Severe withdrawal symptoms after discontinuation of alprazolam in eight patients with combat-induced posttraumatic stress disorder.

Eight patients with combat-induced chronic posttraumatic stress disorder (PTSD) receiving long-term alprazolam therapy for anxiety or depression (maximum dose of 2-9 mg/day for 1-5 years) had alprazolam therapy withdrawn. Most of the patients underwent gradual medication withdrawal. All patients had a prior history of alcohol abuse or benzodiazepine dependence. During withdrawal, all patients had severe reactions including anxiety, sleep disturbance, rage reactions, hyperalertness, increased nightmares, and intrusive thoughts; and 6 of the 8 patients had homicidal ideation. As a result of this

Micro-costing analysis of harm reduction services in office-based addiction treatment.

OBJECTIVES: The U.S. opioid crisis, driven by rising polysubstance use and limited access to treatment, continues to strain healthcare systems with nearly 87 % of people with substance use disorders (SUD) lacking addiction treatment and SUD-related hospitalizations increasing. Integration of harm reduction services (HRS) into office-based addiction treatment (OBAT) can reduce high-risk drug use, overdose, infection, and healthcare utilization. However, stigma and reimbursement barriers have hindered adoption. To inform future implementation, we conducted a micro-costing analysis of HRS integra

Gender differences in progression to crack-cocaine use and the role of sexual and physical violence.

OBJECTIVE: This study aimed to evaluate whether progression from first drug use to crack-cocaine use differs according to gender, and whether the report of sexual or physical violence impacts the time of progression. METHODS: We interviewed 896 crack-cocaine users (548 men; 348 women) from addiction treatment units. Cox regression models evaluated the time of progression from first drug use to crack use. We analyzed gender differences according to the absence or presence of sexual or physical violence, also considering whether violence, when present, had occurred before or after the onset of

The importance of considering polysubstance use: lessons from cocaine research.

BACKGROUND: Polysubstance use (PSU) is prevalent among individuals with substance use disorders, but the vast majority of preclinical substance use research has focused on individual substances in isolation. Cocaine has been prevalent in the repertoire of persons who use more than one illicit substance. METHODS: We conducted a meta-analysis combining results from literature searches and secondary data analyses to estimate the prevalence of simultaneous and concurrent cocaine + alcohol and cocaine + cannabis use among cocaine users. We next summarized the small body of literature on behavioral

Prevalence of Nonmedical Use of Prescription Opioids and Association With Co-occurring Substance Use Disorders Among Adolescents in Substance Use Treatment.

PURPOSE: We sought to describe the prevalence of the nonmedical use of prescription opioids (NMUPO) and its association with co-occurring substance use disorders in a sample of adolescents in substance treatment. METHODS: Adolescents in two substance treatment programs were recruited for participation between 2009 and 2013. The Composite International Diagnostic Interview-Substance Abuse Module (CIDI-SAM) was administered to assess substance use patterns and lifetime abuse or dependence. RESULTS: A total of 378 adolescents completed the CIDI-SAM (mean age 16.1 [standard deviation = 1.1], 78%

The Role of Social Capital in African Americans' Attempts to Reduce and Quit Cocaine Use.

BACKGROUND: Research examining substance users' recovery has focused on individual-level outcomes while paying limited attention to the contexts within which individuals are embedded, and the social processes involved in recovery. OBJECTIVES: This paper examines factors underlying African American cocaine users' decisions to reduce or quit cocaine use and uses practice theory to understand how lifestyle changes and shifts in social networks facilitate access to the capital needed to change cocaine use patterns. METHODS: The study, an in-depth analysis of substance-use life history interviews

Osteonecrosis following alcohol, cocaine, and steroid use.

Alcohol, steroids and cocaine have all been shown to be independent risk factors for osteonecrosis when taken in excess. Here we present a case of a young girl who developed debilitating osteonecrosis secondary to low doses of alcohol, steroids and cocaine. We feel it is important to highlight to those caring for such patients of the potential devastating complication of these three agents.

Flupenthixol as a potential pharmacotreatment of alcohol and cocaine abuse/dependence.

Preclinical and clinical studies suggest that the mesolimbic dopamine system plays a major role in mediating the reinforcing effects of drugs of abuse, including alcohol and psychostimulants, and that pharmacological blockade of dopamine D1 and/or D2 receptors may reduce intake of these drugs, as well as relapse rates. The neuroleptic flupenthixol, which has dopamine D1 and D2 receptor antagonist properties and which may be given intramuscularly in order to improve compliance, has been studied as a possible anti-craving drug in substance abuse disorders. Flupenthixol has been shown to attenuat

Drugs and Iran after the Islamic revolution: prophesying the next quarter century.

