INTRODUCTION: Corporoplasty is typically accompanied by circumcision to prevent complications such as phimosis, foreskin edema, and necrosis, yet supporting evidence is limited. In most boys referred for circumcision preputial adhesions are the only problem, but these can predispose to recurrent balanitis. A simple technique using Emla cream (eutectic mixture of lignocaine and prilocaine) has been devised which allows the adhesions to be separated painlessly in the outpatient clinic. Histologic examination of the prepuces removed from 78 consecutive patients with phimosis was performed during a 2-year period. In 15 cases (14.2%), the typical changes of lichen sclerosus et atrophicus (LSA) were found. A different, distinct histologic pattern of fibrosis was noticed in six patients (7.7%). Clinically, the two conditions were indistinguishable and presented as severe, nonretractable phimosis. Caudal anaesthesia in pediatric surgery about 116 cases (children from three weeks to 18 years with 64 below 5 years) is reported. The caudal anaesthesia, at 6 mg/kg, is very usefull for surgery below T 10 because: --it can be used in day surgery, --it avoids intubation and its complications, --it allows feeding as soon as required, --it gives a post operative analgesia. In our practice, this technique is safe and is real progress compare to the general anaesthesia.
