Single-Cell dissection of fibrodysplasia ossificans progressiva identifies SPP1 as a mediator of macrophage-fibroadipogenic progenitors crosstalk.

Fibrodysplasia Ossificans Progressiva (FOP) is a rare genetic disorder caused by gain-of-function mutations in ACVR1/ALK2, leading to progressive heterotopic ossification (HO) through endochondral bone formation. Inflammatory flare-ups often precede new ossification events, but the cellular and molecular mechanisms linking immune responses to progenitor cell fate remain incompletely understood. Here, using a tamoxifen-inducible Acvr1R206H mouse model of FOP and a reproducible muscle injury protocol, we combined single-cell RNA sequencing with in vitro assays to dissect early events during lesi

[Treatment goals for low-risk myelodysplastic syndromes].

Myelodysplastic syndrome (MDS) is an abnormality of hematopoietic stem cells with limited effective drug therapy. Owing to the diversity of disease subtypes, treatment options must be decided on a case-by-case basis based on predictive prognostic scoring systems. The recently published revised WHO and ICC classifications incorporate genetic abnormalities into the diagnosis, and IPSS-M, which incorporates genetic mutations into the IPSS-R, allows for more individualized prognostic prediction. The treatment of low-risk MDS focuses on managing infection and bone marrow failure while preserving th

ACVR2B polymorphism, Adiponectin, and GDF-15 levels as biomarkers for cachexia in gastrointestinal cancer.

Reversing cancer cachexia remains a challenge. Genetic biomarkers for the early detection of the disease have been explored in order to enable the implementation of preventive measures. We therefore genotyped candidate genes based on cachexia phenotype and quantified adiponectin and GDF-15 levels in cachectic patients with gastrointestinal cancer. Patients with a diagnosis of gastrointestinal cancer were divided into a cachectic and a non-cachectic group after the start of chemotherapy. A control group (no cancer) was also included. We genotyped the following single nucleotide polymorphisms (S

Gene panel diagnostics reveals new pathogenic variants in pulmonary arterial hypertension.

BACKGROUND: A genetic predisposition can lead to the rare disease pulmonary arterial hypertension (PAH). Most mutations have been identified in the gene BMPR2 in heritable PAH. However, as of today 15 further PAH genes have been described. The exact prevalence across these genes particularly in other PAH forms remains uncertain. We present the distribution of mutations across PAH genes identified at the largest German referral centre for genetic diagnostics in PAH over a course of > 3 years. METHODS: Our PAH-specific gene diagnostics panel was used to sequence 325 consecutive PAH patients fro

Luspatercept for the treatment of anaemia in non-transfusion-dependent β-thalassaemia (BEYOND): a phase 2, randomised, double-blind, multicentre, placebo-controlled trial.

BACKGROUND: In patients with non-transfusion-dependent β-thalassaemia, haemoglobin concentrations lower than 10 g/dL are associated with a higher risk of morbidity, mortality, and impaired quality of life. No drugs are specifically approved for anaemia management in patients with non-transfusion-dependent β-thalassaemia, other than transfusion therapy administered infrequently in accordance with patients' needs. We assessed the efficacy and safety of luspatercept versus placebo in patients with non-transfusion-dependent β-thalassaemia. METHODS: We did a phase 2, randomised, double-blind, mult

Heterogeneity of the bone marrow niche in patients with myeloproliferative neoplasms: ActivinA secretion by mesenchymal stromal cells correlates with the degree of marrow fibrosis.

Mesenchymal stromal cells (MSCs) represent an essential component of the bone marrow (BM) niche and display disease-specific alterations in several myeloid malignancies. The aim of this work was to study possible MSC abnormalities in Philadelphia-negative myeloproliferative neoplasms (MPNs) in relationship to the degree of BM fibrosis. MSCs were isolated from BM of 6 healthy donors (HD) and of 23 MPN patients, classified in 3 groups according to the diagnosis and the grade of BM fibrosis: polycythemia vera and essential thrombocythemia (PV/ET), low fibrosis myelofibrosis (LF-MF), and high fibr

Amhy/Amhr2y-mediated sex determination in two distantly related teleosts relies on the conserved Alk3-Smad5 axis.

In teleosts, homologs of Anti-Müllerian Hormone (Amhy) and its type II receptor (Amhr2/Amhr2y) have been independently recruited as master sex-determination genes in about 50% of known cases. However, it remains unknown whether a conserved transducer pair exists, as the requisite type I receptors and R-Smad effectors remain unidentified amidst their diversity and potential redundancy. In this study, we employed an in vitro reporter assay to screen five type I receptors (Alk2a, Alk2b, Alk3, Alk6a, Alk6b) and three R-Smads (Smad1, Smad5, Smad8), discovering that only Alk3, Alk6a, or Alk6b, in co

Fibrodysplasia ossificans progressiva in children: diagnostic pitfalls and ACVR1 genotype-phenotype spectrum.

UNLABELLED: Fibrodysplasia ossificans progressiva (FOP) is a rare multisystem disorder with the triad of congenital hallux valgus, flare-ups and progressive heterotopic ossification. The flare-ups (episodes of painful soft-tissue swelling) start in early childhood, and progress to ossification with cumulative functional disability. We describe the cohort of FOP from a single tertiary center in Türkiye and highlight the phenotypic variabilities and diagnostic pitfalls. The subjects included 10 affected individuals (7 females and 3 males, aged 10 days to 20 years), whose diagnosis was confirmed

Family phenotypic profile in hereditary hemorrhagic telangiectasia: genotype-phenotype correlation in a pediatric Italian population.

