Understanding the Risk Factors and Pathogenesis of Disseminated Nocardiosis in Immunocompromised Patients.

Nocardia is a genus of aerobic, Gram-positive, partially acid-fast, filamentous bacilli notoriously known for causing multisystemic infections in immunocompromised individuals. Notably, this genus of bacteria commonly infects the pleural and central nervous system, leading to pneumonia and brain abscesses, respectively. Our patient is a 71-year-old female who initially presented to the emergency department complaining of shortness of breath and altered mental status. Imaging revealed multiple enhancing brain lesions, a pleural effusion, and a paraspinal abscess, which upon aspiration and cultu…

A case of nocardiasis complicated with meningitis in a patient with immune thrombocytopenic purpura.

Nocardia infection is a well recognized complication of the immunocompromised hosts. It is mostly a primary pulmonary infection, which may disseminate to other organs. The central nervous system (CNS) involvement of nocardiosis is usually manifested as brain abscesses. We report a 25-year-old male patient who presented with nocardial pneumonia and meningitis without brain abscess. He was diagnosed as immune thrombocytopenic purpura and methyl prednisolone was started 5 weeks previously. Nocardia spp. was obtained from his cerebrospinal fluid culture, but he died at the 7th day of intensive car…

Serial MRI in listeria mesenrhombencephalitis: a case report.

Listeria mesenrhombencephalitis is a rare disorder, especially in the non-immunocompromised host. The predilection for brain stem involvement is unexplained. The anatomical correlation of brain stem signs with serial MRI over three months is shown in a 64 year old white man. The pathological lesions seemed to be a combination of micro-abscesses and associated oedema.

Post-traumatic intracranial epidural Aspergillus fumigatus abscess.

We report an intracranial epidural abscess caused by Aspergillus fumigatus in an immunocompetent patient. Infection occurred in a 20-year-old man 2 months after a frontal craniotomy following trauma. The abscess was encapsulated by a thickened dura and although the fungus did not invade the brain, frontal bone was infected and the patient presented with a subcutaneous frontal cellulitis. Initial management combined surgical drainage, resection of necrotic bone and liposomal amphotericin B (1 mg kg-1 per day). After 3 weeks of antifungal treatment a second evaluation surgery was performed. A cl

Severe complications of perianal sepsis in patients with human immunodeficiency virus.

Fifty human immunodeficiency virus (HIV)-infected patients with perianal sepsis were studied. Seven (14 per cent) had serious septic complications, four patients with severe necrotizing gangrene, and three with abscesses in the mediastinum, liver and brain respectively. CD4+ lymphocyte counts were significantly lower in patients with severe septic complications as compared with those with uncomplicated perianal sepsis (P < 0.05). In patients with HIV presenting with rare (metastatic) abscesses, perianal sepsis must always be kept in mind as a possible focus. Although HIV-infected patients have

A case report on the co-presentation of pituitary abscess and POEMS-like syndrome.

RATIONALE: POEMS syndrome is a rare paraneoplastic syndrome, manifesting with progressive distal polyneuropathy, organomegaly, endocrinopathy, monoclonal protein, and skin changes, which has multisystem manifestations and chronic cytokine overproduction. Pituitary abscess is also a rare life-threatening disease that could lead to panhypopituitarism. We report a case of pituitary abscess with high suspicion of POEMS syndrome, which to our knowledge has not been reported previously. PATIENT CONCERNS: The co-presentation of pituitary abscess and POEMS-like syndrome is rare and its clinical manif

Neuroimaging of infections.

Neuroimaging plays a crucial role in the diagnosis and therapeutic decision making in infectious diseases of the nervous system. The review summarizes imaging findings and recent advances in the diagnosis of pyogenic brain abscess, ventriculitis, viral disease including exotic and emergent viruses, and opportunistic disease. For each condition, the ensuing therapeutic steps are presented. In cases of uncomplicated meningitis, cranial computed tomography (CT) appears to be sufficient for clinical management to exclude acute brain edema, hydrocephalus, and pathology of the base of skull. Magneti…

Successful management of cerebral and pulmonary mucormycosis with liposomal amphotericin B in a 28-year-old woman with acute lymphoblastic leukemia.

A 28-year-old woman with acute lymphoblastic leukemia developed fever and unilateral pleural based pulmonary infiltrate during prolonged chemotherapy induced neutropenia. CT-guided lung biopsy confirmed the diagnosis of pulmonary mucormycosis and liposomal amphotericin B therapy was started. A few days after the initial symptoms, the patient developed convulsions and a brain abscess was detected in computerized tomography and magnetic resonance imaging. Fungal hyphae detected in histopathological examination of a brain biopsy had identical morphology with those seen in previous lung biopsies. …

Clinical features, microbiology, and management of pediatric brainstem abscess.

