Cardiac sympathetic nerve biology and brain monoamine turnover in panic disorder.

Panic disorder serves as a clinical model for testing whether mental stress can cause heart disease. Our own cardiologic management of panic disorder provides case material of recurrent emergency room attendances with angina and electrocardiogram ischemia, triggered arrhythmias (atrial fibrillation, ventricular fibrillation), and documented coronary artery spasm, in some cases with coronary spasm being complicated by coronary thrombosis. Application of radiotracer catecholamine kinetics and clinical microneurography methodology suggests there is a genetic predisposition to panic disorder that

Instantaneous orthostatic hypotension in children and adolescents: a new entity of orthostatic intolerance.

We are the first to report clinical characteristics and circulatory and catecholamine responses to postural change in 44 children with instantaneous orthostatic hypotension (INOH). The symptoms include chronic fatigue, orthostatic dizziness, weakness, sleep disturbance, syncope or near syncope, headache, and loss of appetite. We divided the patients into two groups: group I (30 patients) had either a recovery time for mean arterial pressure of >25 s or a recovery time of >20 s with a 60% or greater decrease in mean arterial pressure at the initial decrease; group II (14 patients) had a prolong

Sympathetic control of circulation in hypertension and congestive heart failure.

Adrenergic overactivity is a common hallmark of both essential hypertension and congestive heart failure. Indirect and direct measures of sympathetic function have clearly shown that sympathetic activation characterizes essential hypertension. This adrenergic overactivity appears to be related to the severity of the hypertensive state, being detectable in its early stages and showing a progressive increase with the severity of the disease. Essential hypertension is also associated with an impaired baroreflex control of vagal activity, whereas baroreceptor modulation of sympathetic nerve traffi

A case of Guillain-Barré syndrome accompanied by sympathetic overactivity and hypertensive encephalopathy.

Hypertension has often been observed in patients with Guillain-Barré syndrome, and various underlying mechanisms have been implicated. We report the case of a child with the Guillain-Barré syndrome whose hypertension was associated with increases in both renin activity and circulating catecholamines. This case also appeared to be complicated by hypertensive encephalopathy. Following administration of intravenous regitine, a blood pressure change was observed and cardiac performance was assessed by two-dimensional echocardiography. Results indicated that increased total peripheral resistance ma

Cardiac baroreflexes and hypertension.

1. The role of cardiac reflexes in baroreflex control mechanisms and the changes that occur in chronic hypertension is reviewed. The rapid resetting properties of the arterial baroreceptors ensures its role in short-term rather than long-term control of blood pressure. 2. In hypertensive humans and animals, the baroreceptor-heart rate reflex has diminished sensitivity due mainly to reduced maximum capacity of the cardiac vagal component rather than a change to the sympathetic. 3. The development of this vagal deficit in spontaneously hypertensive rats (SHR) coincides with the onset of cardiac

[Phenothiazine derivatives, chlorpromazine and triflupromazine, produce different effects on sympathetic nerve activity in urethane-anesthetized rabbits].

UNLABELLED: This study was designed to evaluate effects of chlorpromazine (CPZ) or triflupromazine (TPZ) on renal sympathetic nerve activity and hemodynamics in urethane-anesthetized rabbits. METHODS: Thirty-five rabbits were divided into the following two groups: CPZ group (N = 10) and TPZ group (N = 25), challenged by an intravenous injection of either CPZ (0.4 mg.kg-1) or TPZ (0.2 mg.kg-1), respectively. Each experimental group was further divided into the following groups. CPZ group was divided into CPZ-INTACT group of animals with neuraxis intact (N = 6) and CPZ-SAD group of animals with

Schwannoma of the cervical sympathetic chain.

Two cases of schwannomas of the cervical sympathetic chain (CSC) are described. Eleven clearly documented cases have been described in the previous literature. These tumours are usually solitary, asymptomatic cervical masses that occur within the upper carotid sheath or parapharyngeal space. The lesions tend to be hypervascular, but not to the same extent as chemodectomas. Angiography may be employed for differential diagnosis. Schwannomas of the CSC are usually readily excised by a cervical approach. Horner's syndrome is a frequent post-operative sequel, despite anatomical preservation of the

Galanin--sympathetic neurotransmitter in endocrine pancreas?

The effects of sympathetic neural activation on basal pancreatic hormone secretion cannot be explained solely by the actions of the classic sympathetic neurotransmitter norepinephrine. The nonadrenergic component may be mediated by the 29-amino acid peptide galanin in that this neuropeptide meets several of the criteria necessary to be considered a sympathetic neurotransmitter in the endocrine pancreas. 1) Galanin administration inhibits basal insulin and somatostatin secretion and stimulates basal glucagon secretion from the pancreas, qualitatively reproducing the effects of sympathetic nerve

Function of salivary glands and sialochemistry in sialadenosis.

Human sialadenosis is a non-inflammatory disease affecting the cephalic salivary glands, mainly the parotid, characterized by a mostly bilateral, painless swelling due to a substantial increase of the glandular parenchyma. After the presentation of clinical, cytological and biochemical data the probable etiology of this disease is discussed. It is concluded that sialadenosis is not a disease of the salivary glands themselves but of their sympathetic innervation.

Protective value of sympathectomy of the ear in noise.

Noise-induced hearing loss was assessed in unilaterally sympathectomized rats exposed one month to 100 dB Leq(lin) noise. The systolic blood pressure of these animals was in the range of 110-140 mmHg. Auditory sensitivity was determined electrophysiologically by brainstem response audiometry to 1/3-octave filtered clicks. The results showed that the loss of sensitivity was about 10 dB smaller (p less than 0.01) on the sympathectomized side. This observation may partially explain the individual differences in susceptibility to noise-induced hearing loss and point towards protective measures and

[Nerve block in the treatment of cranio-facial pain].

The purpose of antalgic nervous block is described and the reasons why a patient suffering from pain syndrome should be examined at the same time by a multidisclipinary team (whos composition will vary depending on the site of the pain) are explained. Such a team will also include the anaesthetist, maxillofacial surgeon, neurologist, neurosurgeon, orthopaedist, radiophysiothherapist, non-medical psychalagist and neurologist. 2436 antalgic nervous blocks undertaken between 17-3-1963 and 31-5-1974 on the head and neck are reviewed. They are analysed in relation to the site of the block, the path
