Sleep-disordered breathing and obesity.

Recent epidemiological data indicate that obstructive sleep apnoea (OSA) and related conditions are extremely common in the middle-aged population. Obesity is an important aetiological factor in sleep-disordered breathing with a multifactorial role in the pathogenesis of upper airway occlusion. One extreme of the spectrum of sleep-disordered breathing is obesity-hypoventilation syndrome (one type of OSA with awake respiratory failure). Sleep-disordered breathing has a number of clinical consequences, including excess cardiovascular morbidity. Obesity is an important confounder of this associat…

Sleep disorders: simple or complex?

The diagnosis may be as simple as sleep deprivation or a more complex, mixed disorder, such as obstructive sleep apnea complicated by a CNS abnormality. At least 65 primary sleep disorders have been identified. Polysomnographic and other sleep evaluations may be indicated to guide diagnosis and treatment.

Treatment of sleep apnea syndromes.

Effective means exist to treat most patients with obstructive and central apnea, but acceptance by patients remains uneven. Further work is needed to develop alternative approaches, and to better define the role of therapy of any kind in milder disease.

Management of obstructive sleep apnoea.

Obstructive sleep apnoea (OSA) is characterized by abnormal breathing during sleep, and occurs when the upper airway is obstructed but respiratory effort continues. Causes of OSA include obesity, overindulgence in alcohol, and the use of sedatives. The patient complains of unrefreshed sleep, morning headaches and drowsiness, sometimes leading to depression and intellectual impairment. The treatment of choice is weight loss; however, the patient may also require mechanical nocturnal support to prevent the upper airway collapsing and causing OSA. A simple and effective non-surgical treatment for…

Sleep disorders in neuromuscular diseases.

This article reports the results of several studies on neuromuscular patients (9 Duchenne's muscular dystrophy, 10 myotonic muscular dystrophy, 8 mitochondrial myopathies, 6 spinal muscular atrophy). An evaluation of respiratory function during wakefulness, a sleep questionnaire and an overnight polysomnography were performed in all patients. Recurrent hypoxaemia of variable degree was observed during sleep. In most cases, night-time hypoxaemia appears not to be predictable during wakefulness. Nocturnal hypoxaemia occurs in relation to apnoeas or hypopnoeas, mainly of central type, especially …

Sleep disturbance in depression reconsidered.

The well documented observation of a characteristic sleep disturbance in depression has been accorded a good deal of significance. Several current hypotheses regarding the etiology of depression, are based on these findings. However, a review of the descriptive studies shows that care has not generally been taken either to restrict or objectively measure daytime sleep. Evidence is presented suggesting that daytime sleep may occur in depressed patients more frequently than commonly thought and that, in normal subjects, daytime napping can result in nocturnal sleep changes similar to those seen …

Diagnosis of obstructive sleep apnea.

The diagnosis of obstructive sleep apnea is frequently made by taking a meticulous history coupled with a high index of suspicion. Snoring and hypersomnolence are clinical features common to individuals with sleep apnea. Since snoring is said to be a "disease of listeners," it is not uncommon that bed partners reported an increased incidence of depression and marital displeasure. It is for this reason that the spouse or bed partner should be interviewed, since the patient may not be aware of any sleeping problems. Physicians should also be alert to complaints of excessive daytime somnolence, b…

Genetic factors in sleep disorders.

Several sleep disorders have a genetic basis. These conditions include the narcoleptic syndrome, sleep walking, periodic movements in sleep, circadian delay syndromes and familial insomnia. These disorders illustrate different control mechanisms involved in sleep and wakefulness, including those determining the prevalence and timing of NREM and REM activity, somatomotor inhibition and excitation, autonomic discharge, and the circadian framework of sleep. The genetic defect in narcolepsy has been localised to the short arm of chromosome 6, but the chromosomal localisations of the genetic basis …
