[Kidney transplantation: prevention and treatment for bone loss after transplantation].

Despite the ability of renal transplantation to restore many of the excretory metabolic and hormonal abnormalities of chronic renal failure and dialysis implicated in the development of renal osteodystrophy, osteoporosis remains a frequent and serious complication affecting renal transplant recipients. Short-term studies of bone loss following renal transplantation have indicated rapid bone loss within the first 12 months after transplantation. Cross-sectional data suggest that beyond 3 years after transplantation, the bone mineral density does not change or may increase slightly, but remains

Post-transplant multiple myeloma in a pediatric renal transplant patient.

Post-transplant lymphoproliferative disease (PTLD) is a well-recognized complication of the intense immunosuppression required in solid organ and bone marrow transplant recipients. The clinical presentation is varied and can range from a benign infectious mononucleosis-like syndrome to malignant lymphoma. PTLD manifesting as multiple myeloma occurs rarely. We report the unique occurrence of Epstein-Barr virus (EBV)-associated post-transplant multiple myeloma in a 16-year-old male. In contrast to previously described cases of PTLD-myeloma type, this patient was very young, had a clear associati

Post-transplantation growth among pediatric recipients of liver transplantation.

Improving a patient's quality-of-life (QOL) post-liver transplantation is of great importance. An aspect of improved QOL is the restoration of normal growth patterns in pediatric patients. To describe the post-transplantation growth patterns of 72 children included in the National Institute of Diabetes and Digestive and Kidney Diseases - Liver Transplantation Database (NIDDK-LTD), multilevel models were used, according to which children who waited more than a year for transplantation were smaller, compared with age and sex matched peers, at transplantation than children who waited less than a

[Combined pancreas kidney transplantation after isolated kidney transplantation].

Simultaneous pancreas kidney transplantation (SPK) is an established therapy for type 1 diabetics with end stage or preterminal renal diseas … . Four recipients were on dialysis again, whereas two had preterminal renal insufficiency. Pancreas transplants were drained through the bladder (n = 1) or into the small intestine (n = 5) with systemic venous anastomosis. After a median observation period of 28 months (8 to 99 months) all six recipients are insulin free. One patient lost his kidney graft due …

[Membranous aspects of kidney transplantation and reasons of transplant antioxidant defense].

Special membranological studies of the role of antioxidant emoxipine in combined pharmacological support of stable function of the transplanted kidney in 30 patients have shown that this drug restores phospholipid pool of red cell membranes and improves their physical properties. No significant fluctuations of cyclosporin A blood concentrations were registered. Emoxipine addition to combined therapy of patients with transplanted kidney is recommended.

Psychiatric aspects of lung transplant. The Toronto Lung Transplant Group.

Psychiatric problems in a consecutive series of lung transplant applicants (n = 116), candidates (n = 57) and recipients (n = 42) are reported. Fifty percent of all the applicants for lung transplantation reported a history of psychiatric disorder including organic brain syndrome (19%), major depressive episode (16%), panic or anxiety disorder not otherwise specified (11%) and alcohol or substance abuse (11%). In most cases, psychiatric problems had occurred in these patients as a complication of endstage lung disease. The most common new diagnoses in candidates awaiting surgery were adjustmen

Anaesthesiological complications in renal transplantation: a retrospective study of 500 transplantations.

This retrospective study consisted of 500 consecutive renal transplantations performed between September 1977 and September 1981. Preoperatively, congestive heart failure was registered in 262 cases (53.0%) and blood pressure disease in 352 cases (71.3%). The total number of patients with ischaemic heart disease was 22 (4.5%). General anaesthesia was given in 493 and regional anaesthesia in seven cases. In general anaesthesias, tubocurarine was the main relaxant and halothane the main inhalation agent used. Major complications during anaesthesia were blood pressure changes with a higher incide

Liver Transplantation in Japan: Registry by the Japanese Liver Transplantation Society.

As of December 31, 2023, a total of 11,731 liver transplants were performed in 70 institutions in Japan. There were 922 deceased donor transplants (919 from heart-beating donors and 3 from non-heart-beating donors) and 10,809 living-donor transplants. The annual total of liver transplants in 2023 was 470. Deceased-donor transplants increased to 118 in 2023 from 86 in 2022. The most frequent indication was cholestatic disease, followed by hepatocellular disease and neoplastic disease. As for hepatocellular disease in 2023, alcoholic cirrhosis and non-alcoholic steatohepatitis were the most comm…

A Case of Post-Transplantation Lymphoproliferative Disorder Following Kidney Transplantation.

