C1 fracture: Analysis of consolidation and complications rates in a prospective multicenter series.

INTRODUCTION: Three types of C1 fracture have been described, according to location: type 1 (anterior or posterior arc), type 2 (Jefferson: anterior and posterior arc), and type 3 (lateral mass). Stability depends on transverse ligament integrity. The main aim of the present study was to analyze complications and consolidation rates according to fracture type, age and treatment. MATERIAL AND METHODS: The French Society of Spinal Surgery (SFCR) performed a multicenter prospective study on C1-C2 trauma. All patients with recent fracture diagnosed on CT were included. Consolidation on CT was stu

Assuring optimal physiologic craniocervical alignment and avoidance of swallowing-related complications after occipitocervical fusion by preoperative halo vest placement.

STUDY DESIGN: A retrospective review. OBJECTIVE: To assess the utility of preoperative halo immobilization in the avoidance of swallowing complications-associated occipitocervical fixation. SUMMARY OF BACKGROUND DATA: The craniocervical region is commonly affected by a number of pathologic processes. Fixation of the upper cervical spine to the occiput provides an excellent means of treating these conditions. Occipitocervical fixation, however, is associated with a number of potential complications. One under-reported postoperative complication is the swallowing difficulty that some patients

Thoracic Discogenic Syndrome

Symptomatic thoracic discogenic syndrome poses a diagnostic challenge due to its rarity. The relative immobility of the thoracic spine due to its particular orientation, structure, and function in the vertebral column contributes to the rarity of thoracic discogenic syndrome. At birth, the thoracic spine and sacrum exhibit a kyphotic curvature, whereas the cervical and lumbar spine develop full lordosis around puberty. The lordotic nature of the cervical and lumbar spine enables them to bear most of the axial skeleton's weight compared to the thoracic and sacral spine. As a result, the latter

The transverse occipital ligament: an anatomic, histologic, and radiographic study.

BACKGROUND CONTEXT: The craniocervical region is an osteoligamentous complex that provides structural stability and movement by means of numerous ligaments. Fundamental knowledge of these ligaments is important for physicians who treat patients with disorders of this region to reduce morbidity and mortality. There is a paucity of data in the literature regarding the morphology, function, and classification of the transverse occipital ligament (TOL). PURPOSE: The purpose of this study was to investigate the prevalence, morphology, and variations of the TOL in a large number of adult human cada

Endoscopic Endonasal Odontoidectomy with Anterior C1 Arch Preservation in Rheumatoid Arthritis: Long-Term Follow-Up and Further Technical Improvement by Anterior Endoscopic C1-C2 Screw Fixation and Fusion.

OBJECTIVE: To examine the long-term outcomes (minimum of 4.5 years) of endoscopic endonasal odontoidectomy (EEO) with preservation of anterior C1 ring to treat irreducible ventral bulbo-medullary compressions in rheumatoid arthritis (RA) and to illustrate a novel technique of anterior pure endoscopic craniovertebral junction (CVJ) reconstruction and fusion. In fact, long-term clinical studies are still lacking to elucidate the effective role of EEO and whether it can obviate the need for posterior fixation. METHODS: From November 2008 to January 2012, clinical and radiologic data of 7 patient

C1 lateral mass fixation: a comparison of constructs.

OBJECTIVE: We review our experience and technique for C1 lateral mass screw fixation. We compare the results of 3 different constructs incorporating C1 lateral mass screws: occipitocervical (OC) constructs, C1-C2 constructs, and C1 to mid/low cervical constructs. METHODS: We performed a retrospective chart review of 42 consecutive patients who underwent C1 lateral mass fixation by 2 of the authors (PVM and DC). The patient population consisted of 24 men and 18 women with a mean age of 64 years. Twenty-two patients had C1-C2 constructs. Twelve patients had constructs that started at C1 and ext

Surgical approach for hypoglossal schwannomas to prevent deformity of the atlanto-occipital joint.

BACKGROUND: Hypoglossal schwannomas are very rare tumors that often enlarge the hypoglossal canal and jugular foramen, and erode the bone bridge of the occipital condyle. We compared pre- and postoperative 3D bone CT images and discussed the ideal craniotomy to prevent fracture formation. METHOD: Seven patients with hypoglossal schwannomas underwent surgery in our department. Six cases were type B and 1 case was type C. All patients complained of hypoglossal nerve paresis and nuchal pain. FINDINGS: We used the condylar fossa approach for four cases of type B, the lateral suboccipital approac

Ideal screw entry point and projection angles for posterior lateral mass fixation of the atlas: an anatomical study.

Although various posterior insertion angles for screw insertion have been proposed for C1 lateral mass, substantial conclusions have not been reached regarding ideal angles and average length of the screw yet. We aimed to re-consider the morphometry and the ideal trajections of the C1 screw. Morphometric analysis was performed on 40 Turkish dried atlas vertebrae obtained from the Department of Anatomy at the Medical School of Ankara University. The quantitative anatomy of the screw entry zone, trajectories, and the ideal lengths of the screws were calculated to evaluate the feasibility of post

C1 arch regeneration, tight cisterna magna, and cervical syringomyelia following foramen magnum surgery.

BACKGROUND: Although bone regrowth following craniocervical decompression has been rarely reported to cause late recurrence of Chiari symptoms, syringomyelia has not been observed in such cases. We report a unique case of cervical syringomyelia resulting from spontaneous regeneration of the posterior C1 arch after foramen magnum decompression. CASE DESCRIPTION: A 38-year-old male patient underwent resection of a symptomatic foramen magnum meningioma. Three years later, he developed neuropathic pain in his left upper extremity with worsening dysphagia and dysphonia. MRI revealed regeneration o

Long-term health-related quality of life outcomes following Jefferson-type burst fractures of the atlas.

OBJECT: The primary goal of this study was to describe the long-term health-related quality of life (HRQOL) outcomes in patients who have suffered Jefferson-type fractures. These outcomes were compared with matched normative HRQOL data and with the patient's perceptions of their HRQOL prior to the injury. Variables that potentially influence these HRQOL outcomes were analyzed. No standardized outcome assessments have been published for patients who suffer these fractures; their outcomes have long thought to be excellent following treatment. Determining the optimal surrogate measure to represen

Assessment of adult skeletons to detect prenatal exposure to trypan blue in mice.

A Skeletal Variant Assay System (SVAS) consisting of a group of 88 spontaneously occurring qualitative variations of the adult mouse skeleton was studied in CD-1 mice which had been exposed in utero by way of three daily ip injections of their dams on days 7-9 of gestation with trypan blue. Treatment groups received daily doses of 0.25 cc of 0, .037, .075, .15, or .30% trypan blue dissolved in 0.9% NaCl. Two separate series of experiments were performed, and skeletons were examined at 62 +/- 2 days postnatal. Sixty-six and 58 of the variants occurred in the two series, respectively. Frequencie

Clinical Application of Diagnostic Imaging of Chiari-Like Malformation and Syringomyelia.

Chiari-like malformation (CM) and syringomyelia (SM) is a frequent diagnosis in predisposed brachycephalic toy breeds since increased availability of MRI. However, the relevance of that MRI diagnosis has been questioned as CM, defined as identification of a cerebellar herniation, is ubiquitous in some breeds and SM can be asymptomatic. This article reviews the current knowledge of neuroanatomical changes in symptomatic CM and SM and diagnostic imaging modalities used for the clinical diagnosis of CM-pain or myelopathy related to SM. Although often compared to Chiari type I malformation in huma
