Altered Resting-State Functional Connectivity in Wernicke's Encephalopathy With Vestibular Impairment.

Objectives: To reveal the neural basis of Wernicke's encephalopathy (WE) with impaired vestibulo-ocular reflex (VOR), we evaluated resting-state functional connectivity (rs-fc) in the vestibular processing brain regions. Methods: Rs-fc between the vestibular regions and the rest of the brain were compared with neurotological features including the head-impulse tests (vHIT) and caloric responses in patients with WE (n = 5, mean age 53.4 ± 10 years) and healthy controls (n = 20, mean age 55.0 ± 9.2 years). Rs-fc analyses employed a region of interest (ROI)-based approach using regions selected a

Effectiveness of three vestibular rehabilitation exercises for treating acute unilateral peripheral vestibular dysfunction: a multicenter randomized study.

BACKGROUND: Vestibular rehabilitation therapy (VRT) is an established intervention for patients with acute unilateral peripheral vestibular dysfunction (aUPVD). However, the relative effectiveness of different VRT strategies remains uncertain. This multicenter randomized pilot trial aimed to compare the effects of customized vestibular rehabilitation therapy (CVRT), generic Cawthorne-Cooksey exercises (GVRT), and vestibulo-ocular reflex (VOR) adaptation exercises on vestibular symptom recovery and functional improvement. METHODS: A total of 34 patients with aUPVD were enrolled across three ce

Cochlear Implantation in Ménière's Disease.

IMPORTANCE: Though the effects of cochlear implantation (CI) on patients with hearing loss has been well described, its effect on the auditory and vestibular symptoms in patients with Ménière's disease (MD) has not been firmly established. OBJECTIVES: To determine whether CI in patients with MD improves hearing performance as measured by standard post-CI audiometric testing and to describe the impact of CI on subjective auditory and vestibular quality-of-life measures. DESIGN, SETTING, AND PARTICIPANTS: Retrospective review of all patients with MD 18 years or older who met diagnostic criteri

Modeling postural instability with Galvanic vestibular stimulation.

In this study the effect of a pseudorandom binaural bipolar Galvanic stimulus generated by a sum of nonharmonically related sine waves on postural control was functionally assessed using computerized dynamic posturography (CDP), and the results compared to vestibulopathic patient populations and astronauts exposed to microgravity. The standardized CDP test battery comprised six sensory organization tests (SOTs) that combined three visual conditions (eyes open, eyes closed, and sway-referenced vision) with two proprioceptive conditions (fixed and sway-referenced support surfaces). Subjects (12)

Exploring Vestibulo-Masseteric Pathways: An Insight into Motion Sickness Susceptibility.

BACKGROUND: Motion sickness is an autonomic reaction to sensory conflict among vestibular, somatosensory, and visual inputs, manifesting as drowsiness, dizziness, discomfort, nausea, pallor, sweating, headache, malaise, vomiting, and apathy. Although vestibular function in motion sickness has been probed using various vestibular tests, the utility of masseter vestibular evoked myogenic potential (mVEMP)-a novel assay of the vestibulo-masseteric reflex elicited via masseter muscle stimulation-remains largely unexplored. PURPOSE: This study sought to explore mVEMP responses among individuals wi

Age-related decline in vestibulo-ocular reflex gain is associated with an increased sway response to electrical vestibular stimulation.

Despite considerable loss of peripheral vestibular input with age, there is relatively little change in function. This has led to speculation that increasing central neural gain mitigates such losses. Here, we test this hypothesis by comparing the gain of the vestibulo-ocular reflex (VOR) with the response to electrical vestibular stimulation (EVS) across the lifespan. The VOR assesses the full vestibulo-motor pathway, whereas EVS bypasses the peripheral vestibular mechanics. Hence, any dissociation between the two may reveal a difference in central versus peripheral function. A rotating chair

Early Assessment of Vestibular Function after Unilateral Cochlear Implant Surgery.

INTRODUCTION: Cochlear implantation (CI) has been reported to negatively affect vestibular function. The study of vestibular function has variably been conducted using different types of diagnostic tools. The combined use of modern, rapidly performing diagnostic tools could prove useful for standardization of the evaluation protocol. METHODS: In a group of 28 subjects undergoing CI, the video head impulse test (vHIT), the cervical vestibular evoked myogenic potentials (cVEMP) and the short form of the Dizziness Handicap Inventory (DHI) questionnaire were investigated preoperatively and postop

A novel black-hooded mutant rat (ci3) with spontaneous circling behavior but normal auditory and vestibular functions.

Abnormal circling behavior in rodents is usually attributed to vestibular dysfunction. In rats, all circling mutants described previously have inner ear defects resulting in auditory and vestibular dysfunctions. Here, we describe a new mutant rat with abnormal spontaneous circling behavior but normal auditory and vestibular functions. The new circling mutant rat was discovered in progeny of an apparently normal black-hooded (BH) rat inbred line [BH.7A(LEW)/Won] and was termed ci3, because we recently found two other mutant circling rats (ci1 and ci2) in a Lewis (LEW) inbred rat strain. The ci3

Diagnostic evolution of vestibular neuritis after long-term monitoring.

