The neuroretina is a functional unit of the central nervous system that converts a light signal into a nerve impulse. Of neuroectodermal origin, derived from the diencephalon, the neuroretina is a layered tissue composed of six types of neuronal cells (two types of photoreceptors: cones and rods, horizontal, bipolar, amacrine and ganglion cells) and three types of glial cells (Müller glial cells, astrocytes and microglial cells). The neuroretina lays on the retinal pigmentary epithelium, that together form the retina. The existence of the internal and external blood-retinal barriers and intra-

A 26-year-old man with sex-linked juvenile central and peripheral retinoschisis showed on fluorescein angiography an avascular lower temporal periphery, bordered at the central side by abnormal capillaries in a dendrite-like structure. In dominant exudative vitreoretinopathy an identical picture without schisis can be seen in the periphery. This feature seems to be a rare phenomenon in sex-linked juvenile retinoschisis. However, routine peripheral ophthalmoscopy and fluorescein angiography (especially inferotemporal) may reveal more cases.

The use of krypton red laser photocoagulation (KRLPC) to treat subretinal neovascular membranes (SRNVM) is increasing. The complications of this treatment in a large series of patients have not been reported. Over a 15-month period, 195 patients had KRLPC to SRNVM of the following etiologies: senile macular degeneration (151), presumed ocular histoplasmosis (33), miscellaneous (11). Fourteen patients had the following complications attributable to KRLPC: choroidal hemorrhage (9), retinal pigment epithelial tears (3), choroidal ischemia (1) and choroidal folds (1). The visually significant comp

Retrobulbar injection has been widely practiced as a technique of ocular anesthesia for many decades. Nevertheless, the technique is not free from complications. Vascular occlusion secondary to retrobulbar injection is rare but can be vision threatening. We report a case series of two such patients who presented with poor vision following retrobulbar injection. Fundus showed pale retina with cherry red spot suggestive of central retinal artery occlusion in case 1 and pale disc with sclerosed vessels and multiple superficial hemorrhages suggestive of a combined occlusion of retinal artery and v

Human congenital anomalies provide information that contributes to the understanding of developmental mechanisms. Here we report bilateral optic nerve aplasia (ONA) with microphthalmia in the autopsy of the cadaver of a 70-year-old Japanese female. The gross anatomical inspection of the brain showed a cotton thread-like cord in the presumed location of the optic nerve tract or chiasm. Histologically, no neural retina, optic nerve bundle or retinal central vessels were formed in the eye globe, and the retinal pigment cells formed rosettes. The cornea, iris, and lens were also histologically abn

Vascular anastomoses between the retina and choroid may occur in different congenital and acquired conditions. It has been postulated that the direction of blood flow in these anastomoses follows a pressure gradient characterized as follows: choroidal artery greater than retinal artery greater than retinal vein greater than choroidal vein. Of the four possible directions of blood flow between the two circulations that this theory allows, only three have previously been reported. The fourth, described in this report, is a choroidoretinal vascular anastomosis with blood flow from a choroidal ves

PURPOSE: To evaluate the feasibility of visualizing intravascular moving particles within retinal and choroidal vessels using real-time high-resolution optical coherence tomography (High-Res OCT), a prototype device offering 3-µm axial resolution.
METHODS: In this cross-sectional study, 20 healthy eyes were imaged using both High-Res OCT and the standard SPECTRALIS HRA+OCT. A dedicated in-built research tool enabled ART-1 B-scan movie acquisition with real-time eye-tracking. Two masked graders qualitatively assessed visibility and direction of intravascular moving particles, and intergrader a

BACKGROUND: The purpose of this study was to assess retinal vascular characteristics of patients with Laron syndrome (LS) as a genetic model of IGF-I deficiency before and after rhIGF1/IGFBP3 treatment and to compare them with healthy controls.
METHODS: A total of 28 subjects (11 LS, and 17 controls) were enrolled. Patients with LS received combined rhIGF1/rhIGFBP3 1-2 mg/kg/d in a single dose and digital fundus imaging was performed. The number of branching points and tortuosity of retinal vessels were studied. Pre- and post-treatment findings were compared with each other and with controls.…

"Traumatic brain injury (TBI) frequently produces visual symptoms along the visual pathway, from the eye and optic nerve to cortical visual processing and oculomotor control. Manifestations include decreased visual acuity and visual fields, traumatic optic neuropathy (TON), oculomotor dysfunctions (saccades, vergence, tracking), photophobia/"visual snow", and higher-level visuo-perceptual problems. Recognition of these symptoms by ophthalmologists and optometrists is essential, as many deficits are treatable (vision therapy, optical aids, medical/surgical management for TON, multidisciplinary

