Bullous morphea: a blistering sclerotic disease.

Bullous morphea is a rare variant of localised scleroderma characterised by skin thickening and blister development. The following case report describes a woman in her 60s diagnosed with bullous morphea. The patient initially presented with a shiny, pruritic rash that spread from her back to her legs. The rash progressed to include fluid-filled bullae. A skin biopsy revealed thickened collagen bundles and epidermal lifting. Differential diagnoses included generalised morphea and lichen sclerosis, but clinical and histopathological findings supported the diagnosis of bullous morphea. The patien

Autoimmune activation and hypersensitization of the AT1 and ETA receptors contributes to vascular injury in scleroderma renal crisis.

OBJECTIVES: Scleroderma renal crisis (SRC) is a rare vascular complication of systemic sclerosis with substantial risks for end-stage renal disease and premature death. Activating autoantibodies (Abs) targeting the angiotensin II type 1 (AT1R) and the endothelin-1 type A receptor (ETAR) have been identified as predictors for SRC. Here, we sought to determine their pathogenic significance for acute renal vascular injury potentially triggering kidney failure and malignant hypertension. METHODS: IgG from patients with SRC was studied for AT1R and ETAR dependent biologic effects on isolated rat r

MicroRNA in skin diseases.

MicroRNAs are essential regulators of various cellular processes such as cell growth, differentiation, apoptosis, and the immune response, acting as factors for translational repression and/or degradation of target messenger RNA. Currently, microRNAs are considered as promising biomarkers and therapeutic targets for different pathological conditions. Skin may serve as a convenient model for microRNA modulation studies due to the comparatively easy access to targets cells. Cutaneous diseases are characterized by multiple intercellular communication pathways, triggered by diverse stimuli and med

Orthopedic complications of linear morphea: Implications for early interdisciplinary care.

Linear morphea of the limb primarily affects children, and extracutaneous manifestations are common. Orthopedic surgeons are often essential in the care of patients with linear morphea, yet there are few reports outlining specific orthopedic complications in this population. We sought to improve the understanding of orthopedic complications in linear morphea of the limb. Between 1999 and 2014, 51 children were evaluated for linear morphea of an extremity. Twenty-six (51%) had documented orthopedic manifestations. Outcome measures included limb length discrepancy, angular malalignment, limb atr

Serum autoantibodies and their clinical associations in patients with childhood- and adult-onset linear scleroderma. A single-center study.

OBJECTIVE: To determine the frequency of selected serum autoantibodies and their clinical associations in patients with childhood-onset (ChO) or adult-onset (AO) linear scleroderma (LiScl) evaluated at a single institution. METHODS: Seventy-two patients (ChO = 40, AO = 32), including 12 with en coup de sabre, were studied. All ChO patients had disease onset before age 16 years. Clinical features (extent of cutaneous disease, activity, and joint contractures) were recorded. Antinuclear antibodies (ANA) were identified by indirect immunofluorescence (HEp-2 cells), and anti-single-stranded DNA (

[Skin manifestations of Lyme borreliosis].

Lyme borreliosis is the most common tick-borne disease which is caused by Borrelia burgdorferi and transmitted in the Poland, as well as Europe, primarily by Ixodes ticks. After inoculation spirochetes spreads in the skin, activate locally immune response mechanisms and cause characteristic skin lesions like erythema migrans, lymphadenosis benigna cutis, erythema migrans multiplex and acrodermatitis chronica atrophicans. The correct clinical diagnosis of skin lesions and antibiotic treatment is most important for regression of symptoms and prevention of late manifestations of Lyme disease.

Sarcoidosis mimicking lipodermatosclerosis.

The clinical presentation of cutaneous sarcoidosis is highly variable. Rare presentations include ulcerated plaques, morpheaform lesions, and unilateral lower extremity edema. We report the case of a woman who presented with unilateral ulcerating sarcoidosis of the lower leg with progressive fibrosis and edema mimicking lipodermatosclerosis. This case is unique in that the patient exhibited all 3 of the rare manifestations of sarcoidosis; to our knowledge, this presentation has not been previously reported in the literature.

