Multiparametric whole-body MRI of patients with neurofibromatosis type I: spectrum of imaging findings.

Neurofibromatosis (NF) type I is a neuroectodermal and mesodermal dysplasia caused by a mutation of the neurofibromin tumor suppressor gene. Phenotypic features of NF1 vary, and patients develop benign peripheral nerve sheath tumors and malignant neoplasms, such as malignant peripheral nerve sheath tumor, malignant melanoma, and astrocytoma. Multiparametric whole-body MR imaging (WBMRI) plays a critical role in disease surveillance. Multiparametric MRI, typically used in prostate imaging, is a general term for a technique that includes multiple sequences, i.e. anatomic, diffusion, and Dixon-ba

Radiation induced malignancy in retinoblastoma: new pathology in a case report.

OBJECTIVE: Patients with a genetic history of retinoblastoma have an increased risk of developing a second neoplasm. When these secondary malignancies occur in the previously irradiated field of the primary tumor they are most commonly osteosarcomas, fibrosarcomas, and squamous cell carcinomas. We present the first case of a radiation induced adenocarcinoma in a patient after treatment for retinoblastoma. STUDY DESIGN: A case report of one patient. METHODS: This case study underwent a chart review, comprehensive history, physical exam, rigid nasal endoscopy, and radiographic imaging. A liter

[Abducens paralysis from the viewpoint of the ENT physician and skull base surgeon].

BACKGROUND: Among the nerves supplying the extraocular muscles, abducent nerve paralysis is the most common one resulting in double vision. Surveys confirm that the palsy is most often benign and transient. In most cases Ophthalmologist, Neurologist, Pediatrician and Physician are involved in diagnosis and treatment of this symptom. The Otorhinolaryngologist is needed in the least common cases of abducent nerve palsy due to intra- or extracranial diseases, for example trauma, inflammatory complications or tumors of the skull base. PATIENTS AND RESULTS: 12 patients (6 male, 6 female) with abdu

The principles of skull base radiosurgery.

Stereotactic radiosurgery is commonly used for selected patients with benign cranial base tumors. The goal of radiosurgery is cessation of tumor growth and preservation of neurological function. Over the last 2 decades, the technique of radiosurgery has evolved due to improved imaging, better radiosurgical devices and software, and the continued analysis of results. In this report, the authors discuss technical concepts and dose selection in skull base radiosurgery.

New interdisciplinary approach for removing large clivus and upper cervical spine tumors.

We have developed a new interdisciplinary approach for removing large clivus and upper cervical spine tumors. This approach is a combination of the Le Fort I osteotomy, midfacial degloving, and median labiomandibular glossotomy. Our approach gives an excellent, wide surgical field from the nasopharynx, including the base of the skull, to the base of the tongue and permits sufficiently safe extirpation of clivus and upper cervical spine tumors that may not be removed by transoral or transcervical approaches. Our approach not only incorporates the merits of each approach but also creates a large

[Radiation-induced ototoxicity during the treatment of head, neck and skull base cancers].

Radiotherapy, with or without cisplatin, is a cornerstone in the treatment of head and neck cancers and skull base tumors. However, it poses a significant risk of ototoxicity, affecting the cochlea and vestibular apparatus, with major repercussions on hearing, balance, and overall quality of life. Patients may experience hearing loss, tinnitus, or vestibular disorders that can be debilitating. This toxicity stems from complex mechanisms, including direct damage to sensory cells, local ischemia, and inflammatory responses, leading to a progressive decline in auditory and vestibular functions. A Rare Case of Metastatic Clear Cell Renal Cell Carcinoma to the Petroclival Region Presenting as Posterior Cranial Neuropathies.

We present a case of metastatic renal cell carcinoma (RCC) to the petroclival region with clinical symptoms manifested as posterior cranial neuropathies. A 65-year-old male presented with headache symptom for over 1 year, during which time he was diagnosed with RCC and underwent right nephrectomy. However, the headache symptom and cranial neuropathies had been attributed to cranial neuritis all the time until the surgical removal of metastatic tumor in the petroclival region led to the diagnosis of metastatic RCC. RCC has a tendency for metastasis early. The incidence of spread of RCC to brain

Endoscopic Endonasal Approach for Resection of Tuberculum Sellae Meningioma: A Promising Surgical Approach.

BACKGROUND: Over the past decade, endoscopic approaches have been increasingly successful in removing various types of extra-dural lesions from the skull base. Resection of tuberculum sellae meningiomas (TSMs) is challenging. The endoscopic endonasal approach (EEA) for the removal of TSMs is currently an acceptable surgical approach in neurosurgery and, despite the surgical results, the indications and limitations of this approach remain controversial. OBJECTIVE: This paper reviews the authors' experience and outcomes with the EEA for TSM, as well as postoperative complications. METHODS: Ret

Orbitozygomatic craniotomy and trans-sylvian approach for resection of a tuberculum sella meningioma with extension to the posterior fossa.

