The continent bladder: indications and techniques for the continent catheterizable segment.

PURPOSE OF REVIEW: Continent catheterizable segments are a substantial part of the urologist's armamentarium for providing bladder drainage. It is used for a myriad of indications, and there are multiple techniques currently used for its formation. Despite refinements in these techniques significant complications still occur, and there is continued advancement and ongoing investigation. This review examines the current status of the continent catheterizable segment with regard to indications for its use, techniques in its formation, discussion of complications, and ongoing and future direction

Nosocomial urinary tract infections in urologic patients: assessment of a prospective surveillance program including 10,000 patients.

OBJECTIVE: Hospital-acquired urinary tract infections (HUTI) represent a significant impairment in the quality of health care. Incidence in catheterized patients has been estimated at approximately 20%, however few data are available in urologic patients. We report a prospective surveillance program over 6 years in our urologic department and evaluate its evolution. METHODS: Population consists of all patients admitted to the urology ward for 48 hours or more over a 6-year period from 1994. Data recorded: age, gender, duration of stay, insertion and removal of catheters, diagnosis of HUTI. A

Intravesical oxybutynin for spinal cord injury patients.

The treatment of choice for the failure to restore neurogenic bladder dysfunction, managed with clean intermittent catheterization program with incontinence, is anticholinergic medication. The goal is to increase bladder capacity, and decrease intravesical pressure in order to maintain continence between catheterizations. The most commonly used anticholinergic medication in the United States is Oxybutynin. Previous clinical studies have shown that 61% of the patients on oral Oxybutynin report adverse systemic side effects and are noncompliant in taking it. In 48% of the patients, possibly due

Incidence of urinary tract and bloodstream infections in Ghotbeddin Burn Center, Shiraz 2000-2001.

Though burn wound infections have been extensively studied, other nosocomial infections (NIs) in burn patients have received less attention. Invasive diagnostic procedures (vascular and bladder catheterization) make the burn patients more susceptible to different nosocomial infections. The aim of this study was to determine the rate of bloodstream and urinary tract infections associated with i.v. line and urinary catheter (UC) in Ghotbeddin Burn Center and also to compare these rates with those of the National Nosocomial Infections Surveillance System (NNIS) in the USA. This study was conducte

Robotic Mainz II pouch for restoring continence and volitional voiding in failed exstrophy-epispadias complex.

A 17-year-old male and a 30-year-old female with bladder exstrophy-epispadias complex and failed primary repair during infancy presented with continuous urinary leakage and an open bladder plate. Considering their age and desire to avoid a stoma and catheterization, a Mainz II continent urinary diversion was performed with robotic assistance. The ureters were implanted into an S-shaped, low-pressure, high-capacity sigmoid pouch using an extramural serosal tunnel technique to create an anti-refluxing anastomosis. At 3-month follow-up, both patients were completely continent, able to hold urine

Purple Urine Bag Syndrome in an 80-year-old Female Cameroonian.

Purple urine bag syndrome is a rare clinical entity commonly seen in elderly females with a long-term urinary catheterization. It presents as purple discoloration of the patient's plastic urine bag and tubing. We report the first case of this rare entity from Central Africa. We present the case of an 80-year-old female with purple urine associated with signs of urinary tract infection. Her symptoms resolved after 3 days of intravenous antibiotic treatment and diet modification. Purple urine bag syndrome remains rare, especially in African literature. Awareness of this benign syndrome is necess…

Effectiveness of Antegrade Access in Bladder Tumors With Inaccessible Urethra.

Inaccessible urethra with no retrograde endoscopic access due to multiple/diffuse strictures or multiple urethrocutaneous fistulas with acute urinary retention due to posturethral instrumentation (transurethral resection of bladder tumor [TURBT], or TURBT with transurethral resection of the prostate [TURP]), is a rare entity. Management of such a case with a bladder tumor for TURBT/surveillance cystoscopy poses a great challenge. The authors present 12 cases of bladder tumor with inaccessible urethra, 10 cases due to multiple strictures (post-TURBT and/or TURP), and 2 cases due to urethrocutan

Selecting the right urinary leg bag drainage system for patient needs.

There are many clinical reasons why a patient may require indwelling urinary catheterization. Selecting the most appropriate closed drainage system to be used in conjunction with an indwelling catheter is essential to reducing the patient's risk of developing catheter-associated urinary tract infection (CAUTI). Leg bags are one of the options available, however, evidence suggests patients may find these products problematic during daily activity. Careful assessment of patients' needs when selecting a leg bag enables the nurse to identify the individual's specific product needs, thereby reducin…

Postobstructive Diuresis in a Cat Associated With Traumatic Urethral Rupture and Abdominal Compartment Syndrome.

OBJECTIVE: To describe abdominal compartment syndrome (ACS) associated with postobstructive diuresis (POD) in a cat with urethral rupture. CASE SUMMARY: A 6-month-old intact male Korean domestic shorthaired cat presented with suspected urethral rupture. Diagnostic imaging revealed multiple pelvic fractures and free fluid in the abdominal cavity, suggesting a uroabdomen secondary to traumatic urethral rupture. No evidence of urethral obstruction was observed, and the urinary bladder was empty. Uroabdomen and septic peritonitis were confirmed via abdominal fluid-to-serum creatinine and potassiu

Two-Step Process for ED UTI Screening in Febrile Young Children: Reducing Catheterization Rates.

BACKGROUND AND OBJECTIVES: Urinary tract infection (UTI) screening in febrile young children can be painful and time consuming. We implemented a screening protocol for UTI in a high-volume pediatric emergency department (ED) to reduce urethral catheterization, limiting catheterization to children with positive screens from urine bag specimens. METHODS: This quality-improvement initiative was implemented using 3 Plan-Do-Study-Act cycles, beginning with a small test of the proposed change in 1 ED area. To ensure appropriate patients received timely screening, care teams discussed patient risk f

Development of a more clinically relevant bladder and urethral model for catheter testing.

This paper sets out the rationale and design for a more physiologically representative bladder/urethral model than the current rigid funnel industry standard. We suggest this flexible model can better serve as a basis for evaluating new catheters. We investigated the parameters and the validation tests required to construct and test with more flexible materials. The protocol includes static and impact force tests, while measuring internal retention balloon pressure. We have designed a flexible elastomer bladder floor and urethral model and tested it with a Foley balloon catheter. The tests sho
