Calcaneoplasty and reattachment of the Achilles tendon for insertional tendinopathy.

PURPOSE: To report the one-year outcome of 44 patients treated with a novel technique of calcaneoplasty and reattachment of the Achilles tendon. METHODS: 15 men and 29 women (mean age, 53 years) with insertional tendinopathy of the Achilles tendon underwent calcaneoplasty, retrocalcaneal bursa excision, debridement and reattachment of the Achilles tendon with suture anchors via a lateral approach. Outcome was measured using the visual analogue score (VAS) for pain, the SF-36 health survey, the American Orthopaedic Foot and Ankle Society (AOFAS) ankle-hindfoot scores. Patient expectation and s

Morphology of the extensor carpi ulnaris groove and tendon.

PURPOSE: Injury to the extensor carpi ulnaris (ECU) fascial supports on the distal ulna can result in ulnar-sided wrist pain, particularly when the tendon subluxates medially out of the fibroosseous groove with forearm rotation. To better understand the potential risk factors for injury and the indications for modifying the ECU groove, we have evaluated and quantified the morphology of the ECU groove and tendon. METHODS: Axial plane magnetic resonance imaging of the wrist obtained for triangular fibrocartilage complex and intercarpal pathology in 60 patients were reviewed. Mean and standard e

Measuring three-dimensional strain distribution in tendon.

Tendons are tough fibrous tissues that facilitate skeletal movement by transferring muscular force to bone. Studies into the effects of mechanical stress on tendons have shown that these can either accelerate healing or cause tendon injuries depending on the load applied. It is known that local strain magnitude and direction play an important role in tendon remodelling and also failure, and different techniques to study strain distribution have been proposed. Image registration and processing techniques are among the recently employed methods. In this study, a novel three-dimensional image pro…

Achilles tendon injuries in a United States population.

BACKGROUND: Most studies on Achilles tendon ruptures involved US military or European populations, which may not translate to the general US population. The current study reviews 406 consecutive Achilles tendon ruptures occurring in the general US population for patterns in a tertiary care subspecialty referral setting. METHODS: An institutional review board-approved, retrospective review of the charts of 331 (83%) males (6 bilateral, nonsimultaneous) and 69 (17%) females diagnosed with Achilles tendon ruptures over a 10-year period was undertaken. Average age was 46.4 years with 310 (76%) ru

A clinical and radiological study of peroneal tendon pathology.

BACKGROUND: The proportion of patients with clinical findings referable to peroneal pathology and magnetic resonance imaging (MRI)-diagnosed peroneal tendon pathology is unknown. Previous studies have correlated surgical findings with clinical data and preoperative MRI, but there are no published studies that reference clinical examination findings to imaging findings. PURPOSE: To determine the relationship between peroneal tendon pathology as diagnosed by MRI and clinical findings of peroneal tendon pathology. METHODS: Fifty-six patients who had both MRI evidence of peroneal tendon patholog

The junctura lasso: correction of extensor tendon displacement.

Recurrent volar displacement of the extensor digitorum communis tendon (EDC) at the metacarpophalangeal joint (MCPJ) more commonly occurs po … IIrd and IVth MCPJ are exposed using a longitudinal incision. The junctura tendinum between the EDC tendons was next extended proximally by 2 cm on the radial side of the IVth EDC and then divided to create a distally based junctura slip. Next, a 'lasso' was created by passing the slip beneath the IVth EDC tendon and then securing it onto itself with a 5/ …

Current methods and biomechanics of extensor tendon repairs.

Extensor tendon injuries occur frequently. An in-depth understanding of the intricate anatomy of the extensor mechanism is necessary to guide management. Careful counseling is helpful in ensuring patient compliance and optimal outcomes for nonoperative and surgical treatments. For distal lacerations in Zones II-V, we prefer the running-interlocked horizontal mattress technique. Prolonged immobilization or inadvertent shortening of the extensor mechanism can create the unintended consequence of joint stiffness. While clinical outcomes have improved with modern repairs and rehabilitation, patien

Percutaneous techniques for tendon transfers in the foot and ankle.