This study was undertaken to identify features of optic nerve head architecture that might explain, in part, why ganglion cell loss occurs in patients with low-tension glaucoma. We compared retrospectively the disc photographs of 25 patients with low-tension glaucoma with those of 26 control patients with typical open-angle glaucoma. No differences were observed in any of the following features: pore length, shape, or size variation; cup/disc and disc/arteriole ratio; clock positions of total rim loss; central retinal vessel entry site; rim pallor; number of rim-crossing vessels; and presence

As the name suggests, in pigmented paravenous chorioretinal atrophy (PPCRA), patches of chorioretinal atrophy and pigment clumping are distributed along the veins (Figs. 22.1 and 22.2). In most cases, the retinal vessels, macula, and optic discs are normal, and the disease is usually nonprogressive.

Early retinal vascular changes in diseases such as diabetic retinopathy often occur at a microscopic level. Accurate evaluation of retinal vascular networks at a micro-level could significantly improve our understanding of angiopathology and potentially aid ophthalmologists in disease assessment and management. Multiple angiogram-related retinal imaging modalities, including fundus, optical coherence tomography angiography, and fluorescence angiography, project continuous, inter-connected retinal microvascular networks into imaging domains. However, extracting the microvascular network, which

PURPOSE: The purpose of this study was to evaluate the relationship between retinal vascular density and retinal thickness (RT) using optical coherence tomography angiography (OCTA) in subjects with diabetic retinopathy (DR).
METHODS: OCTA scans were obtained on non-diabetic (control) and subjects with DR. DR was classified as mild nonproliferative DR (NPDR) or referable DR (moderate NPDR and worse) based on color fundus photographs. Full, superficial, and deep RT were quantified. Corresponding layer-specific capillary densities were quantified as vessel skeleton density (VSD) with and withou

Optic neuritis, a characteristic feature of multiple sclerosis (MS), involves the inflammation of the optic nerve and the degeneration of retinal ganglion cells (RGCs). Although previous studies suggest that retinal blood flow alterations occur during optic neuritis, the precise location, the degree of impairment, and the underlying mechanisms remain unclear. In this study, we utilized two emerging non-invasive imaging techniques, laser speckle flowgraphy (LSFG) and optical coherence tomography angiography (OCTA), to investigate retinal vascular changes in a mouse model of MS, known as experim

PURPOSE: In this experimental study, we quantify retinal microvasculature morphological features with depth, region, and age in immature and mature ovine eyes. These data identify morphological vulnerabilities in young eyes to inform the mechanics of retinal hemorrhage in children.
METHODS: Retinal specimens from the equator and posterior pole of preterm (n = 4) and adult (n = 9) sheep were imaged using confocal microscopy. Vessel segment length, diameter, angular asymmetry, tortuosity, and branch points were quantified using a custom image segmentation code. Significant differences were iden

PURPOSE: Abusive head trauma (AHT) is the leading cause of infant death and long-term morbidity from injury. The ocular consequences of AHT are controversial, and the pathophysiology of retinal research findings is still not clearly understood. It has been postulated that vitreoretinal traction plays a major role in the retinal findings. A computer simulation model was developed to evaluate the vitreoretinal traction and determine whether the distribution of forces in different layers and locations of the retina can explain the patterns of retinal hemorrhage (RH) seen in AHT.
DESIGN: Computer

PURPOSE: Retinal vein pulsation properties are altered by glaucoma, intracranial pressure (ICP) changes, and retinal venous occlusion, but measurements are limited to threshold measures or manual observation from video frames. We developed an objective retinal vessel pulsation measurement technique, assessed its repeatability, and used it to determine the phase relations between retinal arteries and veins.
METHODS: Twenty-three eyes of 20 glaucoma patients had video photograph recordings from their optic nerve and peripapillary retina. A modified photoplethysmographic system using video recor

PURPOSE: Several lines of evidence indicate that altered blood flow regulation may contribute to the pathogenesis of glaucomatous optic neuropathy. Recent data support the hypothesis that the endothelin system is involved in the processes that lead to vascular dysregulation in glaucoma. This study was conducted to test the hypothesis that bosentan, a dual endothelin receptor antagonist, increases ocular blood flow in patients with glaucoma.
METHODS: Fourteen patients with primary open-angle glaucoma and 14 sex- and age-matched healthy volunteers were included. Both groups received bosentan 50
