OBJECTIVE: The aim of this study is to follow longitudinally the prominent ears treated by percutaneous adjustable closed otoplasty (PACO) and evaluate this procedure in terms of technical efficiency, recurrence, complications, and patient satisfaction.
MATERIALS AND METHODS: Percutaneous adjustable closed otoplasty was used to treat 28 ears in 15 patients presenting with prominent ear deformities. To determine the success of each operation, distances between the helical rim and scalp were measured at 4 levels preoperatively, at the end of the surgery and again postoperatively at the first we…

Microvascular replantation, when possible, is the treatment of choice for total ear amputations. Both arterial and venous reconstruction should be attempted. The present case report describes a successful total ear replantation in a 45-year-old woman whose ear was amputated due to a horse accident. Venous thrombosis subsequently occurred and was managed with anticoagulation and leech therapy. Eighty hours after the replantation, arterial thrombosis took place. The posterior auricular artery thrombosed anastomosis was resected and reconstructed with an interposition vein graft. This report illu…

Office-based transtympanic endoscopy through the perforation made by laser-assisted myringotomy (LAM) has been introduced for diagnosis of ossicular interruption, stapes fixation, tympanosclerosis, congenital cholesteatoma, and perilymphatic fistula. Endoscopy of the middle ear reveals minor anomalies around the incudostapedial joint and the superstructure of the stapes. This procedure can be also applied to diagnose stapes fixation and perilymphatic fistula not diagnosed presurgically, and requires exploratory tympanotomy to make a definite diagnosis. Transtympanic endoscopy using LAM is a di…

Nystatin is a topical antifungal agent. Its wide spectrum of action and low cost make it a treatment of choice. Though it is safe for external ear use, no study has proven its safety in cases of tympanic membrane perforation (TMP). We aim to test the safety of Nystatin when applied directly to the middle ear of a Guinea Pig model. We performed an experimental study with 18 Hartley Guinea Pigs that were divided into two groups. All tympanic membranes were perforated at the beginning of the study. Exposing one group to Nystatin and the other to the ototoxic Neomycin, we compared results of audit…

Floating-Harbor syndrome is a rare syndrome of unknown etiology, which was first described in 1973. A triad of main features characterizes Floating-Harbor syndrome: short stature, characteristic face, and an expressive speech delay. We present a patient in whom the hearing thresholds improved insufficiently after placement of grommets. High-resolution CT scan of the temporal bone showed a prominent soft-tissue thickening suspected of causing fixation of the malleus, and fusion of the malleus head with the body of the incus. To our knowledge this is the first reported abnormal middle ear anatom…

CONCLUSION: Dysequilibrium is one of the most important side effects of ear surgery. The subjective visual vertical can be used as a good indicator for the evaluation of otolithic function in patients with ear surgery.
OBJECTIVE: To investigate the influence of various types of ear surgery on the otolithic organs.
METHODS: Seventy-one patients underwent ear surgery. Subjective visual vertical (SVV) test was performed before and after ear surgery. We investigated the directional changes of SVV before and after the ear surgery.
RESULTS: The postoperative SVV of two patients who underwent tran…

CONCLUSIONS: The results reported here provide the first evidence of the production of superoxide, a biologically relevant reactive oxygen species (ROS), in human inner ear perilymph (hIP) in pathological conditions, by the activity of the xanthine dehydrogenase/xanthine oxidase (XA/XO) enzyme system.
OBJECTIVE: To investigate the presence of ROS in hIP.
METHODS: Since damage and apoptosis of inner ear hair cells may occur as a result of ROS-mediated injury, we investigated the presence and production of ROS in 105 hIP samples; 98 collected from patients affected by profound sensorineural he…

BACKGROUND: Keloid is not a homogeneous biological entity. The exact pathologic mechanism of the lesion continues to be an enigma for physicians and researchers alike and no specific gene or sets of genes have been identified. The art of body piercing is ancient to people in the developed countries but over years it has rejuvenated to part of their fashion process. Ear-piercing or extra ear-piercing is becoming more rampant among youths in this part of the world, this however have been shown to cause hypertrophic scaring, keloids and its attendant cosmetic problems.

