The reality of sexsomnia.

PURPOSE OF REVIEW: The purpose of this review is to help further the understanding of the clinical profile of patients with sexsomnia and to better understand the spectrum of the clinical manifestations of sexsomnia. We will review the literature from the past decade on the subject and then compare it with our own clinical experience from patients who were retrospectively identified with sexsomnia at a tertiary sleep clinic over a 6-year period. RECENT FINDINGS: The prevalence of sexual behaviours in sleep remains unknown, but it seems to involve predominantly younger male adults who also fre

Epilepsy and sleep.

PURPOSE OF REVIEW: There is a complex relationship between epilepsy, sleep and sleep disorders. Recent studies have provided new insights into the links between the disorders that may facilitate differential diagnosis and treatment but may also improve our understanding of underlying pathophysiological mechanisms. RECENT FINDINGS: Sleep and sleep deprivation have long been recognized to influence interictal epileptiform discharges and seizures. More recent studies have shown that primary sleep disorders such as obstructive sleep apnoea may worsen epilepsy and treatment of these sleep disorder

Physiology of REM sleep, cataplexy, and sleep paralysis.

The main neural structures generating muscle atonia and other phenomena characteristic of REM sleep are present in dorsolateral portions of the pons in the brainstem. Occurrence of REM sleep and the NREM-REM sleep cycle are probably determined by a balance or interaction between the cholinergic and cholinoceptive REM sleep-on neuronal populations and the monoaminergic REM sleep-off neuronal population. Neural activities producing generalized muscle atonia in REM sleep originate mainly in dorsolateral portions of the pontine reticular formation, descend through the medulla and spinal cord, and

Alzheimer's disease. Sleep and sleep/wake patterns.

Significant changes in sleep/wake patterns, particularly loss of SWS and increased amount and frequency of nighttime wakefulness, apparently occur even at an early stage of the AD process. These disruptions of nighttime sleep increase in magnitude with increasing severity of dementia. While the REM sleep of early stage AD patients is relatively unaffected by the disease process, later stages of AD are marked by significant losses of REM sleep and perhaps more importantly the breakdown of the sleep/wake circadian rhythm with significant amounts of sleep occurring during the day. This daytime sl

Sleep apnea, sleep disorders, and hypothyroidism.

The symptoms of hypothyroidism are protean and include apathy, somnolence, lethargy, personality change, and intellectual deterioration. Many of these symptoms may be related to hypothyroid-induced sleep disorders. Hypothyroidism is associated with abnormal ventilatory drive, abnormal sleep architecture, and sleep apnea. Central, obstructive, and mixed patterns of sleep apnea are commonly observed in hypothyroidism. A case of severe sleep apnea in a grossly myxedematous patient who improved dramatically following thyroid replacement alone is presented. Myxedema is a reversible cause of sleep a

A deep transfer learning approach for wearable sleep stage classification with photoplethysmography.

Unobtrusive home sleep monitoring using wrist-worn wearable photoplethysmography (PPG) could open the way for better sleep disorder screening and health monitoring. However, PPG is rarely included in large sleep studies with gold-standard sleep annotation from polysomnography. Therefore, training data-intensive state-of-the-art deep neural networks is challenging. In this work a deep recurrent neural network is first trained using a large sleep data set with electrocardiogram (ECG) data (292 participants, 584 recordings) to perform 4-class sleep stage classification (wake, rapid-eye-movement,

Congenital central hypoventilation syndrome: a neurocristopathy with disordered respiratory control and autonomic regulation.

Congenital central hypoventilation syndrome (CCHS), a rare neurocristopathy with disordered respiratory control, is characterized by alveolar hypoventilation and diffuse autonomic nervous system (ANS) dysregulation. Mutations in the paired-like homeobox 2B (PHOX2B) are causative, leading to physiologic ANS dysregulation and pathologic abnormalities. Presentation is typically during the newborn period with alveolar hypoventilation during sleep, or in more severely affected individuals, during sleep and wakefulness. Breathing complications occur despite the lungs and airways being normal. Disord

Prevalence of Probable Shift Work Disorder in Non-Standard Work Schedules and Associations with Sleep, Health and Safety Outcomes: A Cross-Sectional Analysis.

PURPOSE: We aimed to estimate the prevalence of probable shift work disorder (pSWD) in a representative sample of Australian workers and identify sleep, health and safety correlates. PATIENTS AND METHODS: In 2019, data were collected from working respondents as part of a cross-sectional national sleep health survey conducted online (n=964 total; n=448 individuals on non-standard work schedules). We established the prevalence of pSWD according to International Classification of Sleep Disorders criteria (ICSD-R, ICSD-2 and ICSD-3). Poisson regression was used to determine crude and adjusted pre

Daily dynamics in sleep and behavior of young African-American children: A convoluted dyad?!

Prior research has provided evidence that in children sleep and behavior are related. We aimed to determine the association between naturalistic daily variations in sleep and behavioral functioning. African American children, 5.4±1.7years old, living on the south side of Chicago participated in a repeated measures study to assess this sleep-behavior link. Data was obtained from three separate two-week periods of 24-hour actigraphy and the parental version of the Behavioral Assessment System for Children. Canonical correlations analyses were applied to investigate the relation between individua

Impact of Developmental Alcohol Exposure on Sleep Physiology.

The present chapter summarizes the clinical and preclinical findings collected to date, showing the impact of developmental alcohol exposure on sleep physiology. Sleep is a complex physiological process that plays a pivotal role in maintaining overall health and well-being via its involvement in regulating physiological, cognitive, and emotional functions. Clinical studies consistently report a high prevalence of sleep disturbances in children and adolescents diagnosed with fetal alcohol spectrum disorders (FASDs), including short sleep duration, sleep anxiety, bedtime resistance, increased sl

The role of sleep in regulating structural plasticity and synaptic strength: Implications for memory and cognitive function.

Dendritic spines are the major sites of synaptic transmission in the central nervous system. Alterations in the strength of synaptic connections directly affect the neuronal communication, which is crucial for brain function as well as the processing and storage of information. Sleep and sleep loss bidirectionally alter structural plasticity, by affecting spine numbers and morphology, which ultimately can affect the functional output of the brain in terms of alertness, cognition, and mood. Experimental data from studies in rodents suggest that sleep deprivation may impact structural plasticity

Automated classification of cyclic alternating pattern sleep phases in healthy and sleep-disordered subjects using convolutional neural network.

Sleep contributes to more than a third of a person's life, making sleep monitoring essential for overall well-being. Cyclic alternating patterns (CAP) are crucial in monitoring sleep quality and associated illnesses such as insomnia, nocturnal frontal lobe epilepsy (NFLE), narcolepsy, etc. However, traditionally medical specialists practice manual division techniques of CAP phases which are sensitive to human weariness and inaccuracies. This might result in a false sleep stage diagnosis. This study proposes an automated approach using a deep learning model based on a 1-dimensional convolutiona

Sleep disturbances in HIV infection and their biological basis.

Antiretroviral therapy has significantly reduced morbidity and mortality in people living with HIV (PLWH). However, a direct consequence of higher survival is the development of ageing-related co-morbidities that have considerable potential to affect quality of life. Sleep disturbances in PLWH are a significant source of morbidity. A meta-analysis has estimated the prevalence of self-reported sleep disturbances in PLWH to be 58%, with commonly identified disturbances including insomnia, obstructive sleep apnoea and poor sleep quality. Not only do sleep disturbances impair daytime functioning,