BACKGROUND: Hereditary Hemorrhagic Telangiectasia (HHT) is a rare, hereditary, autosomal dominant vascular disease, characterized by visceral arteriovenous malformations (AVMs), mucocutaneous telangiectasias and epistaxis. While symptomatic medical and surgical therapies are used in the management of pediatric patients, data comparing phenotypic presentation between genetically related individuals (e.g., parent-child pairs) remain limited. AIMS AND METHODS: We conducted a single-center retrospective study involving pediatric patients with genetically confirmed HHT and their affected parents a

Dermatologic findings in hereditary hemorrhagic telangiectasia: association with epistaxis, visceral involvement, and genetics.

BACKGROUND: Hereditary hemorrhagic telangiectasia (HHT) is a genetic vascular disorder characterized by epistaxis, mucocutaneous telangiectasias, and visceral arteriovenous malformations (AVMs). Despite the central role of dermatologic manifestations in diagnosis, there is a lack of comprehensive studies examining these features in large HHT populations. The aim of this paper is to describe the clinical and dermatologic features of a cohort of HHT patients, focusing on the prevalence, distribution, and morphology of mucocutaneous telangiectasias using videodermatoscopy, and to analyze their co

Efficacy and safety of luspatercept in non-transfusion-dependent β-thalassemia: long-term results from the BEYOND study.

Chronic anemia due to non-transfusion-dependent β-thalassemia (NTDT) can result in clinical morbidities, particularly with inadequate management. Luspatercept was previously shown to improve hemoglobin levels in patients with NTDT in the phase 2, randomized, double-blind, placebo-controlled BEYOND trial (ClinicalTrials.gov identifier: NCT03342404). Here, we report long-term efficacy and safety results from the final analysis of BEYOND spanning an additional 26 months (∼2.2 years) of follow-up. Median treatment duration was 202.8 weeks for luspatercept and 61.1 weeks for placebo. Overall, 94.8%

Germline genetic mutations in pediatric cerebrovascular anomalies: a multidisciplinary approach to screening, testing, and management.

OBJECTIVE: Genetic alterations are increasingly recognized as etiologic factors linked to the pathogenesis and development of cerebrovascular anomalies. Their identification allows for advanced screening and targeted therapeutic approaches. The authors aimed to describe the role of a collaborative approach to care and genetic testing in pediatric patients with neurovascular anomalies, with the objectives of identifying what genetic testing recommendations were made, the yield of genetic testing, and the implications for familial screening and management at present and in the future. METHODS:

How we manage adults with myelodysplastic syndrome.

The prognosis in Myelodysplastic syndromes (MDS), although recently refined by molecular studies, remains largely based on conventional prognostic scores [International Prognostic Scoring System (IPSS), revised IPSS], classifying patients into "lower risk" MDS (LR-MDS) and "higher risk" MDS (HR-MDS). In LR-MDS, treatment mainly aims at improving cytopenias, principally anaemia, while in HR-MDS it aims at delaying disease progression and prolonging survival. In LR-MDS without deletion 5q, anaemia is generally treated first by erythropoietic stimulating factors, while second line treatments are

Follicular localization of growth differentiation factor 8 and its receptors in normal and polycystic ovary syndrome ovaries.

Polycystic ovary syndrome (PCOS) is the most common endocrine disorder in women of reproductive age and its etiology has not been characterized. Growth differentiation factor 8 (GDF8) is a member of the transforming growth factor-β superfamily that plays a critical role in the regulation of ovarian functions. However, the expression pattern of GDF8 in the human ovary is not yet clear. This study examined the cellular distribution of GDF8 and its putative cellular receptors (ACVR2A, ACVR2B, and ALK5) in a series of normal (n = 34) and PCOS ovaries (n = 14). The immunostaining of GDF8, ACVR2A, A

Rendu-Osler-Weber disease: update of medical and dental considerations.

Rendu-Osler-Weber disease, also known as hereditary hemorrhagic telangiectasia (HHT), is an autosomal dominant inherited disorder characterized by an aberrant vascular development. The reported prevalence is approximately 1 per 5,000-10,000. The clinical manifestations consist of recurrent spontaneous nosebleeds, telangiectasias characteristically at the lips, oral cavity, fingers, and nose, and visceral arteriovenous malformations. Timely recognition of this syndrome makes screening for complications, preventive measurements, and genetic counselling possible. The important role of the dental

Localized episcleral ALK-positive histiocytosis: a novel adult presentation confirmed by ALK gene rearrangement.

We report the first documented case of isolated episcleral ALK-positive histiocytosis (ALK+H) in an adult. A 36-year-old woman presented with an episcleral nodule clinically mimicking nodular scleritis. Excisional biopsy revealed sheets of foamy CD68+/CD163+ histiocytes with Touton-type giant cells. Immunohistochemistry demonstrated strong cytoplasmic ALK1 expression with negative CD1a and BRAF V600E, and fluorescence in situ hybridization confirmed ALK gene rearrangement in 60% of nuclei. Whole-body PET-CT identified a suspicious inguinal node, subsequently proven reactive on biopsy. Followin
