Electrical treatment of non-united fractures.

The semi-invasive technique for electrical stimulation of bone healing developed by Brighton et al. (1977) was used in 23 patients with nonunited fractures of the tibia (14 cases), humerus (4 cases), scaphoid, femur and fibula as well as one failed arthrodesis of the ankle. The fractures were clinically not healed and not operated on within a minimum of 6 months. The mean period from fracture to treatment was 18 months. Electrical stimulation led to solid bone healing in 10 cases. Two deep infections occurred during the treatment. Of 13 cases that did not unite, a great range of motion in the

Perfusion of the proximal scaphoid pole: correlation between preoperative ge-MRI and intraoperative findings.

BACKGROUND: Gadolinium enhanced MRI (ge-MRI) is considered as gold standard for perfusion evaluation in case of scaphoid nonunion (SNU). However, its clinical value and specificity is still not clearly evaluated. This study compares preoperative ge-MRI-based perfusion assessment and intraoperative proximal pole (PP) perfusion after scaphoid reconstruction by vascularized bone grafts. In addition, the postoperative osseous consolidation (OC) was correlated to intraoperative perfusion findings. METHODS: Between 08/2010 and 01/2020, 60 of 271 patients with scaphoid nonunion received a vasculariz

Acceptability and safety of 3D printed wrist-based orthoses compared to fiberglass casts for the treatment of non-surgical distal radius- and scaphoid fractures: A randomized feasibility trial.

BACKGROUND: Non-invasiveness and comfort are crucial in the conservative management of distal radius and scaphoid fractures. While fiberglass casts are standard, three-dimensional (3D)-printed orthoses offer a promising alternative. PURPOSE: To compare patient experiences, safety perceptions, and satisfaction between a 3D orthosis and fiberglass cast for distal radius or scaphoid fractures. STUDY DESIGN: Randomized feasibility trial. METHODS: Nineteen adult patients were randomly assigned (3D orthosis group, n = 10; fiberglass cast group, n = 9) and followed until orthosis or cast removal a

Scaphoid fractures in children and adolescents: contemporary injury patterns and factors influencing time to union.

BACKGROUND: Historically, scaphoid fractures in children and adolescents have predominantly involved the distal pole, requiring neither surgical care nor extended follow-up. Changing patient characteristics, however, appear to be altering fracture epidemiology and treatment. The purpose of this investigation was to characterize contemporary fracture patterns in children and adolescents and to identify factors influencing time to healing following both nonoperative and operative treatment. METHODS: A retrospective analysis of 351 scaphoid fractures that had been treated from 1995 to 2010 was p

A case of atypical cleft hand - reported with ontogenetic review.

An asymptomatic atypical U shaped cleft hand has been found in a 21-year-old lady attending OPD. On digital skiagram it was found that central digits were absent with remnants of bases of the metacarpals, which have fused with the carpal bones. Moreover, the scaphoid and trapezium had fused to form a single mass. There was no other anomaly in other limbs, so far searched for. An endeavor has been made to explain the anomaly with ontogenetic review.

Orthopantography of the wrist and the knee.

Initial experience with panoramic radiography of the wrist and the knee is presented. The technique offers potential new evaluation of these joints. A standard PA projection demonstrates the carpal bones with almost no superimposition. Additional projections permit more detailed demonstration of the scaphoid and wrist compartments. Similar panoramic studies of the normal knee demonstrated alternative images of the patella, the femoral condyles and tibial plateau. Orthopantography of the joints may provide additional information in the diagnosis of joints abnormalities.

Ultrasound-Based Registration for the Computer-Assisted Navigated Percutaneous Scaphoid Fixation.

An ultrasound (US)-based computer-assisted approach has the potential to improve the accuracy and precision of screw placement for the percutaneous fixation of scaphoid fractures and also reduce the radiation dose for patient and clinical staff. Therefore, a surgical plan based on preoperative diagnostic computed tomography (CT) is registered with intraoperative US images, enabling a navigated percutaneous fracture fixation. However, approaches published so far rely on semimanual methods for intraoperative registration and are limited by long computation times. To address these challenges, we

Mixed Reality Assisted Percutaneous Scaphoid Fixation: A Proposed New Surgical Technique.

The success of percutaneous fixation of non or minimally displaced scaphoid waist fractures is reliant on optimal placement of the screw. This can be challenging for surgeons to achieve, potentially involving a large volume of intraoperative imaging, and surgical time. Mixed reality (MR) is a new technology that allows the projection of holographic imagery within the surgeon's field of vision intraoperatively. This imagery can include surgical planning data and 3D reconstructions of a patient's anatomy that can be used in order to aid the surgeon in achieving accuracy. We describe a technique

The Optimal Location to Measure Scapholunate Diastasis on Screening Radiographs.

BACKGROUND: Lack of a universally accepted location and normal value limits the utility of measuring scapholunate diastasis. The primary purpose of this study was to define the optimal location to measure the scapholunate gap throughout sequential ligament transections. Secondary purposes were to compare plain radiographs with fluoroscopy and to evaluate interrater reliability in measuring scapholunate diastasis. METHODS: Five cadaver forearms were imaged with intact carpal ligaments and after sequentially transecting the scapholunate, radioscaphocapitate, and scaphotrapezium-trapezoid ligame
