Robot-Assisted Surgery Performed Using a Fluorescent Ureteral Catheter for Locally Advanced Sigmoid Colon Cancer: A Case Report.

In recent years, robotic surgery for colorectal cancer has increased and is now being performed even for locally advanced colorectal cancers. However, due to adhesions and tissue fibrosis caused by infiltration and inflammation, the risk of misidentification during dissection is high. In robotic surgery, which lacks tactile feedback, particular caution is required to avoid injuries such as ureteral damage. Navigation surgery using fluorescent ureteral catheters that emit fluorescence upon near-infrared light irradiation has attracted attention. In this report, we describe a case where a safe r

Genetic analysis for diagnosing local recurrence of sigmoid colon cancer mimicking a small intestinal tumor: a case report.

BACKGROUND: With recent advances in genetics research, genetic analysis is increasingly being used in clinical practice. We report a case in which genetic analysis aided in diagnosing a local recurrence of sigmoid colon cancer, initially suspected to be a primary neoplasm of the small intestine. CASE PRESENTATION: A 61-year-old male underwent laparoscopic sigmoidectomy for stage IIIB sigmoid colon cancer, followed by 8 cycles of CAPOX adjuvant chemotherapy, one and a half years prior. A follow-up CT scan performed one and a half years postoperatively revealed a mass in the small intestine nea

Minimally invasive surgical treatment for sigmoid colon cancer in a patient with situs inversus totalis: a case report.

Situs inversus totalis (SIT) is a rare congenital condition characterized by a complete reversal of the internal organ arrangement, presenting significant surgical challenges. While previous cases of colorectal cancer in SIT patients have been reported, cases involving sigmoid colon cancer are sporadic. Here, we present the case of a 68-year-old female with sigmoid colon cancer and SIT. Following thorough preoperative preparation and a multidisciplinary team (MDT) consultation, the patient successfully underwent a laparoscopic radical sigmoidectomy. Her postoperative recovery was smooth, and s

Pathological Complete Response in Locally-advanced MSI-H/dMMR Sigmoid Colon Cancer Treated With Pembrolizumab and Abdominal Reconstruction.

BACKGROUND/AIM: Microsatellite instability-high (MSI-H) and mismatch repair-deficient (dMMR) colorectal cancers have shown significant responses to immune checkpoint inhibitors such as pembrolizumab, presenting novel opportunities for potentially curative strategies in cases previously deemed unresectable. CASE REPORT: A 61-year-old Japanese man presented with abdominal pain and was diagnosed with locally advanced sigmoid colon cancer with abdominal wall invasion. The tumor also involved the small intestine and had formed a fistula extending to the skin. Imaging studies revealed no distant me

Solitary Lymph Node Metastasis of Sigmoid Colon Cancer to Posterior Site of Inferior Vena Cava: A Case Report.

Colorectal cancer (CRC) remains a leading cause of cancer-related mortality worldwide. Despite advances in surgical and systemic treatments, recurrence continues to impact prognosis. Paraaortic lymph node metastasis (PALM) is an uncommon site of recurrence, occurring in only 1-2% of CRC cases. Among PALMs, solitary lymph node metastasis located posterior to the inferior vena cava (IVC) is exceptionally rare and challenging to diagnose. We present the case of a 66-year-old woman initially diagnosed with sigmoid colon cancer and multiple liver metastases. Following laparoscopic sigmoidectomy for

Total neoadjuvant therapy and abdominoperineal resection with sigmoid colon vaginoplasty for locally advanced rectal adenocarcinoma invading the vagina.

This report presents a woman in her 70s with locally advanced rectal adenocarcinoma invading the posterior vaginal wall. She received total neoadjuvant therapy (TNT) consisting of six cycles of capecitabine-oxaliplatin followed by long-course chemoradiotherapy, resulting in significant tumour downstaging. Definitive surgery involved extralevator abdominoperineal excision with near-total vaginectomy and reconstruction using a sigmoid colon flap vaginoplasty. Histopathology confirmed an R0 resection, with two of 17 lymph nodes positive for metastasis. The patient recovered uneventfully, with a v

Management of Perforated Sigmoid Colon Cancer in an Incarcerated Inguinal Hernia Using a Combined Abdominal and Inguinal Approach.

