The management of biliary tract disease has changed completely as a result of minimally invasive treatment. For most patients with gallstones that cause symptoms a laparoscopic cholecystectomy will treat the condition with minimal morbidity and a short recovery period. If complications are encountered, conversion to a mini-cholecystectomy gives results that are nearly as good. Between January 1986 and July 1988 needle knife papillotomy was attempted in 103 patients after failure of conventional access for endoscopic sphincterotomy (96 cases) or diagnostic cholangiography (seven cases). Access was obtained at the same session in 36 cases and at a subsequent attempt within 2 to 5 days in a further 43, an overall success rate of 77%. We report a case of a 75-year-old woman who developed acute cholecystitis and acute pancreatitis due to hemobilia after a percutaneous liver biopsy. Diagnostic pancreatoscopy was performed in 38 patients with suspected lesions by means of an endoscopic retrograde approach using two different endoscope types. A 3.1-mm endoscope, which required sphincterotomy, and an 0.8-mm ultra-thin pancreatoscope, which could be passed through a standard catheter into the pancreatic duct, were used.
