Assessment of dental maturation on orthopantomograms among children with various dental malocclusions at a tertiary care hospital.

PURPOSE: To describe the diagnostic challenges, clinical course, and multidisciplinary management of an invasive spinal Aspergillus fumigatus infection in a patient presenting with acute neurological deterioration. CASE PRESENTATION: A 42-year-old woman with poorly controlled type 1 diabetes presented with progressive neck and arm pain, followed by acute paraparesis and urinary dysfunction. Imaging revealed an apical lung mass invading the T1 to T3 vertebrae with severe spinal cord compression. She underwent urgent decompressive laminectomy and biopsy, followed by antifungal therapy with vori

Odd presentations of skeletal tuberculosis: A case series.

Tuberculosis has been afflicting mankind since times immemorial and yet can still present itself in such a disguised manner that even the bests of experts may be duped. Any site from head to toe can be affected but certain sites are far less common than the others. We came across three inconspicuous manifestations at atypical sites-parapharyngeal abscess, wrist joint and foot ulcer. No other primary site could be identified in any case. Two cases were diagnosed microbiologically and one with radiological evidence. All the three cases were medically managed and depicted positive response.

[Multilevel tuberculous spondylitis].

A case of a 20-year-old man with multilevel non-contiguous tuberculous spondylitis (cervical, dorsal 6, dorsal 10 and lumbar) is presented. In the context of disseminated tuberculosis in an HIV-negative patient with serious compromise of his general condition and multiple locations of the disease, some of these with fistulas that secreted caseum. The acute paraplegia led, considering the sensory level at dorsal 6, to a first urgent decompression surgery via the posterior approach. A scheduled surgery was then performed, first in the cervical region via the anterior approach, with corpectomy, p

Imaging findings of Pott's disease.

Tuberculosis (TB) continues to be an important public health problem in developed countries especially in deprived socioeconomic groups, older people, immunocompromised patients, drug-therapy resistant cases and the immigrant population. The spine is the most frequent location of musculoskeletal TB. The wide range of clinical presentations results in difficulties and delays in diagnosis. Advanced disease mimics other infections and malignancy. The diagnosis of spinal infections relies on three main factors: clinical symptoms, imaging and bacteriological culture. Advanced imaging such as Magnet

[Infectious spondylodiscitis: the surgical approach].

Introduction of antibiotics in the last century and recent progress in surgical techniques have significantly reduced mortality and morbidity rates of spondylodiscitis. However, when unrecognized, it can have catastrophic consequences for the patient. Antibiotic drugs remain the cornerstone of treatment, and surgery is to be discussed in general as a last resource in case of acute neurologic deficit or severe sagittal or frontal spinal imbalance. Metallic implants are not absolutely contraindicated in surgical treatment of spondylodiscitis and relapse of infection has been reported to be rare.

Retroperitoneal fibrosis and membranous nephropathy.

We report on a patient with a past history of Pott's abscess who suffered both from a retroperitoneal fibrosis and a membranous glomerulonephritis. Five cases of retroperitoneal fibrosis and immune complex glomerulonephritis are already reported in the literature. These associations might result from a particular systemic immune response to an unknown antigen. Consequently, we consider the role of tuberculosis in our case.

[Late sequelae of tuberculous meningitis].

The paper contains a clinico-morphological description of a case with late sequelae of treated tubercular meningitis. The patient at 7 years of age had tuberculous meningitis which was treated by suboccipital and endolumbar administration of streptomycin. Following 12 years, epileptical seizures ensued with an eventual gradual weakness in the extremities leading to profound spastic tetraparesis. During the last years of life contractures and stem symptoms developed. The morphological picture was characterized by posttuberculous fibrous leptomeningitis with an extensive internal hydrocephalus,

Three-dimensional finite-element analysis of a pedicle screw system combined with a titanium mesh support and fixation in the treatment of L4-5 vertebral tuberculosis.

This study aimed to assess the biomechanical stability of pedicle screws combined with titanium mesh bone grafting for spinal reconstruction and fixation during surgery for lumbar spine (L4-L5) tuberculosis using the finite-element method. A model of the L1-sacral segment of the human body was developed using the finite-element method. After verifying its effectiveness, a postoperative model was developed for treating vertebral tuberculosis at the L4-L5 level, which involved implanting an interbody titanium mesh and securing it with pedicle screws. Subsequently, the internal fixation materials

Association between neuromyelitis optica and tuberculosis in a Chinese population.

BACKGROUND: A number of reports have described the presence of tuberculosis (TB) in neuromyelitis optica (NMO) patients. However, a definite association between the two conditions has not been conclusively demonstrated. METHODS: To investigate the association between NMO and TB in a Chinese population, we performed a retrospective review of hospital records of NMO patients, control patients and tuberculosis meningitis (TBM) patients from January 1, 1995 to December 31, 2011. RESULTS: The frequency of preceding/simultaneous active pulmonary TB (PTB) was not significantly different between NMO

Predictive Factors for Late-Onset Neurological Deficits in Patients with Posttuberculous Thoracic Kyphosis.

BACKGROUND: Patients with severe posttuberculous (TB) kyphosis might suffer from late-onset neurological deficits, and surgical correction may improve neurological function. However, there is a lack of predictive factors for neurological function in these patients. OBJECTIVE: This study was aimed at identifying the risk factors for late-onset neurological deficits in spinal TB patients at initial and final assessments. METHODS: Seventy-eight patients with severe kyphosis caused by old thoracic tuberculosis were retrospectively analyzed. Patients with active spinal TB and other spinal disease

Tuberculous spondylitis and paravertebral abscess formation after kyphoplasty: a case report.

STUDY DESIGN: Case report. OBJECTIVE: To report a patient with spinal tuberculosis (TB) and paravertebral abscess formation after kyphoplasty of L1. The literature is reviewed, and diagnostic options are discussed. SUMMARY OF BACKGROUND DATA: Kyphoplasty is a well-established procedure in the treatment of osteoporotic compression fractures and metastatic tumors of the vertebrae. Although complication rates are low, there is evidence for an increased risk of serious local infections after kyphoplasty in patients with any history of systemic infection. Spinal TB accounts for 2% of all TB cases
