypertensive disorders of pregnancy (HDP). This analysis aimed to investigate blood pressure (BP) control, BP phenotypes, therapeutic intensity scores (TIS), and albuminuria across the spectrum of PPHT in the short-medium term. This analysis prospectively followed 370 cases of PPHT. Automated office BP measurements (AOBPM), 24-hour ambulatory BP measurements (24ABPM), TIS and Kidney Disease Improving Global Out

A novel score to predict left ventricular recovery in peripartum cardiomyopathy derived from the ESC EORP Peripartum Cardiomyopathy Registry.

BACKGROUND AND AIMS: There are no established clinical tools to predict left ventricular (LV) recovery in women with peripartum cardiomyopathy (PPCM). Using data from women enrolled in the ESC EORP PPCM Registry, the aim was to derive a prognostic model to predict LV recovery at 6 months and develop the 'ESC EORP PPCM Recovery Score'-a tool for clinicians to estimate the probability of LV recovery. METHODS: From 2012 to 2018, 752 women from 51 countries were enrolled. Eligibility included (i) a peripartum state, (ii) signs or symptoms of heart failure, (iii) LV ejection fraction (LVEF) ≤ 45%,

[De Novo Hypertensive Disorders in the Postpartum Period: Considerations on Diagnosis, Risk Factors, and Potential Intervention Strategies].

Postpartum de novo arterial hypertension (PPDNAH) is defined as blood pressure ≥140/90mmHg, without a history of hypertension during pregnancy or delivery. Its prevalence ranges from 0.3% to 27.5% of all pregnancies. Late-onset postpartum preeclampsia (LOPPP) and late-onset postpartum eclampsia (LOPPE) typically occur between 48hours and 6 weeks postpartum, although recent studies demonstrate the possibility of developing these disorders up to 12 months postpartum. While sharing risk factors with pregnancy-related disorders, they differ in some aspects such as primigravida status. Regarding pr

Chronic Hypertension and the Risk of Readmission for Postpartum Cardiovascular Complications.

OBJECTIVE: Preeclampsia is an important risk factor for cardiovascular disease (CVD, including heart disease and stroke) along the life course. However, whether exposure to chronic hypertension in pregnancy, in the absence of preeclampsia, is implicated in CVD risk during the immediate postpartum period remains poorly understood. Our objective was to estimate the risk of readmission for CVD complications within the calendar year after delivery for people with chronic hypertension. METHODS: The Healthcare Cost and Utilization Project's Nationwide Readmission Database (2010-2018) was used to co

Frequency and Clinical Implications of Referrals to Heart Failure Among Patients with Peripartum Cardiomyopathy.

Peripartum cardiomyopathy (PPCM) is a rare but significant cause of new-onset heart failure (HF) during the peri- and post-partum periods. Advances in GDMT for HF with reduced ventricular function have led to substantial improvements in survival and quality of life, yet few studies examine the longitudinal care received by patients with PPCM. The aim of this research is to address this gap by retrospectively characterizing patients with PPCM across a multihospital health system and investigating the frequency of cardiology and HF specialty referrals. Understanding whether surveillance and medi
