[Differentiation of atypical Parkinson syndrome and delineation from idiopathic Parkinson syndrome with routine magnetic resonance tomography].

Atrophy of frontal lobe, midbrain, pons, and cerebellum was studied in 16 patients with progressive supranuclear palsy (PSP), 14 with multiple system atrophy of striatonigral type (MSA-P), 20 with idiopathic Parkinson's disease (IPS), and 12 age-matched healthy controls using axial T2-weighted MR images (1.5 Teslar). With <16 mm, the PSP group showed significantly lower anteroposterior midbrain diameters than the IPS, MSA-P, and control groups. We conclude that measurement of the anteroposterior diameter of the midbrain with axial T2-weighted MRI is a useful feature and should be incorporated

Expression of apolipoprotein E in ballooned neurons-comparative immunohistochemical study on neurodegenerative disorders and infarction.

Apolipoprotein E (ApoE) in neurons is suggested to play crucial roles in neuronal degeneration and regeneration. We used antibodies against ApoE and phosphorylated neurofilament (pNF) to investigate the immunohistochemical features of ballooned neurons (BNs) in infarction and in various chronic degenerative disorders, including Pick body disease, corticobasal degeneration/progressive supranuclear palsy, Alzheimer's disease, and frontotemporal dementia. BNs in these chronic degenerative processes were intensely labeled with the anti-pNF as reported, whereas BNs in infarction showed less intense

Abeta42-carboxy-terminal-like immunoreactivity is associated with intracellular neurofibrillary tangles and pick bodies.

Co-occurrence of neurofibrillary tangles (NFTs) and beta-amyloid (Abeta)-containing plaques is the pathological hallmark of Alzheimer's disease (AD). Much research has been carried out to elucidate the possible interactions of these two characteristic lesions. Here we show by immunohistochemistry that an Abeta42-carboxy-terminal-like epitope (tAbeta42) is associated with NFTs at an early, intraneuronal stage. The NFTs immunoreactive for tAbeta42 were also positive for tau2. tAbeta42 NFTs occurred even in cases without plaques or any other form of extracellular Abeta deposits. NFTs were tAbeta4

Clinical features, diagnosis, and imaging of parkinsonian syndromes.

The differential diagnosis of parkinsonian syndromes continues to challenge clinicians. The clinical diagnosis of idiopathic Parkinson's disease is correct in only about three quarters of cases when reevaluated neuropathologically, emphasizing the need for more discriminative diagnostic criteria. The clinical spectrum of brain stem Lewy body idiopathic Parkinson's disease itself may be heterogeneous, including dementing and nondementing, familial and sporadic, and levodopa-responsive and -nonresponsive subgroups. Recent clinicopathologic evidence suggests that other parkinsonian syndromes such

Square wave jerks in parkinsonian syndromes.

The frequency of square wave jerks (SWJ) was compared in eight patients with progressive supranuclear palsy (PSP), 25 patients with multiple system atrophy or Parkinson's disease plus (MSA/PP), 85 patients with idiopathic Parkinson's disease (PD) and 20 age-matched normal volunteers. In the control group, the mean (SD) SWJ frequency (SWJ larger than 1 degree amplitude) was 2.3 (2.4)/min. Abnormal ocular fixation (SWJ frequency greater than 10/min) was observed in a large proportion of PSP patients (7/8) and of MSA/PP patients (16/25) but in few PD patients (13/85). Brain destruction alone does not elevate brain aluminum.

Graphite furnace atomic-absorption spectroscopy was used to measure aluminum concentrations in brain samples from 33 patients dying from a variety of neurologic diseases. Four samples from patients dying of nonneurologic causes also were studied. Nine samples (one from each of nine patients) of Creutzfeldt-Jakob disease brain contained normal amounts of aluminum. Aluminum was increased in 9 of 18 brain specimens with seven different pathologic processes. This included three of seven Alzheimer disease, two of three Huntington disease, two of two Parkinson disease, one of one progressive supranu

Protein fingerprints of brain-derived extracellular vesicles predict types of tau pathology.

BACKGROUND: Tauopathies are a heterogeneous group of neurodegenerative disorders characterized by the brain-regional aggregation of three-repeat (3R) or four-repeat (4R) tau isoforms. Current fluid and imaging biomarkers rarely discriminate these isoforms, hampering early, pathology‑specific diagnosis. OBJECTIVE: To determine whether proteomic fingerprints of brain‑derived extracellular vesicles (BD‑EVs) isolated from the prefrontal cortex can (i) distinguish 3R from 4R tauopathies and (ii) mirror the histopathological burden of phosphorylated tau. METHODS: BD‑EVs were purified from post‑morte

Brain proteasomal function in sporadic Parkinson's disease and related disorders.

