Cortical morphometric gradients reveal molecular and cognitive underpinnings of bipolar disorder.

BACKGROUND: Structural brain alterations in bipolar disorder (BD) have been widely reported, yet the hierarchical organization of cortical morphometric networks and their molecular and cognitive underpinnings remain unclear. METHODS: We applied the morphometric inverse divergence (MIND) network approach to structural MRI data from 49 BD patients and 119 healthy controls. Principal MIND gradients were derived using diffusion map embedding, followed by multiscale analyses linking gradient alterations to neurotransmitter systems, cognitive-behavioral domains, and transcriptomic profiles from the…

Application of the Random Forest Algorithm for Accurate Bipolar Disorder Classification.

Bipolar disorder (BD) is a complex psychiatric condition characterized by alternating episodes of mania and depression, posing significant challenges for accurate and timely diagnosis. This study explores the use of the Random Forest (RF) algorithm as a machine learning approach to classify patients with BD and healthy controls based on electroencephalogram (EEG) data. A total of 330 participants, including euthymic BD patients and healthy controls, were analyzed. EEG recordings were processed to extract key features, including power in frequency bands and complexity metrics such as the Hurst …

Communication with children about parental bipolar disorder: a qualitative interview study.

BACKGROUND: The impacts of parental bipolar disorder (BD) on families and children highlight the need to understand how best to talk to children about their parents' diagnosis, especially as their developmental capacity for understanding grows. This qualitative study aims to explore the strategies, challenges, and support needs of parents in relation to communicating with their children (5-12 years) about BD, in order to inform the development of further interventions and resources. METHODOLOGY: Purposive and snowball sampling strategies were used to recruit parents with BD, their partners, a…

Addressing the unmet needs of bipolar disorder in Australia and beyond.

People impacted by bipolar disorder are confronted by many unmet needs that contribute to the overall burden associated with the disorder. We do not have a good understanding of the underlying pathology of bipolar disorder, so we do not have biomarkers to accurately identify those who are at risk of developing the disorder. Delayed diagnosis is the norm, and it can take a decade or more for an individual to receive a diagnosis and to start appropriate treatment. We have evidence-based treatments such as lithium and psychosocial therapies; however, their availability and use are limited. We nee…

Novel and Emerging Treatments for Agitation in Schizophrenia and Bipolar Disorder.

Background: Agitation is a frequent and challenging symptom in schizophrenia and bipolar disorder, characterized by heightened motor activity, emotional distress, and potential aggression. This symptom is most observed during acute episodes, representing a significant burden on patients, caregivers, and healthcare systems. Agitation is a leading cause of emergency department visits and psychiatric hospitalizations, necessitating prompt and effective interventions to ensure safety and mitigate its far-reaching impact. Traditional treatments, including high-potency antipsychotics and benzodiazep…

Peripheral neurofilament light chain and intracortical myelin in bipolar I disorder.

BACKGROUND: Neurofilament light chain (NfL) is a cytoskeletal protein that supports neuronal structure. Blood NfL levels are reported to be higher in diseases where myelin is damaged. Studies investigating intracortical myelin (ICM) in bipolar disorder (BD) have reported deficits in ICM maturation over age. This study investigated the association between ICM and peripheral blood NfL levels in BD. METHODS: NfL was quantified using a high sensitivity ELISA kit in 72 BD and 71 healthy control (HC) participants. t-test was used to determine group difference in NfL levels between BD and HC, and ri…

Digital Biomarkers for Passive Remote Monitoring of Bipolar Disorder: Systematic Review.

BACKGROUND: Bipolar disorder (BD) features episodic shifts among (hypo)mania, depression, mixed states, and euthymia. Timely detection of mood transitions is difficult due to infrequent clinical touchpoints. Digital health technologies, including wearables and smartphones, offer a unique opportunity to passively and continuously monitor behavior and physiology that could reflect underlying mood dynamics in real-world settings. OBJECTIVE: We aim to systematically review passively collected digital biomarkers for BD mood states, characterize devices/modalities and analytic approaches, appraise …

Enhancing Early Diagnosis of Bipolar Disorder in Adolescents Through Multimodal Neuroimaging.

BACKGROUND: Bipolar disorder (BD), a severe neuropsychiatric condition, often appears during adolescence. Traditional diagnostic methods, which primarily rely on clinical interviews and single-modal magnetic resonance imaging (MRI) techniques, may have limitations in accuracy. This study aimed to improve adolescent BD diagnosis by integrating behavioral assessments with multimodal MRI. We hypothesized that this combination would enhance diagnostic accuracy for at-risk adolescents. METHODS: A retrospective cohort of 309 participants, including patients with BD, offspring of patients with BD (w…

Concussion May Result in New-Onset Bipolar Disorder: A Case Report.

Emotional dysregulation following a concussion is well established. New onset of major psychiatric diseases such as bipolar disorder (BPD) post-concussion has not been investigated. BPD typically presents with an initial depressive episode followed by mania and concurrent depressive and manic states. Multiple theories explaining the etiology of BPD exist, including the diathesis-stress model (DiSM), though an accepted theory is not established. In this case study, medical records of a 50-year-old ambidextrous male with co-morbid attention deficit hyperactivity disorder (ADHD) inattentive type,…

New onset gender dysphoria in bipolar affective disorder - A case series.

Gender confusion in the context of mania is very less frequently described in the literature. The actuality of a primary psychiatric condition in gender identity complaint has significant bearing on the applicable operation and prognostic. This case series describes cases of bipolar affective complaint presenting in a manic occasion whose mania was marked by hypersexuality and the desire to be of opposite gender. Both of these symptoms resolved with treatment of the manic occasion. Case 1 describes a 17-year-old male presenting with an episodic illness, with current manic episode. He is curren
