Glenohumeral contact force, peak muscle forces, and thorax motion increase with fatiguing wheelchair propulsion in persons with a spinal cord injury.

Shoulder problems are highly prevalent among manual wheelchair users with spinal cord injury, affecting their functioning and quality of life. This study investigates the impact of fatigue on wheelchair propulsion technique and shoulder loading in manual wheelchair users (MWU) with SCI. Twelve MWU with a paraplegia performed a standardized fatiguing wheelchair propulsion protocol; a biomechanical assessment of treadmill propulsion was obtained before and after the fatiguing protocol. Rate of perceived exertion (RPE), upper extremity kinematics, and wheelchair propulsion kinetics were assessed.

Upper Extremity Surgeon's Guide to the Evaluation of the Shoulder Girdle and Diagnosis of Associated Pathology.

Shoulder injuries can pose as a diagnostic challenge for clinicians. Owing to the wide variety of pathologies and the fact that they frequently coexist, diagnosis can be difficult. Accurate diagnosis and management not only require a thorough understanding of each condition but the ability to narrow down the broad differential through detailed physical examination. This review article provides an in-depth examination of common shoulder conditions and outlines key physical examination techniques for upper extremity surgeons and other clinicians who manage these conditions. It begins by providin…

[Results of arthroscopic ventral limbus capsule refixation after primary traumatic shoulder dislocation].

The three-year results for arthroscopic refixation of the labrum-ligament-capsule complex after traumatic primary dislocation of the shoulder will be discussed. This prospective study included only patients with primary dislocations showing radiographic signs of anterior instability in the drawer test and no accompanying bony injuries. Twenty-six patients with an average age of 31 years and arthroscopic suture had been dismissed from postoperative treatment after an average 15 weeks showing a fully mobile and pain-free shoulder joint. At the time of follow-up the average results obtained with

Descriptive Profile of Upper Limb Injuries in a Specialized Police Intervention Unit: A Retrospective Study.

BACKGROUND: Police work is associated with high physical demands and an increased risk of occupational injury, particularly among specialized tactical units exposed to intense operational activity. Musculoskeletal injuries are common in law enforcement personnel and frequently result in work incapacity, but data specifically addressing upper limb injuries in intervention units remain limited. The aim of this study was to characterize the clinical profile of upper limb injuries reported as duty-related accidents in a Portuguese police intervention unit and to analyze their association with surg

Overuse Injuries in the Pediatric Population.

Athletic overuse injuries are becoming an increasingly common entity in the active pediatric population. The prevalence of these injuries may be attributed to the combination of an underdeveloped musculoskeletal system, increased participation in competitive sport at a younger age, and increased duration and intensity of training. Many of these injuries may go unreported and/or undiagnosed, as they do not all result in time lost from sports, and are not always appreciated on imaging. Left unrecognized, untreated, or poorly managed, overuse injuries can have long-term health consequences for yo

Rehabilitation of the Overhead Throwing Athlete: There Is More to It Than Just External Rotation/Internal Rotation Strengthening.

The repetitive nature of throwing manifests characteristic adaptive changes to the shoulder, scapulothoracic, and hip/pelvis complexes that result in a set of unique physical traits in the overhead throwing athlete. An effective rehabilitation program is dependent upon an accurate evaluation and differential diagnosis to determine the causative factors for the athlete's pathologic features. The treatment program should be individualized with specific strengthening and flexibility exercises to achieve the dynamic stability that is required for overhead function. In this article we describe the

Bridging self-reinforcing double-row rotator cuff repair: we really are doing better.

Single-row versus double-row repair of rotator cuff tears is currently a controversial topic. In this Level V article, we articulate why we believe that second-generation double-row repair techniques, which use bridging sutures to link the 2 rows of suture anchors together in a self-reinforcing manner, are producing superior clinical and biomechanical results.

Management of multidirectional instability.

Since 1980, several authors have reported successful treatment of multidirectional instability with use of the inferior capsular shift. Neer's initial report in a series of 32 patients noted only one unsatisfactory result. One decade later he reported that ¿more than 100 additional inferior capsular shifts have been done with similar satisfactory results.¿ The authors have reported preliminary results following 75 inferior capsular shifts performed in young athletes. Eighty-nine percent were able to return to their major sport while seventy-three percent maintained the same level of competitiv

A review of the Bristow operation for recurrent anterior shoulder dislocation in athletes.

Twenty-four recurrent dislocating shoulders, in patients ranging from 15 to 26 years, were evaluated after surgical treatment using the Bristow procedure. The operative procedure is discussed in detail recommending the use of an A-O malleolar compression screw, because of ease in insertion, to fix the caroacoid process to the anterior scapular neck. The Bristow procedure as described is an effective operation for achieving shoulder stability while maintaining nearly a complete range of motion in most cases. There was one case of recurrent dislocation after surgery which is described in detail.

Range of Motion Adaptations During a Simulated Game Exposure in Softball Pitchers.

Hip and shoulder range of motion (ROM) alterations are correlated with increased risk of injury in softball athletes. The purpose of this study was to investigate bilateral shoulder and hip ROM adaptation during a simulated softball double-header exposure. It was hypothesized that shoulder and hip ROM would decrease after simulated game exposure and would not return to baseline following a 30-minute break. Thirty (14.8±1.9 yrs, 162.5±18.3 cm, 71.79±16.03 kg) adolescent softball pitchers participated. Testing included: bilateral hip and shoulder ROM (preSG), simulated game exposure (100 pitches

Imaging of the overhead throwing athlete.

Knowledge of overhead throwing biomechanics is crucial to understand specific injuries encountered in throwing athletes on diagnostic imaging. Most specific injuries of overhead throwing athletes occur at the shoulder and elbow. Throwing athletes are susceptible to rotator cuff tears from tensile overload and external and internal impingement. The labrum is also commonly degenerated or torn secondary to overuse syndrome, internal impingement, and microtrauma. The elbow is typically injured secondary to excessive valgus forces during throwing. The ulnar collateral ligament, ulnar nerve, and com

Isolated Radial Nerve Palsy After Glenohumeral Dislocation in a Collegiate Athlete: A Review of the Literature.

Isolated mononeuropathies are uncommon complications after shoulder dislocations. Of these, injuries to the radial nerve are the rarest. Here, we present a case of an isolated radial nerve palsy after a collegiate athlete was hit during a football game and sustained a glenohumeral dislocation. After reduction of the shoulder, he went on to full recovery of motor and sensory function of the radial nerve 1 year after the injury. This case report is further unique given the long-term follow-up in a young, active patient. We review the sparse literature behind the epidemiology and management of th

A wide and versatile combined surgical approach to the shoulder.

Complex injuries and conditions of the shoulder may necessitate wide exposure during the operative procedure. The commonly used approach today is a wide anterior deltopectoral approach with extensive detachment of the deltoid muscle, leading to muscle damage, which may result in difficult postoperative rehabilitation and deteriorated function. We describe a simple combined approach that consists of a deltopectoral approach in conjunction with a subcutaneously performed lateral deltoid split with a single skin incision. We believe this surgical technique provides another useful tool to achieve

Rotator cuff injury: addressing overhead overuse.

Rotator cuff injuries in sports are usually a result of microtrauma from repetitive movements. Classic, or primary, impingement results directly from overhead motions, and secondary impingement is related to underlying shoulder instability. A variety of physical maneuvers are used to assess pain, muscle weakness, and shoulder stability. The workup also includes plain x-rays, supplemented by other imaging tests if a cuff tear is suspected. Nonoperative treatment, which may include steroid injections, is often effective for an inflamed rotator cuff tendon. Surgery is indicated if the patient has