Because genetic defects relating to the ubiquitin-proteasome system were reported in familial parkinsonism, we evaluated proteasomal function in autopsied brains with sporadic Parkinson's disease. We found that proteasome peptidase activities in a fraction specific to the proteasome were preserved in five brain areas (including the striatum) of Parkinson's disease where neuronal loss is not observed. Striatal protein levels of two proteasome subunits were normal in Parkinson's disease but reduced mildly in disease controls (multiple system atrophy). Our brain data suggest that a systemic, glob

The epidemiology of PSP.

The age-adjusted prevalence of PSP as measured in central New Jersey is 1.5 cases per million population, about 1% of that of Parkinson's disease. It incidence is 3-4 new cases per million population per year, similar to that of such better-known illnesses as myasthenia gravis, the hereditary ataxias as a group and Tourette syndrome. Median actuarially adjusted survival after symptom onset is 5.9-6.9 years. PSP appears to favor no geographical, racial, ethnic or occupational group, though there is anecdotal evidence for hydrocarbon exposure as a candidate etiologic factor. No familial cases of

Subtracting First Principal Component May Improve 4R Tau Detectability on [18F]Flortaucipir Tau PET.

[18F]flortaucipir PET imaging has shown relatively low sensitivity for detecting four-repeat (4R) tau, which limits its utility in 4R tauopathies such as progressive supranuclear palsy (PSP). Previous studies have suggested that the first component of principal-component analysis includes non-disease-related uptake in non-Alzheimer tauopathies. In this study, we tested the hypothesis that subtracting this component could increase sensitivity to 4R tau using a large PSP cohort. Methods: Prospectively recruited patients with PSP (n = 141; 80 Richardson syndrome, 44 PSP-subcortical, and 17 PSP-co

Developing a novel reference region for [18F]PI-2620-PET imaging to facilitate the assessment of 4-repeat tauopathies.

PURPOSE: Progressive supranuclear palsy (PSP) is a fatal 4-repeat (4R) tauopathy with progressive movement phenotypes. In-vivo 4R tau biomarkers are therefore crucial for PSP diagnosis, monitoring, and treatment evaluation. The tau-PET tracer [18F]PI-2620 binds to 4R tau and shows increased uptake in PSP-associated regions (e.g., globus pallidus), and is therefore a candidate 4R tau biomarker. However, commonly used cerebellar tau-PET reference regions show regional proximity to cerebellar 4R tau deposits in PSP, confounding semiquantitative [18F]PI-2620 assessments. Therefore, we employed bia

Association between adiponectin and lipids in Parkinson's disease.

OBJECTIVES: There is increasing evidence linking Parkinson's disease (PD) to lipids, such as the presence of lipids in the core of Lewy bodies in PD brains or high-molecular-weight adiponectin (APN) in phosphorylated α-synuclein-positive Lewy bodies. This study aimed to verify whether APN levels are associated with neurodegenerative diseases. The association between APN and body weight was also investigated. MATERIALS AND METHODS: The following parameters were measured using venous blood sampling: HDL-C, LDL-C, glucose, and lipids, including APN. RESULTS: PD patients receiving dopaminergic t

Subcortical tau is linked to hypoperfusion in connected cortical regions in 4-repeat tauopathies.

Four-repeat (4R) tauopathies are neurodegenerative diseases characterized by cerebral accumulation of 4R tau pathology. The most prominent 4R tauopathies are progressive supranuclear palsy (PSP) and corticobasal degeneration characterized by subcortical tau accumulation and cortical neuronal dysfunction, as shown by PET-assessed hypoperfusion and glucose hypometabolism. Yet, there is a spatial mismatch between subcortical tau deposition patterns and cortical neuronal dysfunction, and it is unclear how these two pathological brain changes are interrelated. Here, we hypothesized that subcortical

Ethnic Differences in Atypical Parkinsonism-is South Asian PSP Different?

BACKGROUND: Progressive supranuclear palsy (PSP) is a progressive atypical parkinsonian condition that results in severe disability. There are few studies of PSP in patients of non-white European ancestry. OBJECTIVES: We aim to perform deep phenotyping in a South Asian PSP cohort to uncover possible ethnic differences in disease characteristics. METHODS: Consecutive PSP patients had their clinical records reviewed for clinical features operationalized in the Movement Disorder Society (MDS)-PSP diagnostic criteria and relevant investigations, including imaging and genetic tests. Clinical vari

Reproducible metabolic topographies associated with multiple system atrophy: Network and regional analyses in Chinese and American patient cohorts.

PURPOSE: Multiple system atrophy (MSA) is an atypical parkinsonian syndrome and often difficult to discriminate clinically from progressive supranuclear palsy (PSP) and Parkinson's disease (PD) in early stages. Although a characteristic metabolic brain network has been reported for MSA, it is unknown whether this network can provide a clinically useful biomarker in different centers. This study was aimed to identify and cross-validate MSA-related brain network and assess its ability for differential diagnosis and clinical correlations in Chinese and American patient cohorts. METHODS: We inclu