Difficulties in differentiation of Parry-Romberg syndrome, unilateral facial sclerodermia, and Rasmussen syndrome.

INTRODUCTION: Parry-Romberg syndrome (progressive facial hemiatrophy) is a unilateral, slowly progressive atrophy affecting the skin, subcutaneous tissues, muscles, and bones. The relationship between Parry-Romberg syndrome and connective tissue disorders, especially scleroderma en coup de sabre, is still unclear. The neurological symptoms, including epilepsy, migraine, and brain lesion, on neuroimaging may be similar. Rasmussen encephalitis (RE) is connected with chronic inflammation and damage of one hemisphere. Clinically, it is manifested by epileptic partial seizures and unilateral neurol

Subfascial endoscopic perforator surgery is associated with significantly less morbidity and shorter hospital stay than open operation (Linton's procedure).

BACKGROUND: Subfascial endoscopic perforator surgery (SEPS) is the minimally invasive alternative to the open (Linton's) procedure. This new technique may allow perforating vein interruption with fewer complications and a shorter postoperative hospital stay. METHODS: This study was a case note review of 67 procedures: 30 SEPS and 37 Linton's. RESULTS: There were no significant differences between the two groups in age, sex and indication for surgery. SEPS was associated with a significantly reduced postoperative stay in hospital (median 2 (range 1-49) days) compared with the Linton's procedu

Congenital fascial dystrophy: the stiff skin syndrome.

Congenital fascial dystrophy is an uncommon disorder that may mimic scleroderma. The skin is bound down and feels "wood hard" on palpation of the trunk and extremities. Sclerodactyly, telangiectases, and nail fold changes are not present. Unlike scleroderma, the onset is at birth. Flexion contractures result and other associated abnormalities may include developmental delay and mild hirsutism. We report a typical case to illustrate the clinical and pathologic findings of patients with this disorder.

[Varicose liposclerosis amd ulcers: importance of cryo-stripping].

Varicose veins stripping is often a difficult procedure when lipodermatosclerosis or ulcers are present, because malleolar incision is impossible. On the contrary, cryo-stripping is performed by a single incision of the groin or the popliteal fossa. A cryroprobe is inserted into the vein lumen, after ligating and interrupting the sapheno-femoral or popliteal junction. The lower saphenous extremity is frozen. The vein is broken and returned inside-out by removing the cryo-probe upwards. Upon 3,811 legs operated on from 1988 to 1994, there were 265 liposclerosis and 114 ulcers. Deep venous insuf

Scleroderma 'en coup de sabre'.

A case of scleroderma 'en coup de sabre' presenting with uveitis, predominantly posterior, and intractable grand mal seizures is described. The histopathology specimens and neuroradiology investigations reaffirm the clinical impression that inflammation is not solely confined to the skin in this form of linear scleroderma.

High titers of antibodies to single-stranded DNA in linear scleroderma.

Seven patients with severe linear scleroderma were initially found to have antibodies to double-stranded DNA (dsDNA) in higher titers, using the Farr technique. These patients, however, lacked the systemic involvement normally accompanying such antibodies. A detailed investigation of their sera using Crithidia luciliae assay and single-stranded DNA (ssDNA) labeled with iodine 131 disclosed high titers of antibodies to ssDNA and absent dsDNA antibodies. The ssDNA antibody titer was considerably higher than the mean for unselected patients with systemic lupus erythematosus. It is possible that t

The anticentromere antibody: disease specificity and clinical significance.

Serum samples from 539 subjects were screened for the presence of the anticentromere antibody on a human laryngeal carcinoma (HEp-2) cell line (Antibodies, Inc.). The antibody was present in 61 patients (11%), most of whom had features of limited scleroderma or the CREST syndrome (calcinosis cutis, Raynaud's phenomenon, esophageal dysfunction, sclerodactyly, and telangiectasia), either independently or in association with primary biliary cirrhosis. The antibody was rarely found in patients with rapidly advancing or diffuse scleroderma. The anticentromere antibody is therefore a useful prognost
