Anterior elbow dislocation with potential compartment syndrome: a case report.

Anterior elbow dislocation is an infrequent lesion, usually produced by direct trauma to the proximal ulna after a fall on the elbow in flexion, and is often associated with soft tissue injuries. The authors report a case of a complex injury produced by a high-energy trauma in the right arm of a 65-year-old patient. His limb was trapped inside an industrial spin-dryer, resulting in a closed anterior elbow dislocation, diaphyseal ulnar shaft, radial styloid process fractures, and an associated compartment syndrome. The injury mechanism and its treatment are described to better manage the soft t…

Treatment of chronic elbow tendinosis with buffered platelet-rich plasma.

BACKGROUND: Elbow epicondylar tendinosis is a common problem that usually resolves with nonoperative treatments. When these measures fail, however, patients are interested in an alternative to surgical intervention. HYPOTHESIS: Treatment of chronic severe elbow tendinosis with buffered platelet-rich plasma will reduce pain and increase function in patients considering surgery for their problem. STUDY DESIGN: Cohort study; Level of evidence, 2. METHODS: One hundred forty patients with elbow epicondylar pain were evaluated in this study. All these patients were initially given a standardized …

The thrower's elbow: arthroscopic treatment of valgus extension overload syndrome.

Injury to the medial collateral ligament of the elbow (MCL) can be a career-threatening injury for an overhead athlete without appropriate diagnosis and treatment. It has been considered separately from other athletic injuries due to the unique constellation of pathology that results from repetitive overhead throwing. The past decade has witnessed tremendous gains in understanding of the complex interplay between the dynamic and static stabilizers of the athlete's elbow. Likewise, the necessity to treat these problems in a minimally invasive manner has driven the development of sophisticated t…

Brachioradialis transposition for elbow extension in obstetrical brachial plexus palsy.

Disorders of elbow extension occur following traumatic or neurologic injuries of the triceps muscle. Restoration of elbow extension is an integral part of the entire upper extremity surgical reconstruction to improve the following daily activities: bringing down an object from above, handwriting, using the hand in the supine position, steering a wheelchair, driving a car, and swimming. The transfer of the posterior head of the deltoid muscle to triceps (Moberg procedure) and the transfer of the biceps to triceps (Friedenberg procedure) are previously described procedures for the functional res…

Tennis elbow--outcome following the Garden procedure: a retrospective study.

PURPOSE: From 1993 to 1999, 17 patients (18 elbows) underwent the Garden procedure for clinically diagnosed tennis elbow. Non-operative treatment was ineffective for these patients. METHODS: The study group consisted of 10 men and 7 women, with a mean age of 49.6 years. Follow-up duration ranged from 18 months to 6 years, with a mean of 37 months. Patients' pain, activity level, and hand grip strength were evaluated postoperatively. RESULTS: 14 (77.8%) elbows had an excellent or good result, 2 elbows had a fair result, and another 2 elbows had a poor result and required revision surgery. Mos…

Surgical approaches for nonneurogenic elbow heterotopic ossification with ulnar neuropathy.

BACKGROUND: Few reports are available concerning elbow heterotopic ossification (HO) and its optimal management in nonneurologic or nonburn patients after repetitive elbow manipulation. The unique anatomic relationship of the ulnar nerve at the elbow renders it rather vulnerable to injury when elbow HO occurs medially or posteromedially and extends into the vicinity of the cubital tunnel. METHODS: A total of 16 consecutive patients without neurologic injury were diagnosed with heterotopic ossification formation in the elbow and referred to the Upper Extremity Unit of Chang Gung Memorial Hospi…

A novel method for assessing elbow pain resulting from epicondylitis.

OBJECTIVE: To describe a novel orthopedic test (Polk's test) which can assist the clinician in differentiating between me- dial and lateral epicondylitis, 2 of the most common causes of elbow pain. This test has not been previously described in the literature. CLINICAL FEATURES: The testing procedure described in this paper is easy to learn, simple to perform and may provide the clinician with a quick and effective method of differentiating between lateral and medial epicondylitis. The test also helps to elucidate normal activities of daily living that the patient may unknowingly be performin…

Open arthrolysis for functional restoration in post-traumatic elbow stiffness.

A prospective study of nine patients who had elbow stiffness after trauma and had open arthrolysis to restore function is presented: seven were children aged 6-10, two were adults--six males and three females. Five patients had supracondylar fracture as the initial lesion, one had posterior dislocation of the elbow and three had elbow sprains. They were all previously treated by native bonesetters and presented with elbow stiffness and fixed extension of the elbow. All had open arthrolysis using a posterior approach. The flexion range and loss of extension were measured pre-operatively and at …

Posterior elbow dislocation with associated vascular injury after blunt trauma.