Prior to the Islamic revolution of 1979, research on Iran's long-standing drug use patterns had begun in clinic settings, but there were few data available on countrywide use. Opium, the traditional drug used in social settings and produced in quantity in the east of the country, was the first target of fierce antidrug measures in 1979. The course of future drug cultivation, use, treatment and, therefore, research needed on the drug problem is closely linked to political developments in Iran. Much will depend on whether Iran maintains an independent stance or rejoins the international agencies

Prevalence of psychiatric disorders in patients with chronic solvent induced encephalopathy (CSE).

INTRODUCTION: Long term occupational exposure to organic solvents may induce chronic solvent-induced encephalopathy (CSE), characterized by mild to severe cognitive impairment, generally seen as the key diagnostic feature. Psychiatric disorders are often diagnosed in subjects with CSE, but were never studied in more detail. This study was designed to establish the prevalence rates of DSM IV mood, anxiety, and alcohol and substance related disorders in patients with CSE. MATERIALS AND METHODS: In CSE, n=203 (consecutively recruited between 2002 and 2005), defined according to the criteria of t

Linking electric field pattern to clinical efficacy of transcranial electrical stimulation in substance use disorder: A modeling-based meta-analysis.

Transcranial electrical stimulation (tES) studies in substance use disorders (SUDs) have shown promise in reducing drug craving and consumption. Effect sizes reported in previous studies provide a quantitative measure of the efficacy of this intervention. However, the specific brain regions where electric field strength is most strongly associated with therapeutic effect sizes remain unclear. To address this gap, we quantified the efficacy of tES in reducing craving and consumption using Hedges' g and employed a combined meta-analysis and electric field modeling (meta-modeling) framework to id

A community engaged learning model to expand student compassion and understanding of the complexities associated with substance use.

Stigma surrounding substance use disorders (SUDs) is widespread and has even been identified among healthcare professionals and trainees, highlighting the need for educational interventions that foster compassionate care, understanding, and emphasize approaches that reflect the complexities of drug use. Community engaged learning (CEL) is an evidence-based pedagogy designed to connect classroom learning with real-world applications. Through community driven collaborative projects that integrate classroom learning with real-world experience, students develop the ability to connect scientific kn

A behavioral economics substance-free activity session (SFAS) approach to reducing substance use: Narrative review and intervention manual.

Standard brief alcohol and drug use interventions focus on motivating reductions in use and associated harms and have shown reliable reductions in these outcomes. However, effect sizes are small, and dissemination is limited by low rates of help seeking. Incorporating intervention elements focused on enhancing wellness, mood, and goal pursuit might enhance efficacy and appeal. The substance-free activity session (SFAS) integrates behavioral economic and motivational interviewing elements to enhance goal-directed and mood-enhancing activity patterns that might provide alternatives to alcohol- a

Experiment-Induced Nociception in Humans with Substance Use Disorders: A Review of Psychophysical Studies.

BACKGROUND: Pain perception in individuals with substance use disorders (SUDs) is shaped by neuroadaptive changes in both peripheral and central nociceptive pathways. Accurate assessment of pain sensitivity in this population is essential for effective clinical management but remains challenging due to overlapping effects of drug use, tolerance, and withdrawal. OBJECTIVE: This review synthesizes findings on pain sensitivity in individuals with SUDs, focusing on experimentally induced nociception-specifically thresholds, tolerance, and perceived intensity of pain. STUDY DESIGN: A systematic n

Psychological and Psychosocial Interventions for People With Schizophrenia and Co-Occurring Substance Use Disorders: A Systematic Review and Meta-Analysis.

IMPORTANCE: Substance use disorder (SUD) is commonly found in individuals with schizophrenia, with a high co-occurrence rate of approximately 41.7%. Despite this high prevalence, people with both schizophrenia and SUD are frequently excluded from clinical trials and systematic reviews; this special group is particularly challenging to treat and imposes a significant economic burden on health care systems. OBJECTIVE: To evaluate the efficacy, acceptability, and tolerability of psychological and psychosocial interventions in patients with schizophrenia and co-occurring SUD. DATA SOURCES: The C

Understanding betel nut addiction: a review of harmful consequences, underlying neurobiology, and emerging intervention strategies.

Betel nut is the fourth most commonly used psychoactive substance globally and is particularly prevalent in the Asia-Pacific region. Betel nut chewing is closely associated with a variety of health hazards, including oral cancer, cardiovascular diseases, and metabolic syndrome. This article reviews the epidemiological characteristics, health hazards, neurobiological mechanisms, and intervention strategies of betel nut addiction. The major active component in betel nut, arecoline, leads to addiction by modulating the cholinergic, dopaminergic, and glutamatergic systems, with the involvement of