Tendon transfer procedures are useful for replacing a dysfunctional or diseased tendon or for restoring muscle imbalance. The tendon to be transferred is harvested as distal as is necessary to provide adequate length for rerouting and attachment at the different site. The harvesting of tendon itself can be attained using an open surgical approach or minimally invasive percutaneous techniques that limit surgical exposure. This article describes percutaneous techniques for tendon transfer procedures used to address foot and ankle disorders.

Complications in brief: Quadriceps and patellar tendon tears.

Effective treatment of knee extensor mechanism disruptions requires prompt diagnosis and thoughtful decision-making with surgical and nonsurgical approaches. When surgery is chosen, excellent surgical technique can result in excellent outcomes. Complications and failures arise from missed or delayed diagnoses and from technical problems in the operating room. In particular, inappropriate surgical timing (especially late surgery), misplaced patellar drill holes, and failure to address concomitant injuries can result in complications seen when repairing a patellar or quadriceps tendon tear. We r

Surgical treatment of hip abductor tendon tears.

BACKGROUND: Greater trochanteric pain syndrome is a common orthopaedic condition related to underlying bursitis, but it may reflect gluteal tendinopathy with tendon disruption from the greater trochanter. Our goal was to evaluate our clinical experience with surgical repair of these tears. METHODS: We retrospectively evaluated a consecutive series of twenty-two patients (twenty-three hips) with a tear of the hip abductor tendons who underwent surgical reconstruction and were followed for a minimum of five years. The preoperative evaluation revealed chronic lateral hip pain, a positive Trendel…

Longus colli tendonitis, clinical consequences of a misdiagnosis.

Numerous pathologies diagnosed in the emergency department (ED) are treated with invasive procedures involving anesthetic and surgical risks. Retropharyngeal abscess is a serious condition requiring emergent treatment, often in need of trans-oral incision and drainage under general anesthesia. A misdiagnosis, especially after surgical treatment, might generate undesirable consequences, more so if the final diagnosis is a non-surgical pathology such as longus colli (LC) tendonitis. To discuss the etiology, differential diagnosis and treatment of LC tendonitis, a clinical condition still misdiag

Tendoscopic treatment of recurrent peroneal tendon dislocation.

PURPOSE: To study the possibility of tendoscopic treatment of recurrent peroneal tendon dislocation. METHODS: The case of one patient is described including the tendoscopic technique to deepen the fibular groove. RESULTS: In this single case, there were no complications, recovery time was short, and there was no recurrence of peroneal tendon dislocation. CONCLUSION: Though it seems possible to deepen the fibular groove tendoscopically, further studies are necessary to determine the role of tendoscopy in recurrent peroneal tendon dislocation.

A survey on management of chronic achilles tendon ruptures.

No controlled trials regarding management of chronic Achilles tendon ruptures have been published. We conducted an online survey of orthopedic surgeons affiliated with US medical schools. One hundred twenty-seven surgeons responded, but not all responded to each survey question. Thirty-six percent had foot and ankle fellowship training. Nearly all respondents diagnosed tendon rupture by using palpation of the tendon gap (97%) and the Thompson calf-squeeze test (96%). The Matles test was used by 37% of respondents, with foot and ankle specialists nearly 5 times more likely to use it than nonspe

Fibroma of the tendon sheath - a rare hand tumor.

Fibroma of the tendon sheath (FTS) is a rare, benign, soft tissue lesion. Clinically, FTS presents similarly to the more common giant cell tumor of the tendon sheath. It is distinguished histologically by the lack of giant cells, foamy histiocytes and synovial cells. We presented a case of FTS involving the common tendon sheath surrounding the flexor tendons leading to the third metacarpal. A 63-year-old man presented with a 3-month history of a painless mass in his right palm that had recently tripled in size. Examination demonstrated a 5x4 cm firm, nodular, superficial mass that was adherent…

Tendon coverage using an artificial skin substitute.

BACKGROUND: Soft tissue deficits associated with exposed tendon and absent paratenon pose difficult reconstructive problems due to tendon adhesions, poor range of motion, poor cosmesis, and donor site morbidity. Integra Bilayer Matrix Wound Dressing (Integra Lifesciences Corp Plainsboro, NJ) is a skin substitute widely used in reconstructive surgery, including the incidental coverage of tendons. However, Integra's post-operative functionality of the tendons has not been well documented. We report the results of using Integra for soft tissue reconstruction overlying tendons with loss of paraten…