PURPOSE: Brainstem abscess is a rare condition accounting for merely 1% of brain abscesses incidence in the pediatric population. This study aimed to present a single patient with a pontine abscess and review the literature to highlight clinical features, diagnosis, and management of brainstem abscess. METHODS: The PubMed database was screened for English-language articles concerning pediatric brainstem abscess. We, therefore, identified 22 publications, which concisely depict 23 cases. Our study reports on the 24th pediatric patient diagnosed with that entity. All included reports were analy…

Contiguous spread of melioidosis to the brain: a rare case of subdural empyema following odontogenic infection. Illustrative case.

BACKGROUND: Melioidosis, caused by Burkholderia pseudomallei, is an opportunistic infection endemic to Southeast Asia. CNS involvement is rare, but is associated with significant morbidity and mortality. Subdural empyema arising from odontogenic melioidosis has been rarely reported. OBSERVATIONS: A 50-year-old male with newly diagnosed diabetes mellitus presented with progressive right facial swelling, fever, and dizziness 1 week following dental extraction. CT of the head and neck revealed a right masticator space abscess with extension into the parotid and extraconal orbital spaces. Blood c

Giant hemorrhagic and abscessed pediatric high-grade glioma with osteolysis and transcalvarial herniation: A unique presentation and literature review.

INTRODUCTION: High-grade gliomas (HGGs) in very young children are rare, with intratumoral abscess formation being exceptionally uncommon and primarily reported in adults. We present a unique pediatric case with this rare complication. CASE REPORT: A 5-month-old female infant, born after a complicated pregnancy, presented with rapidly progressive intracranial pressure. Imaging revealed a large, hemorrhagic, left parieto-occipital lesion with osteolysis and subsequent transcalvarial herniation. Intraoperatively, a culture-negative abscess was found within the tumor. Initial biopsy suggested a

Diffusion-Weighted Imaging of Intracranial Ring-Enhancing Lesions.

Ring-enhancing magnetic resonance imaging (MRI) lesions result from various diseases, including infection, neoplasia, inflammation, and vascular etiologies. Differentiation based on standard MRI sequences can be challenging. This study aims to compare the apparent diffusion coefficient (ADC) values of intracranial ring-enhancing lesions of infectious etiology with ring-enhancing lesions caused by other etiologies. Records were reviewed for MRI studies with diffusion-weighted imaging (DWI) and post-gadolinium T1-weighted ring-enhancing lesions with a definitive histopathological diagnosis or a

Neurological Complications of Acute and Chronic Sinusitis.

PURPOSE OF REVIEW: Acute and chronic sinusitis can give rise to a wide array of intracranial and orbital complications. These complications include brain abscess, subdural empyema, epidural abscess, meningitis, venous sinus thrombosis, frontal bone osteomyelitis, and orbital cellulitis and abscess. Despite numerous medical advances, these complications carry a risk of mortality and significant morbidity. RECENT FINDINGS: Recent studies have shown improvement in both the mortality and the morbidity associated with the neurologic complications of acute and chronic sinusitis. However, there are …

Listeria monocytogenes-infected bone marrow myeloid cells promote bacterial invasion of the central nervous system.

Listeria monocytogenes is a facultative intracellular pathogen that is able to invade the central nervous system causing meningoencephalitis and brain abscesses. The mechanisms allowing bacteria to cross the blood-brain barrier are poorly understood. In this work, we used an experimental model of acute listeriosis in the mouse inducing a reproducible invasion of the central nervous system. At the early phase of infection, we find that bacteria invade and rapidly grow in bone marrow cells identified as bone marrow myelomonocytic cells expressing the phenotype CD31pos:Ly-6Cpos:CD11b(pos):LY-6Glo…

Meningitis caused by Streptococcus in adults.

Meningitis caused by Streptococcus is uncommon in adults. Otitis media, mastoiditis, sinusitis, or trauma has antedated most cases reported in the past. Data on 10 recent cases suggest changes in the current pathogenesis of streptococcal meningitis in adults and emphasize the importance of accurate streptococcal speciation. Endocarditis was present in five cases; trauma played a role in two others. Brain abscess, corticosteroids, alcoholic cirrhosis, and peritonitis secondary to chronic peritoneal dialysis were etiologic factors in five patients. Otitis media, mastoiditis, and sinusitis were c…