INTRODUCTION: The diagnosis of vestibular neuritis is based on clinical and laboratory findings after exclusion of other disease. There are occasional discrepancies between clinical impressions and laboratory results. It could be the first vertigo episode caused by other recurrent vestibular disease, other than vestibular neuritis. OBJECTIVE: This study aimed to analyze the clinical features and identify the diagnostic evolution of patients with clinically suspected vestibular neuritis. METHODS: A total of 201 patients clinically diagnosed with vestibular neuritis were included in this study

[The relationship between cognitive impairment, anxiety-depression symptoms and balance and spatial orientation complaints in the elderly].

Balance and spatial orientation complaints, generically defined as "dizziness", are frequent in the elderly. They can cause a greater or lesser degree of handicap, and be associated with a greater or lesser degree of cognitive impairment and anxiety-depression symptoms. We examined 163 patients, referred for these complaints to the otoneurological outpatient services of 6 university hospital centres of the northern and central Italy, performing a bedside vestibular examination. The test allowed to distinguish between subjects with specific vestibular disorders (mainly BVP, Menière's disease an

Ultrastructural evidence of repair and neuronal survival after labyrinthectomy in the squirrel monkey.

Although the vestibular and cochlear branches of the VIIIth cranial nerve originate embryologically from the same primordia, results of the present investigation confirm previous findings indicating that the vestibular branch may be more plastic with respect to recovery after surgical insult than the cochlear division. In this report we show ultrastructural details of changes undergone by the vestibular nerve after surgery. Dendrites peripheral to the vestibular nerve ganglion (VNG) were severed by surgically removing the vestibular end organs; the squirrel monkeys were then allowed to recuper

Functional Consequences of Poor Binaural Hearing in Development: Evidence From Children With Unilateral Hearing Loss and Children Receiving Bilateral Cochlear Implants.

Poor binaural hearing in children was hypothesized to contribute to related cognitive and academic deficits. Children with unilateral hearing have normal hearing in one ear but no access to binaural cues. Their cognitive and academic deficits could be unique from children receiving bilateral cochlear implants (CIs) at young ages who have poor access to spectral cues and impaired binaural sensitivity. Both groups are at risk for vestibular/balance deficits which could further contribute to memory and learning challenges. Eighty-eight children (43 male:45 female, aged 9.89  ±  3.40 years), group

Video Head Impulse Test to Preoperatively Identify the Nerve of Origin of Vestibular Schwannomas.

BACKGROUND: Identification of the nerve of origin in vestibular schwannoma (VS) is an important prognostic factor for hearing preservation surgery. Thus far, vestibular functional tests and magnetic resonance imaging have not yielded reliable results to preoperatively evaluate this information. The development of the video head impulse test (vHIT) has allowed a precise evaluation of each semicircular canal, and its localizing value has been tested for some peripheral vestibular diseases, but not for VS. OBJECTIVE: To correlate patterns of semicircular canal alteration on vHIT to intraoperativ

Automated head motion system improves reliability and lessens operator dependence for head impulse testing of vestibular reflexes.

Deficiency of the eye-stabilizing vestibulo-ocular reflex (VOR) is a defining feature in multiple diseases of the vestibular labyrinth, which comprises the inner ear's sensors of head rotation, translation and orientation. Diagnosis of these disorders is facilitated by observation and measurement of eye movements during and after head motion. The video head impulse test has recently garnered interest as a clinical diagnostic assessment of vestibular dysfunction. In typical practice, it involves use of video-oculography goggles to measure eye movements while a clinician examiner grasps the subj

Efficacy of the silicon based agent for age related decline in vestibular function.

Age-related vertigo and balance disorders can lead to falls, fractures, and prolonged confinement to bed. Loss of mobility and/or social interaction may cause cognitive decline and lower quality of life, resulting in significant social and economic burdens. Aging societies urgently need treatments for vestibular decline, as no cures exist, and current therapies only provide symptomatic relief. Oxidative stress contributes to age-related cochlear balance system damage, making antioxidants a potential treatment. Silicon-based agent (Si-agent) is an excellent antioxidant. When reacting with water

Pharmaceutical countermeasures have opposite effects on the utricles and semicircular canals in man.

INTRODUCTION: Sensory conflicts in the vestibular system lead to motion sickness of which space motion sickness (SMS) is a special case. SMS affects up to 70% of the astronauts during the first 3 days in space. The search for effective countermeasures has led to several nonpharmacological and pharmacological approaches. The current study focuses on the effects of lorazepam (1 mg), meclizine (25 mg), promethazine (25 mg), and scopolamine (0.4 mg) on the vestibular system, with special focus on the canal and otolith functions separately. METHODS: The study had a placebo-controlled, single blind