Tuberculum sella meningiomas can be approached through lateral approaches including pterional/orbitozygomatic craniotomies, frontobasal craniotomy, or through expanded endoscopic transsphenoidal approaches. The authors present the case of a 60-year-old male who presented with bitemporal hemianopia and significant right-side visual acuity compromise due to a large tuberculum sella meningioma. The tumor had an important extension to the posterior fossa. A right orbitozygomatic trans-sylvian approach was deemed most suitable to tackle the posterior extension of the tumor. Some operative nuances a

Cytogenetic analysis of skull base tumors: where do we stand?

PURPOSE OF REVIEW: This study reviewed the value of cytogenetic information for the diagnosis and assessment of prognosis in patients with skull base tumors. RECENT FINDINGS: Chromosomal aberrations can be tumor-specific or nonspecific, and may often show complex karyotypes with numerous chromosomal deletions and gains. These changes may derive from a step-wise genetic transformation commonly seen in these tumors. Most chromosomal abnormalities of skull base carcinomas involve several clones with complex karyotypes. Chromosomal aberrations found in squamous cell carcinoma and salivary gland c

INTRODUCTION: Osteomas are relatively common, benign, slow-growing neoplasms. Mainly occurring in frontal and ethmoid sinuses. Endoscopic surgery plays an important role in management of ethmoid, sphenoid and frontal osteomas. AIM: We discuss our experiences in endoscopic treatment of giant osteomas. RESULTS: In giant osteomas Draf III or Lothrop approaches are prefered. Periostium of the orbit was intact in all cases. Discharge of cerebrospinal liquid was not observed. CONCLUSIONS: Size of tumor determines type of surgical aprroach. Endoscopic surgery is more and more popular way in surgic

Postoperative Gamma Knife surgery for benign meningiomas of the cranial base.

OBJECT: The standard treatment for meningiomas is complete resection, but the proximity of skull base meningiomas to important neurovascular structures makes complete excision of the lesion difficult or impossible. The authors analyzed the mid- and long-term results obtained in patients treated with postresection Gamma Knife surgery (GKS) for residual or recurrent benign meningiomas of the cranial base. METHODS: Thirty-six patients with residual or recurrent benign meningiomas of the skull base following one or more surgical procedures underwent GKS. There were 31 women and five men, ranging

111Indium-octreotide brain scintigraphy: a prognostic factor in skull base meningiomas treated with gamma knife radiosurgery.

AIM: The purpose of this study is to prospectively investigate the prognostic role of somatostatin receptor scintigraphy (SRS) using an 111Indium-labelled somatostatin analogue, Octreotide, in skull base meningiomas (SBMs) treated with gamma knife (GK) radiosurgery. METHODS: From December 1997 to March 2000, SRS was performed both before and within 1 year of radiosurgery on 12 patients. Semi-quantitative data were calculated as SRS index; the index decrease was arbitrarily considered significant above 10%. A potential correlation between the decrease in post/pre-GK SRS index and radiosurgical

A huge osteoblastoma with aneurysmal bone cyst in skull base.

We reported a case of a huge osteoblastoma in the skull base of a young girl who had developed blindness in her right eye 1 year prior to examination. CT showed a large expansile bony lesion with eggshell margin over the skull base. MR imaging showed mixed solid and cystic parts with multiple fluid-fluid levels in the lesion. The optic chiasma and bilateral optic nerves were severely compressed by the tumor.

Gamma knife radiosurgery for skull base meningiomas.

Radiosurgery has been proven to be a safe and effective management strategy for skull base meningiomas either primarily or for tumor recurrence or progression after prior microsurgical resection. With its steep radiation falloff, radiosurgery provides long-term tumor growth control without the complications associated with conventional fractionated radiation therapy. Stereotactic MR imaging has allowed better definition of the tumor margin for precise multiisocenter conformal dose planning, and our current radiation dose prescription has decreased the incidence of new cranial nerve deficits af

A case report of FSH-producing nasal ectopic pituitary adenoma extending to the frontal cranial fossa.

We report the first case of an ectopic pituitary adenoma in the nasal cavity that produced follicle-stimulating hormone (FSH). A 60-year-old man complaining of left nasal bleeding had a polypoid tumor in the left nasal cavity. Findings of computed tomographic scanning and magnetic resonance imaging showed that the tumor originated from the olfactory cleft, occupied the nasal cavity, and extended to the frontal cranial fossa. Results of histologic examination suggested ectopic pituitary adenoma. Magnetic resonance imaging results showed the pituitary gland to be normal. Electron microscopy find

[The 2-stage surgical treatment of a giant chordoma of the clivus growing into the oropharynx and posterior cranial fossa].

Chordomas are relatively rare dysontogenetic tumors developing from the remnants of the spinal chord. Intracranial chordomas consist of 85% of all chordomas. Slowly progressive growth with destructive osteolysis in adjacent bones and low metastatic potential are specific for chordomas. Ninety six surgeries were performed in 71 patients with chordomas at the Burdenko Institute of Neurosurgery from 1987 to 1996. This article presents a clinical case of large clival chordoma with extra-intracranial growth in a 58 year-old female patient. The clinical picture is characterized by lesions in the cau