The ideal management of perforated colorectal cancer in the hernia sac remains a challenge for general surgeons. We report such a case requiring a combined-incision surgical approach and two-stage operation. A male patient in his 80s presented with fatigue and a bulge in his left groin. Contrast-enhanced CT scan revealed a sigmoid colon perforation within the left inguinal hernia sac. A combination of midline abdominal and inguinal incisions was performed for the iliopubic tract repair and Hartmann procedure. Six months later, the recurrent inguinal hernia was repaired using the Kugel mesh. Th

[A Case of Local Recurrent Liver Metastasis from Sigmoid Colon Cancer after Complete Histological Remission, Treated with Rehepatectomy].

We report a case of a patient with sigmoid colon cancer and multiple liver metastases who underwent hepatectomy after chemotherapy and pathological results showed complete remission. However, after chemotherapy was discontinued, the patient developed a local recurrence of the liver metastasis and underwent rehepatectomy. The patient came to our hospital with lower abdominal pain. Colonoscopy revealed a circumferential type Ⅱ, well-differentiated adenocarcinoma. Laparoscopic sigmoidectomy with lymph node dissection was performed. Postoperative CT scan showed multiple liver metastases at S5, S7,

Abscopal effect triggered by radiation sequential mono-immunotherapy resulted in a complete remission of PMMR sigmoid colon cancer.

BACKGROUND: Radiation therapy combined with immune checkpoint inhibitors (ICIs) has recently turned into an appealing and promising approach to enhance the anti-tumor immunity and efficacy of immunological drugs in many tumors. Abscopal effect induced by radiation is a phenomenon that often leads to an efficient immunity response. In this study, we investigated whether the combination of the immunogenic effects derived from radiotherapy sequential ICIs-based therapy could increase the incidence of abscopal effects, and improve the survival rates. CASE PRESENTATION: We described a clinical cas

Endoscopic ultrasound-guided transrectal drainage of a pelvic abscess after Hinchey II sigmoid colon diverticulitis: A case report.

BACKGROUND: Acute diverticulitis is one of the most prevalent complications of diverticular disease and may result in abscess formation, perforation, fistula formation, obstruction, or bleeding. Diverticular abscesses may be initially treated with antibiotics and/or percutaneous drainage and/or surgery. Endoscopic ultrasound (EUS)-guided drainage techniques are increasingly used as a minimally invasive alternative to percutaneous or surgical approaches, as they are associated with better treatment outcomes, shorter recovery time and duration of hospitalization. CASE SUMMARY: A 57-year-old fem

Glomerular Microangiopathy with Cellular Crescent-like Formation and Endotheliopathy Due to Ramucirumab Treatment for Metastatic Sigmoid Colon Cancer.

We encountered a 77-year-old Japanese man who presented with nephrotic-range proteinuria 20 days after receiving ramucirumab treatment for metastatic sigmoid colon cancer. A kidney biopsy showed two characteristic histological findings. The first finding was podocyte injury with cellular crescent-like formation, although focal segmental glomerulosclerosis (FSGS) (collapsing variant) according to the Columbia classification may have been a more appropriate name for this injury, as hypertrophy and hyperplasia of epithelial cells, presumably resulting from podocyte injury, were seen between Bowma

[A Case of Sigmoid Colon Cancer with Retroperitoneal Fibrosis Using Intraoperative Indocyanine Green Fluorescence Angiography for Colorectal Anastomosis].

A 72-year-old man was presented with anemia and diagnosed with sigmoid colon cancer by colonoscopy. CT showed a soft tissue density around the retroperitoneum, leading to the diagnosis of retroperitoneal fibrosis. Stenosis of left ureter, inferior mesenteric artery, and left colic artery due to the soft tissues were detected. Sigmoidectomy and retroperitoneal biopsy were performed, and colorectal anastomosis was completed after confirming the intestinal blood flow by ICG fluorescence angiography. In retroperitoneal fibrosis, identifying blood vessels intraoperatively can be difficult. ICG fluo