BACKGROUND: Ear reconstruction is challenging surgery, often with poor outcomes. Our purpose was to develop a surgical training model for auricular reconstruction.
METHODS: Silicone costal cartilage models were incorporated in a workshop-based instructional program. Trainees were randomly divided. Workshop group (WG) participated in an interactive session, carving frameworks under supervision. Nonworkshop group (NWG) did not participate. Standard Nagata templates were used. Two further frameworks were created, first with supervision then without. Groups were combined after the first carving b…

BACKGROUND/AIM: The use of intratympanic steroids (ITS) has proliferated over the past 10-15 years to include treatments for inner ear disorders, like Ménière's Disease (MD) and sudden sensorineural hearing loss (SSNHL). The aim of this study was to review the clinical trials of ITS for inner ear disorders.
METHODS: PubMed and Ovid Medline databases were searched from 1966 to present for clinical trials on ITS in the treatment of MD and SSNHL. Studies were evaluated based on comparability and internal and external validity.
RESULTS: Thirty-eight studies were identified in total, 13 studies o…

Because the blood supply to the inner ear originates from the vertebrobasilar system, vertebrobasilar ischemic stroke can present with vertigo and hearing loss due to infarction of the inner ear (i.e., labyrinthine infarction). Sometimes vertigo and hearing loss are warning symptoms of impending vertebrobasilar ischemic stroke (mainly in the anterior inferior cerebellar artery territory). In this case, the magnetic resonance imaging (MRI) scan is normal and the clinician must rely on other clinical features to make the diagnosis. Here the authors review the keys to the diagnosis of vertigo and…

Presented here is a first-person account of the evolution of the practice of surgical neurootology to that of medical neurootology shaped mainly by results of treatment directed at underlying otosclerosis-like lesions of the otic capsule and metabolic factors. With new technologies and rapidly evolving concepts, the changing treatment algorithms did not remain constant to provide the usual evidence-based outcome analyses. However, the majority of the patients presenting with neurootological symptoms had undergone previous medical or surgical treatment before undergoing the medical management h

BACKGROUND: Mustardé sutures and conchal setback are widely used for surgical correction of prominent ear, and numerous cartilage-manipulation techniques accompanying these two methods are also available. However, it is unknown how each technique works biomechanically. The effects of otoplasty were evaluated by finite element analysis.
METHODS: Data of eight prominent ears were obtained with a noncontact three-dimensional digitizer, and three-dimensional auricular cartilage models were produced. These models were modified to cartilage-manipulation models: scaphal-incision, abrasion, conchal-i…

INTRODUCTION: Helix valgus or procident ears is a common problem that affects about 5% of the population. The folds of the antehelix and the overdevelopment of the concha are the most commonly found anatomic alterations of the ear pavilion. In children this pathology usually causes anxiety and an emotional trauma that may interfere in their normal development.
MATERIALS AND METHODS: There are a few tipes of techniques to correct helix valgus. We present the application of the technique in our service. We conduct the otoplastia with an outer puntiform technique which allows us to cut the carti…

Drop weld injuries to the tympanic membrane and middle ear caused by hot sparks or molten slag are a rare but significant injury. Steel workers and welders who are regularly exposed to flying sparks and molten metal slag are predisposed. This type of transtympanic thermal injury occurs when the slag literally drops into the external auditory canal and burns through the tympanic membrane. A spectrum of severity of injury occurs which includes chronic tympanic membrane perforation, chronic otorrhoea, facial nerve injury and deafness. Chronic tympanic membrane perforation is the most common seque…

Lidocaine is one of the most widely used local anesthetics. It can be administered topically or parenterally. Allergic contact dermatitis from lidocaine is uncommon, but an increasing number of cases have been reported in recent years. We present the case of a 66-year-old man who presented with acute dermatitis on the pinna and left cheek after applying ear drops. Patch tests were positive for the product itself and for lidocaine in the ear drops, thereby confirming the diagnosis of contact dermatitis from lidocaine.