This was a case of a 65-year-old gentleman known to have diabetes mellitus and hypertension since 2012 and post renal transplantation in 2016. He had also been treated for hepatitis C infection in the past. His regular medications included nebivolol 5 mg once a day, tacrolimus 4 mg twice a day, mycophenolate mofetil 500 mg twice a day, prednisolone 5 mg once a day, and insulin-novomix 14 units in the morning and 6 units at night. He presented to a tertiary teaching hospital in Kenya in October 2023 with abdominal pain, vomiting, and constipation on and off for 6 months, worse in the month prio

Pre-Transplant Immunophenotyping in Pediatric Heart Transplant: A Role for the Immunologist?

BACKGROUND: Pediatric heart transplant recipients are at risk for complications from prolonged exposure to immunosuppressive drugs, possibly worsened due to over-immune suppression in patients with pre-existing immune abnormalities. METHODS: This was a retrospective, single-center pediatric cohort study and review of baseline immune evaluation in patients referred for heart transplant. Referrals included were from January 1, 2021, to June 31, 2022. RESULTS: Fifty-one patients were referred during the time period with a median age of 5 years (ranging 1 month-20 years). Twenty-seven total pati

Infectious Enterocolitis in Hematopoietic Cell Transplant with Post-Transplant Cyclophosphamide.

Despite the high incidence of diarrhea in hematopoietic cell transplant (HCT) and the frequent involvement of infections, evidence concerning patients receiving post-transplant cyclophosphamide (PTCy) as graft-versus-host disease (GVHD) prophylaxis in the molecular diagnostic era is limited. This study aimed to evaluate the characteristics, incidence, risk factors, and outcomes impact of infectious enterocolitis in patients with hematologic malignancies undergoing HCT from matched sibling, matched unrelated, and haploidentical donors using PTCy as GVHD prophylaxis. Retrospective analysis of in

Profile of patients referred for lung transplant and their transplant-free survival.

INTRODUCTION: Lung transplant (LTx) is a potential treatment option for all patients with chronic, end-stage respiratory disease, who are refractory to optimal medical therapy or where no medical therapy exists. In India, LTx is still in its evolving stages and published literature is sparse. The current study was carried out to study the selection criteria for lung transplant and to evaluate the clinical and socio-economic profile of patients referred for the same at a tertiary health care facility. METHODS: The study was a descriptive, prospective, observational study. All adults referred f

Tracheal transplantation: lessons learned that may apply to lung transplantation.

PURPOSE OF REVIEW: The purpose of this review is to explore the lessons learned from experimental and human tracheal transplantation to determine if this information may be applied to lung transplantation. RECENT FINDINGS: Experimental work in animal models and the recent human tracheal transplantation suggests that a robust tracheal vascular supply prevents anastomotic complications. Further, this work demonstrates that tracheal allografts undergo a progressive chimerism as recipient epithelium repopulates the allograft. In contrast to most vascularized composite allografts such as hand and

Transplant oncology: multivisceral transplantation for neuroendocrine tumor and liver metastasis.

PURPOSE OF REVIEW: Neuroendocrine tumor (NET) liver metastatic lesions are often multiple and found to be unresectable. Rationale of multivisceral transplantation (MVT: liver-pancreas-intestine transplantation) include radical and complete resection of primary, visible and invisible metastatic tumors by removing all abdominal organs and the lymphatic system. This review aims to describe the concept of MVT for NET and neuroendocrine liver metastasis (NELM), patient selection, timing of MVT, and posttransplant outcomes and management. RECENT FINDINGS: Although indication criteria of MVT for NET

Transplant selection simulation: Liver transplantation for alcohol-associated hepatitis.

Liver transplantation (LT) for alcohol-associated hepatitis (AH) remains controversial due to concerns about candidate selection subjectivity, post-LT alcohol relapse, and the potential exacerbation of LT disparities. Our aim was to design, perform, and examine the results of a simulated selection of candidates for LT for AH. Medical histories, psychosocial profiles and scores, and outcomes of 4 simulation candidates were presented and discussed at 2 multidisciplinary societal conferences with real-time polling of participant responses. Candidate psychosocial profiles represented a wide spectr

Fecal Microbiota Transplant in Pediatric Solid Organ Transplant Recipients.

BACKGROUND: Fecal microbiota transplant (FMT) is an effective treatment for recurrent Clostridioides difficile infection (CDI). Safety concerns around FMT are increased in immunocompromised populations, such as solid organ transplant (SOT) recipients. Outcomes among adult SOT recipients suggest FMT is efficacious and safe; however, pediatric SOT data are lacking. METHODS: We describe the efficacy and safety of FMT among pediatric SOT recipients in a single-center retrospective study from March 2016 to December 2019. Successful FMT was defined as no recurrence of CDI within 2 mo of FMT. We ide