BACKGROUND: Injury of the brachial artery is a rare (5-13%) but serious complication after closed elbow dislocation without associated fractures. METHOD: Retrospective analysis of long-term results (mean, 4.1 years) in four patients. RESULTS: All patients underwent emergency repair of the arterial injury within 2.5 hours. In three patients, a reversed saphenous vein graft was used; in one patient the artery was sutured. This latter patient needed another operation with interposition of a reversed saphenous graft, because the primary anastomosis occluded. The capsule and the collateral ligame…

Upper extremity radionuclide bone imaging: shoulder, arm, elbow, and forearm.

Along with resurgence of physical activity during these fitness conscious times, there is increasing participation by people of all ages in sporting activities. Enrollment in organized and recreational sports among young adults and children has increased with as many as 30% of adolescents participating in competitive high school athletics. Despite improvements in equipment, the number of sports-related injuries presenting for medical attention has increased. Injuries to the upper extremities account for more than 25% of all sports-related injuries, but receive disproportionately less attention

Elbow dislocation with ipsilateral radial shaft fracture. An unusual outcome.

The authors describe an uncommon injury of the upper limb, namely, dislocation of the elbow with fracture of the ipsilateral radial shaft, in a young man. After closed reduction of the joint, the radial head was found to be subluxated, and when a nonanatomic internal fixation of the radial shaft was undertaken, the radial head became dislocated. Repeat operation with removal of the implants and internal fixation with correction of rotation and angulation at the fracture site and reconstruction of the annular ligament restored normal anatomy.

Cortisone injection with anesthetic additives for radial epicondylalgia (tennis elbow).

In a prospective, randomized, double-blind study of radial epicondylalgia (tennis elbow), 109 patients with an average symptom duration of 8 months were considered for treatment with a single 1-mL injection of the steroid triamcinolone combined with either lidocaine or bupivacaine. The patients received clinical examinations at regular intervals for 1 year, and followup included visual analog pain scale and questionnaires. The 2 groups were comparable and for many factors distributed completely evenly with respect to gender, age, symptom duration, side dominance, type of pain onset, earlier tr…

Elbow flexion contractures treated with serial casts and conservative therapy.

We report on the technique of serial casts to successfully reverse elbow flexion contractures in three patients. Patients considered for this technique had a traumatic injury to the elbow that resulted in residual soft tissue restrictions; their mean flexion deformity was 44 degrees, which had been present for an average of 6 months. Treatment consisted of traditional therapy methods to obtain initial stretch and elongation of tissue followed by application of a cylindrical fiberglass cast in the position of maximal elbow extension. The cast was worn for 3 to 5 days, then removed, and the enti…

Extrahepatic bile duct angulation by T-tube: the elbow sign.

The normal shape of the extrahepatic bile ducts approximates a straight line. The cholangiograms of 50 patients with indwelling T-tube catheters were retrospectively reviewed between 4 and 14 weeks postcholecystectomy. We observed a lateral distortion in the shape of the bile ducts in 35 patients (70%) such that an angle measured between the proximal and distal parts of the duct, centered at the site of T-tube insertion, decreased to between 60 degrees and 158 degrees. We have called this observation the "elbow sign," which to our knowledge has not been previously described and appears to be o…

The importance of elbow aspiration when treating radial head fractures.

A prospective study of elbow aspiration was performed to assess the effect of this procedure on pain relief and function following Type I and II fractures of the radial head. Aspiration of the haemarthrosis produced dramatic pain relief in 77% of elbows thus treated. Earlier return of function was also noted for these elbows compared to controls. We recommend elbow aspiration for patients with these fractures to both relieve pain and restore early function.

[Surgical treatment of neglected elbow dislocations. Report of 81 cases].

The neglected dislocations of the elbow are rare in Europe. In Africa, amongst 81 operated cases, during 10 years, 52 were seen again, with a follow-up between 4 months and 7 years. From this group, the authors are trying to define the place of open reductions and resections of the lower end of humerus. The surgical indications were set up in accordance with functional discomfort. The results of open reductions and those of resections were satisfactory, respectively in 72 per cent and 74 per cent of the cases in flexion-extension movement, and 31 per cent and 41 per cent in the pronosupination…

Brachial artery damage accompanying closed posterior dislocation of the elbow.

Four cases of arterial damage resulting from closed posterior dislocation of the elbow are described. Two of these have been successfully treated by conservative methods and two by vein graft to the damaged artery. Sixteen previous cases are detailed and the management of this uncommon complication of dislocated elbow is discussed.
